E.CHE.LON --------------------0Chapter 2 -Positioning the PatientIn o rder to acquire an image of a particular anatomical reg io n, the patient mustbe properly positioned on the Echelon MRl System patient table using one ormore pads, then fitted with the appropriate coil. The patient table is equ ippedwith lasers to guide you in positioni ng the patient outside the magnetic field.Once you have centered the region to be studied, you can move the patient intothe magnetic field, w here the imaging w ill be perfom1ed.General instructions about the Eche lon MRI System control panels and theproper use of the imaging hardware are provided in this chapte r. Patientpos itioning terms, operation of the patient alert bulb, and patient tablepreparation directio ns are also included.Gantry and Patient Table DataThe gantry is designed w ith a short bore and w ide aperture to create an invitingpatient env ironment w ithout compromising c linical utility. Whi le the large,high-capacity patient table comfortably supports a broad range of body types, the limits listed below shou ld be followed.QS4-52789v2oPatie nt aperture (diameter): 24 in. E.CHE.LON""' MRI System Patient Positioning and Coils ManualTerminologyThe following terminology is used throughout this manual to ensure clear andexplicit instructions: References to the foot of the patient table indicate the end of the patient tablefarthest from the magnetic field. References to the head ofthe patient table indicate the end of the patient tablenearest to the magnetic field. References to the right or left side of the patient table indicate the sides as ifyou were standing at the foot of the patient table looking at the magnet.Patient SafetyWhile you are positioning the patient on the patient table, and whi le the patientis in the magnetic field, you must constantly monitor the patient's safety. Makesure that the receiver coil cable remains away from the patient and that thepatient does not make contact with the gantry while the patient table is moving.WarningDo not allow persons with pacemakers or other implanted devices to enter theControlled Access Area (within the 5-gauss field). The strong magnetic fieldcould cause such devices to malfunction and pose a risk of serious injury ordeath to the person.WarningDo not introduce ferromagnetic materials (such as tools, scissors, gurneys, cleaning equipment, and simi lar items) into the magnetic field. The strongmagnetic field can cause these materials to act like projectiles, drawing theminto the system. Failure to comply could result in death or serious injury.WarningPatients requiring emergency treatment or assistance must be removed from theControlled Access Area. Their proximity to the magnet could prevent the safeand effective use of electronic and/or metallic emergency medical equipment.Failure to comply could result in death or serious injury.2-2Copyright @2013 by Hitachi Medical Systems America, Inc. Positioning the Patient0Patient Alert BulbAfter positioning the patient on the patie nt table and fitting him or her w ith theappropriate coil and pad(s), you should offer use of the patient a lert bulb. Whensqueezed, this dev ice beeps to a lert you that the patient needs assistance during aprocedure. Always offer this device to patients, especially those who havehearing impairments. The patient alert bulb contains dry natural rubber, which may be problematic forpatients with latex a llergies. For detailed safety info rmation, refer to Chapter I, Safety, in the Echelon MRISystem Reference Manual.QS4-52789v22·3

0Positioning the PatientPreparing the Patient TablePatient table preparation involves selecting and plac ing the table pads and theradio frequency (RF) coi l(s). The appropriate pads and coil(s) are essential toacquire optimal images. Fo llow these steps when preparing the patie nt table:Note:The patient table must be movedcompletely out of the magnetbefore its height can be adjusted.I.Press and hold the O UT/ DOWN button to move the patient table complete lyo ut of the mag netic field, then lower the patient table to a positio n where thepatient can easily sit on it.2. Select the appropriate table pads and RF receiver coi l and positio n them onthe patient table. Your c hoice of pads will be affected by three factors: thea natom ical regio n to be imaged, the s ize of the patient, and the coil you planto use.Positioning the Patient for an Examination in the MagnetPatient positioning invo lves using a variety of table and patient positioningpads, placing the RF coil(s) on or around the patient in a variety of ways, andproperly centering the anatomical region to be imaged. Although each patientand examination are different, the basic patie nt positioning procedures remainthe same.Various table pads and positioning pads and spo nges are avai lable for use withthe Eche lon MRI System. In add itio n to providing some degree of comfo rtfor the patient, the pads help to support and position both the patient and thecoil. Pads can be used in a variety of combinations, depending upon the regionof interest, the coil you plan to use, and the s ize of the patient. The detai ledpositioning instructions in the next chapter w ill suggest the appropriate pads touse under many circumstances.QS4-52789v22·7

ECHELON""MRI System Patient Positioning and Coils ManualFor patient comf01t, use the knee cushion with virtually all scans when thepatient is supine (except hip imaging). When a patient is prone, Hitachirecommends placing a pillow under the ankles.Do not use too many pads, as this might raise the patient too high in the staticfield.ACautionSecure long hair before moving the patient onto the imaging table. The patient'shair could become caught in the patient table mechanism. Failure to complycould cause patient injury.1. Ask the patient to sit on the side ofthe patient table and then lie down.2. Press and hold the IN/UP button until the patient table reaches the upperlimit.3. Position the anatomical region in the center of the selected RF coil. Addpositioning pads and sponges as needed.Make sure that all coils placed on the patient table are plugged into a tableconnector. Disco1mected coils could cause patient injury or be damaged ifexposed to RF energy. Remove any coils not being used for the selected scanprotocols.ACautionSome coils consist of two pa1ts that lock together. Be sure that the patient's hair, skin, clothing, and/or other foreign material does not get caught between the twocoil parts.WarningInstruct the patient to close his or her eyes when the lasers are on. Do not stareinto the beam, as eye injury may result.2-8Copyright 2013 by Hitachi Medical Systems America, Inc. All rights reserved.

Positioning the PatientNote:Many of the Echelon RF receivercoils have a transaxial mark toassist in centering the anatomicalregion.4.0Press a nd ho ld the IN /UP butto n to center the transaxial a lignment line ofthe laser to the patient's anatomical region that is to be imaged.5. Connect the receiver coil cable(s) to the patient table connector(s). A total ofsix table co nnectors are available on the patient table--four at the head andtwo at the foot. When making multiple connections, avoid crossing the coilcables. If possible, keep the cables fu lly extended and running parallel toeach other.Avoid loops in the cable. Place any excess cable between the patient table andthe pads. Prevent the cable from touching the patient or any pa1t of the magnetbore.6.Press and ho ld the SET button on the gantry control panel until the patienttable advances the patient to the magnet's isocenter (the center of themagnetic field) and stops. The imag ing region centered under the lasersis now in the center of the magnetic field. The long itudinal move counterdisplay on the gantry will be zero. When moving the patient into the magnetic field, do not allow the receiver coilcable to loop, wi nd around the patient's body, or catch o n the patient table.QS4-52789v22·9

Therefore, multi -echelon policy models take into account the effect that a given echelon's inventory decisions have on the other supply chain echelons (Eruguz et al., 2016). From the concept of echelon stock, a significant number of authors have used different operating parameters and

Companies have different approaches to Multi-Echelon Inventory Optimization (MEIO) based on many business factors. More senior initiatives have taken a more strategic (cross-echelon) approach rather than a functional or tactical approach. Of course, both approaches are valid and have trade-offs in terms of the work involved as well as the
