RFP #XXXXX / Project #XXXSection 01010Issue Date:SUMMARY OF WORKAX Farm Equipment Removal1.0Background/IntroductionThe River Protection Project (RPP) mission is to safely store, retrieve, treat,immobilize, and dispose of the Hanford Site tank waste. The four tanks in AXFarm have been designated for retrieval. It is estimated that this will require theremoval of approximately 32 pieces of existing in-tank equipment to make roomfor the eventual installation of the new equipment required for retrieval.Hereinafter, the construction subcontractor shall be referred to as“Subcontractor” and Washington River Protection Solutions LLC (WRPS) shall bereferred to as “Company.”2.0Project ObjectiveThis project will be to provide Tank Farm qualified construction subcontractorpersonnel to perform equipment removal at AX Farm. All necessary equipmentwill be removed from AX-102 and AX-104 pits and risers. These pits and riserswill then be used in future design and work planning for the installation of newretrieval equipment. This project will also install new access gates and removeBuilding 2707-AX/EB, Building AX80, Building 801A, Building 801B, and existingAX farm lighting. Qualified construction subcontractor personnel will include, butis not limited to, Field Work Supervisors (FWS), Safety Representative, andapplicable construction craft workers (laborers, pipefitters, electricians,equipment operators, etc.).3.0Project DescriptionThe Subcontractor will be responsible for the provision and management of alllabor, material, equipment and services (including supervision, planning support,safety, scheduling, record keeping, documentation, means and methods)required to safely and compliantly complete the work described in this Statementof Work (SOW) and subsequent individual requests, as may be identified duringthe performance of the work.This phase of the Construction Project includes work planning and fieldexecution.The work area is a radiological area and workers will be required to be trained forradiological work.Page 3 of 134

Additional design media, as the need arises,will be provided during the project duration.See Design Document List (DDL) (Appendix A), for the design documentsapplicable to this project scope.The Subcontractor shall verify that all design documents listed have beenreceived. The Subcontractor shall verify that the revisions noted on thedesign documents listed have also been received; that all pages for eachdocument have been received; and that the documents are legible. If theSubcontractor identifies any discrepancies, notify the Company’sSubcontract Administrator in writing.4.2Scope of WorkSee the Work Breakdown Structure (WBS) (Appendix B) for the workscope applicable to this project.The Subcontractor shall review the WBS in conjunction with the designdocuments and assure that the work scope is clear and understandable.If the Subcontractor has any questions or needs clarification as to thework scope, notify the Company’s Subcontract Administrator in writing.4.3CostA Cost Summary Index (CSI) is provided in Appendix C. TheSubcontractor shall submit their proposed cost in accordance with theCSI. Proposals shall be in sufficient detail to permit an analysis of alllabor, material, and equipment costs, and shall cover all work involved inthe proposal. Proposals shall identify, by itemization, any requesteddeviations from the rates as published in the Hanford Site StabilizationAgreement Appendix A.Proposed costs for labor shall be supplemented by identifying which seller(e.g. Subcontractor, Lower-tier A, Lower-tier B) is providing whichdiscipline(s). Make note of any over-lapping disciplines (e.g. qualitycontrol supplied by both the Subcontractor and Lower-tier A).Page 4 of 134

RFP #XXXXX / Project #XXXSection 01010Issue Date:SUMMARY OF WORKAX Farm Equipment RemovalProposed costs for material shall be supplemented by itemized breakoutsto include nomenclature and unit rates for this proposal.Proposed costs for equipment rates shall be supplemented by delineatingoperating time and stand-by time used for this proposal.If the Subcontractor has any questions or needs clarification as to the costsummary index, notify the Company’s Subcontract Administrator in writing.4.4ScheduleSee the WBS for specific construction completion date and/or workcurrently scheduled for completion within specific calendar days ofcontract award.The Subcontractor shall provide, with their proposal, construction activitieswith durations, resource loading, and the logic for developing theconstruction schedule. Appendix D has been provided to facilitate thiseffort. Appendix D is to be considered a guide for developing theschedule that accompanies the proposal. The schedule that accompaniesthe proposal shall be in a Gantt format, and may be in Microsoft Project,Excel, or another software program of the Subcontractor’s choosing.The Subcontractor shall coordinate with the Company to develop aresource loaded P6 construction schedule within five (5) days after award.The company will provide P6 software and a scheduler.The Company will identify activities and dates for Company deliverables.The Subcontractor will provide weekly updates of the ConstructionSchedule to the Company. The Company will revise the ConstructionSchedule and provide the updates back to the Subcontractor fordistribution.At the discretion of the Company construction management, weeklyschedule updates include, but are not limited to, the following: Summary of previous week accomplishments.1-week look ahead.2-week look ahead.30-day look ahead.Page 5 of 134

Actual start dates for eachactivity will be determined at the project status meetings.If it is determined that there are significant variances between theSubcontractor’s actual and scheduled progress (i.e., endangeringcompletion of the Subcontractor’s work within the scheduled time), theSubcontractor may be required to prepare and submit a corrective actionplan. The corrective action plan(s) shall be submitted to the Companyidentifying the following: Completion dates for the activity/activities behind schedule.Identify problem areas, anticipated delays and schedule impacts.Describe corrective actions taken or proposed.Radiological Schedule ConsiderationsWork activities inside a radiological area include, but are not limited to thefollowing: Potholing. Trenching. Any work involving open tank risers. Any work inside a tank pit. Any activity within a trench / excavation area (including entering foran inspection). Handling of radioactive materials (e.g. housekeeping within aradiological area).RadCon personnel will need time allotted at the end of each day toperform their paperwork. No work inside a radiological area will be able tobe performed during this time.Allow for one and one-half (1.5) hours each day that no work can beperformed in a radiological area. Work activities outside the radiologicalarea may continue without RadCon support.Page 6 of 134

See Summary of Work Section 01300, Submittals forfurther information.If the Subcontractor has any questions or needs clarification as to theMSL, notify the Company’s Subcontract Administrator in writing.4.6Company Furnished ItemsThe Company will provide, at no cost to the Subcontractor, the following: Location for installation of Subcontractor provided temporaryfacilities. Underground Scanning. Water for dust control and soil compaction. NEC inspection services. QAT and/or IQRPE inspection services, as required. (QATservices will not include Certified Weld Inspector [CWI] or Nondestructive Examination [NDE] services.) Operational support for LO/TO. Waste disposal containers for radiological waste.oGeneral construction debris will be the responsibility of theSubcontractor.Construction aids, such as building a temporary platform to reach boltsthat are too high for the worker to reach from the ground, are implied asSubcontractor furnished to perform fabrications and work activities.Page 7 of 134

RFP #XXXXX / Project #XXXSection 01010Issue Date:SUMMARY OF WORKAX Farm Equipment RemovalIf the Subcontractor has any questions or needs clarification regardingwhat is GFE and what is Subcontractor furnished, notify the Company’sSubcontract Administrator in writing.4.7Work PackagesSection 01040, Communications and Coordination provides informationfor the Subcontractor to estimate cost for attending team planningmeetings, developing work packages and attending Joint Review Group(JRG).At this time, there are XX work packages estimated for this work scopeand are as follows: Ten (10) low risk work packages.Ten (10) medium risk work packages.Twenty (20) high risk work packages.If the Subcontractor has any questions or needs clarification as to workpackages, notify the Company’s Subcontract Administrator in writing.4.8HazardsSee the Preliminary Hazards Analysis (PHA), located in Appendix F, forhazards identified with the scope of work. The Subcontractor shall reviewhazards identified and assure understanding of the hazards as they applyto the work activities. If the Subcontractor has any questions or needsclarification as to the hazards identified, notify the Company’s SubcontractAdministrator in writing.The Subcontractor Safety Representative and Field Work Supervisor(FWS) shall perform joint safety walk downs during each shift. Results ofsafety walk down are to be documented in the Daily Activity andManpower Report (DAMR) and in the work package. Examples of what the Company is looking for include:oThe safety walk down was performed at (time) and no issueswere identified.oThe safety walk down was performed at (time) and one onthe-spot correction was made.Page 8 of 134

RFP #XXXXX / Project #XXXSection 01010Issue Date:SUMMARY OF WORKAX Farm Equipment RemovaloThe safety walk down was performed at (time) and there hasbeen a change in worksite conditions due to (briefexplanation) (e.g. weather, other personnel in area).If the Subcontractor has any questions or needs clarification as to theAppendix F or the reporting requirements for the safety walk downs, notifythe Company’s Subcontract Administrator in writing.4.9TrainingAll Subcontractor employees working on site are required to have HanfordGeneral Employee Training (HGET) as delineated in Section 01100,Badges, Dosimeters, Training and Qualifications.Performance of the work covered by this SOW requires the performingSubcontractor personnel to be trained and qualified to perform work withinthe Radiologically Controlled boundaries of the Tank Farms.All Subcontractor personnel that are required to perform work within theRadiologically Controlled boundaries of the Tank Farms are required tohave current Respirator User Training and Respirator Fit Tests to Qualifyas a Respirator User for Half-Mask, Full Face Mask APR/PAPR andSCBA use.If the Subcontractor has any questions or needs clarification as to thephysical and training requirements, notify the Company’s SubcontractAdministrator in writing.Contact the Company training department for questions or clarifications onequivalency courses.4.10Medical ExamsThe following exams, as delineated in Section 01100, Badges,Dosimeters, Training and Qualifications, are required as noted for accessto areas subject to radiological control. This is in addition to any othermedical screenings (e.g. drug testing, annual physicals) required toperform work at the Hanford site or required by the individualSubcontractor for their employees.Page 9 of 134

Physiological testing and physicals required to Qualify as aRespirator User for Half-Mask, Full Face Mask APR/PAPR andSCBA use.For hands-on work with contaminated components (includingexcavations): 10 Min WBC.Isotopic Pu Bioassay.Isotopic Sr Bioassay.If the Subcontractor has any questions or needs clarification as to themedical exam requirements, notify the Company’s SubcontractAdministrator in writing.4.11Work HoursSubcontractor shall identify the work week schedule and hours forperformance of project work in their proposal.If the Subcontractor has any questions or needs clarification as to theworking hours, notify the Company’s Subcontract Administrator in writing.4.12Quality AssuranceAll work performed by design media is classified as GeneralService/Quality Level 3 (Enhanced Quality). See Section 01400,Quality Assurance for further information.Subcontractor shall be responsible for performing quality workmanshipand shall conduct the quality control measures necessary to ensure workconforms to drawings and specifications.All items are subject to inspection at the Subcontractor's facility orSubcontractor's lower-tier subcontractor’s facility by a Company qualityassurance/quality control representative. The Subcontractor shall notifythe Company at least five (5) working days in advance of the time itemswill reach any inspection hold point established by the Company.Page 10 of 134

RFP #XXXXX / Project #XXXSection 01010Issue Date:SUMMARY OF WORKAX Farm Equipment RemovalSubcontractor personnel performing weld inspections shall be certified asa Certified Weld Inspector (CWI) in accordance with the requirementsspecified in AWS QC-1.The following visual weld inspection qualification documentation shall besubmitted prior to the start of fabrication and approval shall be obtainedfrom the Company prior to start of fabrication. Current AWS CWI certificates. Current and valid visual acuity examination. The examination mustbe performed annually. Visual weld inspection procedures.Non-Destructive Examination (NDE) personnel shall be qualified andcertified in accordance with the recommended guidelines of the AmericanSociety of Nondestructive Testing's (ASNT) SNT-TC-1A-2001, unlessotherwise specified.The following NDE qualification documentation shall be submitted prior tothe start of fabrication and approval shall be obtained from the Companyprior to start of fabrication. NDE qualification and certification procedures. Personnel Level I, II, and III qualifications and certifications whichinclude objective evidence of NDE training, formal education,examinations, experience, date of hire, and current eyeexamination. NDE method/examination procedures that are in accordance withthe applicable codes/standards specified in procurementdocuments.All NDE reports and radiographs shall be traceable to the item examined,include all essential examination parameters, and signed and dated by theNDE examiner. All NDE reports and radiographs shall accompany orprecede shipment of material. Radiographs and radiographic techniqueand examination reports shall be subject to approval by the Company priorto shipment of completed items.Page 11 of 134

The Subcontractor willprovide information, if needed, to obtain permits and agency approvals.Environmental protection controls will be incorporated into the WorkPackage.4.15ReworkRework due to the Subcontractor (or their lower-tier) supplied materials,equipment, engineering, or workmanship will be the responsibility of, andat the expense of, the Subcontractor.Rework due to the Company supplied materials, equipment, engineering,or workmanship will be the responsibility of, and at the expense of, theCompany.4.16Erosion ControlThe Subcontractor shall provide controls in areas where potentialrain/runoff will affect the construction progress, such as preventingtrenches from collapsing.4.17Subcontractor QualificationsAll Subcontractors shall have performed similar work and providepersonnel with qualification commensurate with their work assignments.Page 12 of 134

RFP #XXXXX / Project #XXXSection 01010Issue Date:SUMMARY OF WORKAX Farm Equipment RemovalIf the Subcontractor has any questions or needs clarification as to thequalification requirements, notify the Company’s SubcontractAdministrator in writing.Field Work Supervisor (FWS)The Subcontractor shall submit, with their proposal, the primary FWS andalternate FWS(s) qualifications (resume) and the appropriate training inSection 01100, Subsection 3.4, Construction Field Work Supervisor(FWS) / Superintendent.SafetyThe Subcontractor shall submit, with their proposal, the on-site SafetyRepresentative qualifications (resume).Quality Assurance (QA)The Subcontractor shall submit with their proposal a letter designatingwho will be the Subcontractor point of contact for quality related matters(e.g. anchor bolt inspections, tracking and disposition of NonConformance Reports, and so forth).This designation shall be on Subcontractor letterhead and include thefollowing: name, phone number (both office and cell if available), andemail address.4.18Construction Completion and AcceptanceUpon completion or cancellation of the work, the Subcontractor shall notifythe Company of substantial completion. Independent systems or facilities,where construction is complete, may be subject to partial acceptance atCompany’s request.The Subcontractor’s notifications will: Verify that all applicable submittals are complete as required by theMaster Submittal Log. Verify that all applicable work packages are complete andapplicable documentation is submitted to the Company.Page 13 of 134

RFP #XXXXX / Project #XXXSection 01010Issue Date:SUMMARY OF WORKAX Farm Equipment Removal Request a walk-down and finalization of a punch list for partial andfinal construction completion.Subcontractor will arrange with the Company a mutually agreeable timefor a walk-down of the completed work.The Subcontractor will prepare and provide a discreet punch list detailingall items required for construction completion and will review the punch listwith the Company.4.19Reporting and Administration4.19.1 In addition to the Supplemental Provisions identified in PART IIIGeneral Terms, the Subcontractor shall: Provide monthly an estimate of the total billable cost frominception of the Subcontract through the current calendarmonth end to Company Contract Administrator’s AccountsPayable. This information must be provided electronicallybetween the 12th and 16th of each month using the logininformation contained in the Subcontractor’s confirmationemail when the Subcontractor completed its initial vendorregistration with the Contract Administrator. Accruals aresubmitted through the Vendor Registration System ic AccrualInstructions.pdf Continue to provide accrual information through contractcloseout, regardless of when the deliverables in Appendix B,Work Breakdown Structure and Appendix E, MasterSubmittal Log have been met. Notify the Buyer’s Technical Representative (BTR) when75% of the funding has been spent. Attend schedule status meetings and provide updates asrequested by the Project Manager or designee. This log can be made available to theSubcontractor upon the Subcontractor’s request.Pricing for all contract changes shall be communicated as follows: A statement indicating if the pricing is a Rough OrderMagnitude (-25% to 50%), Budget Estimate (-10% to 25%), Proposal (not-to-exceed value), or Actual, shall beincluded in the subject line. Rough Order Magnitudes (-25% to 50%), Budget Estimates(-10% to 25%), Proposals (not-to-exceed value), andActuals, shall be in sufficient detail (as determined by theCompany) to permit an analysis of all labor, material, andequipment costs, and shall cover all work involved in theSubcontractor pricing for the requested change. oExamples of acceptable back-up documentationinclude, but are not limited to: catalog sheets, recentsupplier receipts for like items/services, quotes fromsuppliers, identification of standard rate publication,and/or website screen shots.oPricing for equipment rates shall be supplemented bydelineating operating time and stand-by time.Pricing for requested change shall identify, by itemization,any requested deviations from the rates as published in theHanford Site Stabilization Agreement Appendix A.Page 15 of 134

A synopsis of services performed. CAL pricing shall appear as separate line items on monthlyinvoices.In addition to the above requirements, actuals for Time and Material(T&M) contract changes shall be communicated as follows:4.20 Invoicing or receipts must be included for Subcontractor(including lower-tier) rented equipment, task-specificequipment purchased, and materials. Back-up and supplemental information for equipment,materials, and labor that includes multiple applications shallbe marked-up to indicate the scope applicable to eachapplication separately. For example, a time card where anemployee worked 10 hours, 5 hours of which was on acontract action and 5 hours on the base contract. The timecard shall indicate in some fashion which hours are billableto the contract action, and which to the base contract. If the Subcontractor has any questions or needs clarificationregarding the level of detail on invoicing or pricing forchanges, notify the Company’s Subcontract Administrator inwriting.Radiological Source Use ApprovalThe Subcontractor shall obtain approval from the Company prior tobringing any radiological source (e.g., densitometer) on the Hanford site.The Subcontractor shall submit the following for approval: U.S. Nuclear Regulatory Commission (NRC) license or licenseissued by an NRC agreement state (e.g., general license for suchsources issued by the State of Washington).
