How to create Quizzes using Google FormsMadeline Davis, MSLISPart 1: How do I get Google Forms as a Quiz?1. Sign into your Google LCPS Drive Account.1

There are two ways to open Google Forms and set up a Google Form.2. First, Select NEW in the blue box on the left side.1. A drop down menu will appear. Select More to see other GoogleApps.2. Select Google Forms from the More drop down menu.2

3. This opens Google Forms.4. Next, you will need to change the settings of Google Forms to set upthe features of a Google Quiz.i. First, view the upper right hand corner and select the gear(Settings).ii. This will pop up the Settings information and options. You maylimit your students’ responses as well as the ability to edit aftersubmission and a summary. Remember to save your changedSettings!3

iii. Next, select the Quizzes Tab within the Settings.iv. The Quizzes tab will allow you to change the settings andformat the quiz.1. Click the sliding button to make the form a quiz.2. Select how you want the grade to be released:immediately or after some manual editing.3. Select what you want your students to see: MissedQuestions, Correct Answers, and/or Point Values.4. Lastly, SAVE!5

Go to Part 2 to view the Quiz functions.6

3. The Second way to open Google Forms into Google Quizzes:1. Select the Waffle icon2. select Forms from the drop down menu. (You may have to select“More” if you do not see the Forms icon.)3. This will bring up all the Google Forms you have created and thetemplates for Google Forms.7

4. Next, Select Blank Quiz to start your quiz! (Second option from theright.)Go to Part 2 to view the Google Quiz Functions8

Part 2: Google Quizzes Functions1. First, Make yourself familiar with the Google Quiz Settings (the gear icon).2. The settings will allow you to:a. General: Restrict to LCPS users, Limit your answers to one Response,Edit after submissions, and more.9

b. Presentation: Have your quiz show a progress bar, shuffle questionorder, and your own Confirmation message.c. Quizzes: Make this a quiz, How to release the student’s grade, whatthe student can see.10

d. DON’T FORGET TO SAVE!11

3. Next, Title your Quiz and add a description.4. Select the “Untitled Question” to start entering in your Quiz Questions.5. Fill out your question and answer options. You may change your answeroptions by selecting the Gray Box and choosing from the drop down menu.(Tip: Make your first question or last question the student’s name. This wayyou have a record of who submitted which answers.)12

6. Next, select the Answer Key to take you to the grading functiona. Select the correct answer and the point value for the question.b. Add Answer Feedback allows you to have feedback/information comeup if the students get the answer incorrect or correct.c. Edit Question will take you back to editing the question and answeroptions.14

7. Don’t forget to make each question Required!16

8. The major features to any Google Forms or Google Quiz are:a. In Area 1:i. The Plus sign to add questions.ii. The Tt to add titles within the Quiz.iii. The Mountains image to add pictures or images.iv. And the sign to add another Section to your Quiz. (Thesections allow you to separate the Quiz if you have multiplecomponents to your Quiz. It will also jump the student to a newpage and set of questions.)17

b. In Area 2:i. The two boxes image allows you to duplicate the question.c. In Area 3:i. The Trash Can allows you to delete the question from yourquiz.9. Now, if you want to randomize the options so every student has a differentorder or go to another section based on the students’ answers:a. Select the three vertical dotsb. Choose from Shuffle option order or Go to section based on answer.18

10. To view your completed form:a. Click on the eye icon in the upper right hand corner. This will showyou how your form will look to your students.Go to Part 3 to see how to provide the quiz link to your students.20

Part 3: How do I get the link to this Quiz so mystudents can take it?1. Select the Send Box in the upper right hand corner.2. This will create a pop up where you can send the quiz to other people, addcollaborators, and view the link to the form.21

3. Select theicon next to the envelope. This will take you to the link.a. Select Shorten URL to make it easier to use the link.b. Then click copy to copy the link to your quiz.22

4. This link can be added in Google Classroom or on any website you wish touse. In Google Classroom, make an Assignment and link the Google Quizusing the circled icon below.23

Part 4: Grading1. After students submit their Quiz, they will be able to view their score byselecting the link: View your score. If you have short answer questions, theywill not be graded until you manually go into each quiz to view the answers.24

2. To view Responses for your gradebook:a. Select Responses within your Quiz.b. This will take you to a summary of the quiz that shows the average ofthe class (Insights), which questions were missed the most (Frequentlymissed questions), and correct responses.25

c. To view individual quizzes:i. Select Individual. This will allow you to go through each of thestudents’ responses.26

3. To compile all the responses into a spreadsheet:a. Select the green box with the white plus sign. This will transfer yourresponses into a Google Sheet.27

b. Next, select a response destination: Create a new spreadsheet andselect CREATE. This will put your responses in a Sheet in yourGoogle Drive.28

4. To stop Accepting Responses from you students:a. Select the Accepting responses bar to turn it off.5. If you wish to Get email notifications for new responses, downloadresponses or Print all responses:a. Select the vertical three dots next to the Google Sheets icon.b. Select from the options on the command list.29
