The Regional Transit Connection (RTC)Discount ID Card is available to personswith qualifying disabilities. You may usethe card as proof of eligibility to receivediscount fares on fixed-route, rail and ferrysystems throughout the San Francisco BayArea. The RTC Program does not apply toparatransit services.For specific fare information in your area,contact your local transit system.Applications for the RTC Discount ID Cardare accepted at San Francisco Bay Areatransit agency locations and forwardedto a centralized processing office, whereapplication information is verified.Qualified applicants are then mailed thenew RTC Discount ID card.If you are eligible for the RTC DiscountID Card, you will receive a card that isClipper-ready and provides access toClipper features. For more information onhow to use these features, please contactyour transit agency or Clipper CustomerService at (877) 878-8883.IMPORTANT NOTE: The RTC Programno longer issues cards to seniors (age 65and over) unless they are traveling with anattendant. The Senior Clipper card offersthe same features and discounts, with noapplication fee, and does not expire. Youcan obtain a Senior Clipper card by mail,email, and fax, or at your local transitagency. APPLICATION STEPS1. Apply at your local transit agency (referto pages 16 through 19 for locationsand hours of operation). Complete an application form foreither “Basic” or “Medical” eligibility(described below). Incompleteapplications will not be accepted. Bring the original application withyou. Do not mail it in. Present original valid governmentissued photo identification - such asa current driver’s license, CaliforniaIdentification Card, passport,Alien Registration Card, MilitaryIdentification Card or MilitaryDependent Card. (Photo copies arenot accepted) Pay the appropriate fee: 3.00 new, renewal, or visitor 5.00 replacementThese fees are non-refundable. For “Basic” eligibility, you must alsobring the applicable documentationlisted on page 7-8.2. Call your local transit agency if youneed a form sent to you.3. Your photograph will be taken at thetransit agency when you submit yourapplication. Your eligibility will beverified later, and if you are eligible,your RTC Discount ID card will be6mailed to you. If you haven’t receivedyour card or a request for moreinformation within 21 days, contactthe transit agency where you submittedyour application.When you receive your RTC Discount IDcard, you will be immediately eligible forreduced fares on all participating transitsystems. No additional proof of eligibilityis required, but you must show the RTCDiscount ID card at the time you pay the fare.ELIGIBILITYThere are two types of eligibility forthe RTC card: 1) Basic Eligibility, or 2)Medical eligibility. If you have a Medicarecard, we suggest that you submit a “BasicEligibility” application.Basic EligibilityIf you are eligible under one of the “Basic”categories noted below, you may applyunder that category. “Basic” eligibilitydoes not require medical certification. Toqualify for Basic Eligibility, bring originaldocuments of one of the following withyour application: Medicare card, the federally-issued red,white, and blue card – only for thoseunder 65 years of age. (NOTE: Medi-Calis not accepted for eligibility.)7

California Department of Motor Vehicles(DMV) Placard Registration DisabledPerson Parking receipt. If you bringthe blue or red placard, you must alsobring the registration receipt. If you havedisabled license plates (DP) instead ofa placard, you must submit a “MedicalEligibility” form. Other California transit agency card that isequivalent to the RTC Discount ID card. Proof of Veterans Disability-- A copy of yourService Connected Disability ID card andyour VA Certification letter demonstratinga disability rating for aid and attendance,or for a service-connected disability witha rating level of 50% or higher. Pleasenote that it may take more than severalmonths to verify your application with theVA due to their current work volume. Fora quicker turnaround, it is recommendedthat you submit a “Medical Certification”application instead.Medical Eligibility1) to authorize your doctor to releaseinformation, and 2) to indicate youracceptance of RTC Program terms. If yourapplication is not signed in both places, itcannot be processed and will be returnedto you.Bring the Medical Certification formto your “Certifying Professional.” TheCertifying Professional must complete theform and mail the green copy to the RTCMedical Verifier at the address shownon the form. Your application will beconsidered incomplete unless the certifyingprofessional has returned the form.Keep the other copies and bring them withyou when you have your photo taken atyour transit agency. It is recommended thatyou have the form completed before goingto the transit agency. If your doctor has notcompleted his/her portion of the form, theprocess could take more than 21 days.Photocopies of the medical application arenot accepted under any circumstances.To qualify under “Medical Eligibility,” youmust be certified by one of the licensedprofessionals described below. To qualifyfor Medical Eligibility:Complete the appropriate sections of theMedical Certification form. Forms areavailable from your transit agency.Sign the application in two places:89

OTHER WAYS TO GETDISCOUNT FARESIf you are disabled, you may also pay cashdiscount fares without the RTC/Clippercard, but you must show your CaliforniaState ID AND one of the following uponboarding: Medicare card (not Medi-Cal) A current Disabled Person ParkingPlacard Receipt from the CaliforniaDMV Valid transit discount ID card issued byanother transit agency––if it is equivalentto the RTC Discount ID Card.TRAVELING WITH AN ATTENDANTOR SERVICE ANIMALAttendantsYour medical professional can checkthe box on the Medical Certificationapplication, indicating your need foran attendant. The medical verifier willcontact your doctor for confirmation.1. You may use your card with theattendant symbol in order to paydiscounted cash fares for the attendant.2. Or, apply for an “Attendant-Limited”Clipper card at your transit agency. A 3 processing fee applies. The cardcan be loaded with cash or passes to10be used by an attendant when travelingwith you. The attendant must payregular fare at all other times. Use of anattendant card may be monitored.Service AnimalA service animal is any guide dog, signaldog, or other animal individually trained towork or perform tasks for an individual witha disability. Certification or documentationis not required for your service animal.However, the service animal must beunder your control at all times. The RTCDiscount ID Card does not contain anyspecial marking for a service animal, and aphotograph of your animal is not requiredfor your RTC Discount ID card.REPLACING LOST OR STOLEN CARDSFirst, check with your local transit agencyto see if the card has been returned. If so,you may reclaim your old card at no cost.Important: Call Clipper Customer Serviceat 877-878-8883 to block your card toensure that no one can use the money thatyou may have stored on your card.To obtain a replacement card: Go to your nearest transit office and paya 5 replacement fee, or download theapplication at www.transit.511.org/rtc, or11

Call your local transit agency to havethem mail you the form. Mail theapplication or a written request fora replacement directly to the RTCMedical Verifier (P.O. Box 70040,Oakland CA 94612-0040). Be sure toinclude your name (as it appears onyour card), your RTC identificationnumber (if you have it), date of birthand current address with a writtenrequest. You must include payment ( 5)in the form of a 1) personal printedcheck or 2) cashier’s check. Do notsend cash.You will receive a replacement card bymail within 7-10 days. Temporary cardsare not available.Be sure to take good care of your RTCDiscount ID Card! Your card will only bereplaced a maximum of three times withina 12-month period at a cost of 5.00 eachtime. You will not be issued another cardwithin that period. However, if you feelthat your disability is such that you areprone to losing personal items, you maysubmit a “Disability Justification” formfor additional replacements. This formcan be requested at the time of your 4threplacement application. Your doctormust complete this form and mail itdirectly to the RTC Medical Verifier.You will also be charged the 5replacement fee if the card is damageddue to poor care on your part.12CARD RENEWALSRTC Discount cards are valid for a maximum of five years. Your card may be validfor less if 1) your medical professionaldetermines that your disability is of ashorter duration, or 2) your eligibility is tiedto a qualifying document that is valid for ashorter period such as a DMV placard (2years), or 3) you used a card from anothertransit agency as proof of eligibility. Youmust renew your card at expiration even ifyour disability is permanent.If you have an RTC Discount ID cardbased on a permanent disability, you willreceive a renewal notice two monthsprior to the expiration date on your cardat the address on file with RTC. It is yourresponsibility to ensure that the transitagency has your current address. If youdo not receive a mail reminder, call yourtransit agency and request a renewalapplication.Bring your completed application to yourtransit agency or mail it directly to theRTC Medical Verifier (P.O. Box 70040,Oakland CA 94612-0040). The fee is 3.Payment by mail must be in the form of a1) personal or 2) cashier’s check. Do notsend cash.Special Instructions for those using DMVDisabled Person Parking Placards:13

Your renewal application cannot beprocessed until your new DMV Placardnumber is submitted and verified. Ifapplying in person, bring the DisabledPlacard registration issued by the DMVwith you. If applying by mail, send aphotocopy of the receipt with yourapplication.CARDS FOR VISITORSIf you have a discount card from a transitsystem in another city, you may be ableto use it in the Bay Area. If you plan totake transit frequently during your stayyou might consider applying for the RTCDiscount ID card. The RTC Discount IDcard is more familiar to operators andoffers the convenience of a smart card topay fares.California Transit Systems: You are eligiblefor discount fares when you show a validtransit discount ID card from a Californiatransit system outside of the nine-countySan Francisco Bay Area. Alternatively, youmay apply for a one-time courtesy RTCDiscount ID card for the same period thatyour California transit agency ID card isvalid, or up to three years, whichever isless. A 3 processing fee applies, and youwill need to submit a complete medical orbasic application when your courtesy cardexpires.operators may not accept out-of-state IDcards. If your stay is less than 3 months,you may want to apply for a courtesycard, using your other Transit ID as proofof eligibility. You would submit a “BasicEligibility” application, show your out ofstate ID card, pay the 3 processing fee,and receive a three-month temporary RTCDiscount card. This option is available onetime only. INFORMATION FOR MEDICALCERTIFIERSThe purpose of the RTC Discount IDCard Program is to ensure that onlyeligible individuals receive fare discountsas mandated by state and federal law.An individual’s eligibility is based ontheir inability to use fixed-route transit(i.e., regular accessible buses, light rail,commuter rail or BART) without specialfacilities, planning or design. [49 US §1608 (c)(4), Section 99206.5, CA PUC]We are requesting your help to ensurethat recipients meet the eligibility criteria.If you have questions, please contact theRTC Medical Verifier Office at510-208-0200. You may certify eligibilityonly in the categories related to the field ofpractice in which you are licensed in theState of California to diagnose: Licensed physicians with an M.D.or D.O. degree, licensed physician’sassistants and nurse practitioners maycertify in all categories in which theyare licensed to diagnose Licensed chiropractors, categories1, 2, 3 and 4 Licensed podiatrists, disabilitiesinvolving the feet under categories1, 2, 3 and 4 Licensed optometrists (OPT), category 92021

When completing theapplication form, please indicate a SectionCode corresponding to the descriptionsprovided.22ELIGIBILITY CRITERIA SECTION CODESSection 1 – Non-ambulatory DisabilitiesImpairments that, regardless of cause,require individuals to use a wheelchairfor mobility.Section 2 – Mobility AidsImpairments that cause individuals towalk with significant difficulty includingrequiring use of a leg brace, cane, walkeror crutches.Section 3 – Musculo-Skeletal Impairment(Including Arthritis)Musculo-skeletal impairment suchas muscular dystrophy, osteogenesisimperfecta or arthritis of Functional ClassIII or anatomical Stage III. Individual hassignificant mobility impairment.Section 4 – AmputationPersons who suffer amputation of, oranatomical deformity of (i.e., loss of majorfunction due to degenerative changesassociated with vascular or neurologicaldeficits, traumatic loss of muscle mass ortendons and x-ray evidence of bony orfibrous ankylosis at an unfavorable angle,joint subluxation or instability): (a) Bothhands; or (b) one hand and one foot; or (c)amputation of lower extremity at or abovethe tarsal region (one or both legs).23

Section 5 - Cerebrovascular Accident(Stroke)With one of the following: (a) pseudobulbarpalsy; or (b) functional motor deficit;or (c) ataxia affecting two extremitiessubstantiated by appropriate cerebellarsigns or proprioceptive loss post 4 monthsSection 6 - Pulmonary IllsRespiratory impairments of Class 3 (FVCbetween 51 and 59% of predicted, or FEVbetween 41 and 59% of predicted); orClass 4 (FVC less than or equal to 50% ofpredicted, or FEV less than or equal to 40%of predicted.Section 7 - Cardiac IllsCardiovascular impairments of functionalClass III: Individuals with cardiac diseaseresulting in marked limitation of physicalactivity. They are comfortable at rest; lessthan ordinary physical activity causesfatigue, palpitation, dyspnea or anginalpain. (e.g., inability to walk one or morelevel blocks or climbing a flight of ordinarystairs.) Cardiovascular impairments offunctional Class IV: Individuals with cardiacdisease resulting in inability to carry outany physical activity without discomfort.Symptoms of cardiac insufficiency or ofthe anginal syndrome may be present evenat rest. If physical activity is undertaken,discomfort is increased.Section 8 - DialysisIndividuals whose disability requires theuse of a kidney dialysis machine.24Section 9 – Sight DisabilitiesThose individuals whose vision in the bettereye (after correction) is 20/200 or less;or those individuals whose visual field iscontracted (tunnel vision) to 10 or lessfrom point of fixation or widest diametersubtends an angle no greater than 20 and individuals who are unable to readinformation signs or symbols for other thanlanguage reasons.Section 10 – Hearing DisabilitiesDeafness or hearing incapacity that makesperson unable to communicate or hearwarning signals including those personswhose hearing loss is 70 dba or greater inthe 500, 1000, 2000 Hz ranges.Section 11 – Disabilities of IncoordinationIndividuals suffering faulty coordinationor palsy from brain spinal or peripheralnerve injury, functional motor deficit inany two limbs or manifestations whichsignificantly reduce mobility, coordinationor perceptivenessSection 12 - Intellectual DisabilityIntellectual Disability is a disorder thatfeatures concomitant deficits in intellectualfunctions and adaptive functioning thatadversely impacts one or more aspectsof daily living, such as communication,socialization, academic achievement andindependent living. Significant delaysin intellectual functions, which includereasoning, comprehension, judgment,problem solving, working memory and25

others, must be confirmed by standardizedtests and clinical judgment. The significance of the disability, however, isdetermined by the extent of the limitationsin adaptive functioning in the conceptual,social and practical domains. A deficitin adaptive functioning must be presentin one or more of these domains andevidenced across multiple environments.The onset of these deficits must be seenduring the developmental period and maybe a result of a genetic syndrome, headtrauma or acquired illness.Section 13 – Cerebral PalsyA neurological condition that appears ininfancy or early childhood andpermanently affects body movement,muscle coordination, and balance,and which primarily causes physicalimpairment involving limitation or loss offunction and mobility. They experiencedifficulty with muscle coordination, musclecontrol, muscle tone, reflexes, balanceor posture. They may have difficulty withfine or gross motor skills. Individuals withCerebral Palsy may have associative andco-mitigating conditions that also imposeadditional challenges, such as a learningimpairment, seizures, and vision or hearingloss.Section 14 – Epilepsy (Convulsive Disorder)A clinical disorder involving impairment ofconsciousness, characterized by seizures(e.g., generalized, complex partial,major motor, grand mal, petit mal or26psychomotor), occurring more frequentlythan once a month in spite of prescribedtreatment, with (a) diurnal episodes (lossof consciousness and convulsive seizure);(b) nocturnal episodes which show residualinterfering with activity during the day;or (c) a disorder involving absence (petitmal) or mild partial (psychomotor) seizuresoccurring more frequently than once perweek in spite of prescribed treatmentwith: 1) Alternation of awareness or loss ofconsciousness; and 2) Transient post-ictalmanifestations of conventional or antisocialbehavior. Persons exhibiting seizure-freecontrol for a continuous period of morethan six months duration are not includedin the statement of Epilepsy defined in thissection.Section 15 – Autism Spectrum DisorderAutism Spectrum Disorder is characterizedby deficits in verbal and nonverbalcommunication abilities and socialinteraction skills, coupled with thepresence of restricted, repetitive patternsof behavior, interest or activities, whichsignificantly impact the quality of social,educational, occupational, and/or adaptivefunctioning. Symptoms typically manifestearly in the developmental period, but mayonly become fully apparent with increasedsocial expectations. The range and severityof symptoms can vary greatly, producingunique, individual profiles of strengthsand needs. To qualify for discount fareeligibility, an individual must minimally27

demonstrate the need for a substantiallevel of support to address either socialcommunication deficits or restricted,repetitive behaviors.Section 16 – Neurological ImpairmentA syndrome characterized by learning,perception and/or behavioral disorders ofan individual whose IQ is not less thantwo standard deviations below the norm.These characteristics exist as a result ofbrain dysfunctions, neurologic disorderor any damage to the central nervoussystem, whether due to genetic, hereditary,accident or illness factors. This sectionincludes persons with severe gait problemswho are restricted in mobility.Section 17 – Mental DisordersMental Disorders include a broad rangeof mental conditions that affect mood,thought, and behavior, and significantlyimpact one or more life activities. ADSM-5 diagnosis in one of the followingcategories is required for eligibility:Schizophrenia Spectrum and OtherPsychotic Disorders, Bipolar and RelatedDisorders, Depressive Disorders, Traumaand Stressor-Related Disorders, DissociativeDisorders, Somatic Symptoms and RelatedDisorders, and Neurocognitive Disorders.The diagnosis must be at a Class 3 to 5level and a moderate impairment is theminimum level of severity necessary toqualify under this section. Please note thatnot all diagnoses within these categorieswill qualify for eligibility. For example,28disorders in remission and “Unspecified”diagnoses are specifically excluded fromdiscount fare eligibility. Additionally,applicants who have a Substance-Relatedor Addictive Disorder as a primary disabilitywill not qualify for this program.Section 18 – Chronic ProgressiveDebilitating DisordersIndividuals who experience chronic andprogressive debilitating diseases that arecharacterized by constitutional symptomssuch as fatigue, weakness, weight loss, painand changes in mental status that, takentogether, interfere in the activities of dailyliving and significantly impair mobility.Examples include: (a) Progressive,uncontrollable malignancies (i.e. terminalmalignancies or malignancies being treatedwith aggressive radiation or chemotherapy);(b) Advanced connective tissue diseases(i.e., advance states of disseminated lupuserythematosus, scleroderma or polyarteritisnodosa); (c) Symptomatic HIV infection(i.e., AIDS or ARC) in CDC-defined clinicalcategories B and C.Section 19 – Multiple ImpairmentsThis category may include, but notbe limited to, persons disabled by thecombined effects of more than oneimpairment. The individual impairmentsthemselves may not be severe enough toqualify as a Transit Dysfunction; however,the combined effects of the disabilities mayqualify the individual for the program.29
