KRM LOTO guidelines for LOCKOUT TAGOUT System to meet OSHA LOCKOUT/TAGOUTSTANDARD, 29CFR 1910.47 & 1910.269Introduction:Many Industrial accidents / injuries are caused by unexpected energization orStartup of machines or equipment, or by an uncontrolled release of energy, duringTheir maintenance or repairs These incidents can be prevented by implementing aProper “KRM LOTO - LOCKOUT TAGOUT procedure”. KRM LOTO LOCKOUTTAGOUT System is used to ensure that the energized machines or equipment areIsolated from all potentially hazardous energy sources , and isolation point lockedOut before carrying out any repairs or maintenance activities, thus preventingInjuries. Any person upon observing a machine or equipment which is isolated andlocked out shall not attempt to start energize, or use it.Section 24 of Factories act requires that, when any device which can beinadvertently Shifted from OFF to ON position, is provided within a factory tocut off power arrangements shall be provided for locking the device in a safeposition to prevent accidental starting of the transmission machinery or othermachine to which the device is fitted

KRM LOTO guidelines for LOCKOUT TAGOUT System to meet OSHA LOCKOUT/TAGOUTSTANDARD, 29CFR 1910.47 & 1910.269Guidelines:a) All drives (Motor) shall be provided with separate local lockable isolators, toensure safety of personnel required to work on these drives during preventive orbreakdown maintenance.b) Lockout & Tag out procedure shall be established and implemented duringinspection, cleaning , repair or maintenance of any plant, machinery or equipment,so that if inadvertently activated or energized will not cause any personc) Every person carrying out the work shall be fully instructed / trained on the lockout procedure applicable lock devices etc. beforecommencing the work.d) Unit shall identify all energy source & their isolation point and each point shall befully given an identification number and a list of isolation point shall bemaintained,Typical Energy Sources Where isolation is required are;

Electrical Energy Sources switchgears such as ACB, SFU’s, MCB’s, MCCB’s etc.,Motors, Generations , switch etc.iii. water: Gate Valves pumps etc.iv. Acid / ENA/ Blend lines: Control valves, Pumps etc.v. Compressed air: valves receiver tanks etc.vi. Steam : control / valves ,isolation valves etc.vii. Gas : control / isolation valves etc.viii. Unit must ensure that all electrical isolators in the panel are provided withlocking facilities .where the isolator does not have built-in locking facility,external locking facility should be provided.ix. The requirement of KRM LOTO devices (Type as shall no’s)shall be assessedand adequate no’s. of necessary devices shall be made available in the unit atall times. Refer section 4.0 for guidance on selection of KRM LOTO devices. AllKRM LOTO devices shall be kept at easily accessible place & in custody of aresponsible person.

Manager responsible for the repair / maintenance shall ensure that only trained &authorized employee’s carry out KRM LOTOB. Ensure a valid work permit is obtained prior to start of work (where applicable)C. Identify all energy isolation points such as power supply fuel, compressed air supplyfuel supply gas supply etc. For that equipment / machine before locking out.D. Physically locate and turn the respective switches valves etc. to “OFF”/ Neutral”position and provide necessary KRM LOTO devices in such a way that they cannotbe passed.E. Prior to starting work machines or equipment that have been locked after ensuringthat no personnel are exposed, that the authorized person shall operated the pushbutton or normal operating controls to verify that the appropriate equipment ormachine has been de-energized & ,make certain it will not operate.F. If more than one person are required to work one the equipment under isolationeach person shall put a separate lock on all isolating devices.G. A tag consisting of following information shall be fixed to each lock (please seesample tag)a. Name & department of the person carrying out repair/ maintenance work.b. Caution note written as “DANGER LOCKED OUT”H. After the serving and / or maintenance is completed authorized, person shallinspect the removed.I. Supervisor after ensuring that the work is complete, allows the authorized person toremove lockout devices & energizes the machine.
