Volume A – Policies and ProceduresApproved AbbreviationsCanterbury DHBChristchurch Hospital1 Approved Abbreviations and Symbols – Christchurch HospitalPolicy StatementThis policy lists the current and approved abbreviations acceptable withinChristchurch Hospital.ScopeThis policy applies to all Christchurch Hospital staff.Associated DocumentsMinistry of Health NZ Standard NZS 8153:2002 Health RecordsGeneral StatementsThe use of abbreviations should be minimal and avoided whenever possible.Medication names must not be abbreviated, they must be stated in fullAbbreviations must not be used on consent formsApothecary symbols must not be used, use the metric system.Abbreviations should be avoided:in the writing of patient clinical notes, especially when doing an initialassessmentgiving instructions to other people, such as discharge summaries,district nursing referralson letter/reports provided to outside partiesEach abbreviation or symbol should have only one meaning and where thereis more than one meaning abbreviations or symbols should not be used.Do not substitute abbreviations for common English terms.There are some abbreviations that have become traditionally accepted foruse.
