FIRST DIAGNOSIS CANCER BENEFIT POLICYPolicy Form Series CF-960201-UTA, CF-940101-UTA & CF-940101-UTA with CF-940101-UTA-BIADescription: A lump sum benefit cancer policy payable on first diagnosis of internal cancer or malignantmelanoma. Benefit amounts are state specific and range from 10,000 to 50,000. For more information,please see our First Diagnosis Cancer brochure (UTA-7-0002-GN) available through our Supply Department atCSBSupplies@CIGNA.com or on AgentView. State variations apply. Coverage plans available are individual, single parent, or family. Guaranteed renewable for life. Dependent children are covered to age 21, unless they are enrolled as a full-time student, then coverage isextended to age 25, or until date of marriage, after which coverage is dropped. Mandatory 30-day waiting period which begins on the effective date of the policy. Either spouse on a family plan may be the primary insured, but the premiums will be based on the age ofthe oldest insured. Issue ages 20-84 (20-64 in CA).Optional RidersIntensive Care Unit Benefit Rider (ICU) – Pays for Intensive Care or Cardiac Intensive Care when confinementis a result of any injury or sickness. Required formsOutline of Coverage – The use of an outline of coverage is required with all of our products. Please consult withthe Supply Department at CSBSupplies@Cigna.com or AgentView for the proper outline of coverage form forthe appropriate state.Choosing a Medigap Policy: A Guide to Health Insurance for People with Medicare – All agents are required byregulations to leave a current year’s copy to all Medicare eligible applicants when they are applying for disabilityinsurance policies or certificates that provide hospital or medical expense coverage on an expense incurred orindemnity basis. This includes First Diagnosis Cancer, Cancer Treatment, First Diagnosis Heart & Heart Diseasepolicies.Delivery of the Guide must be made to the applicant at the time of application and acknowledgement of receipt ofthe guide must be obtained by the insurance company. The applicant must be able to answer “yes” when asked bythe home office if they received a copy of the Guide. The Guide can be sent to the applicant electronically (emailedas an attachment or provide a link to the Guide to view online) or by paper copy mailed or delivered to the applicant.The Buyer’s Guide is available from the following sources:You can find the Guide on AgentView under Other Required Forms. Medicare Website – The Guide is available online to all consumers via the Centers for Medicare & MedicaidServices (CMS) website at: 0.pdf.Supply Department– You can request a paper copy from CSBSupplies@CIGNA.com or fax your order to(888) 417-8267. Please limit orders to 50 each.New Business Guidelines You must be licensed in the state where the applicant resides. All paper applications must be signed by the primary insured. A power of attorney is not acceptable. The policyowner’s signature is required in addition to the signature of the insured if the policyowner is otherthan the proposed insured. The policyowner must exhibit a definite insurable interest. Generally, we assume this exists when there is ahusband/wife, parent/child, or grandparent/grandchild relationship. If it is necessary to correct a mistake on the paper application, both you and the applicant must initial thestrikeover and this must be done in the presence of the applicant. Be sure to mark the correct benefit amounts on the application. Your assigned agent number must be included in the space provided on the application for the agent’s number. The effective date will be the date of issue unless a specific start date is required due to a replacement onapplication. Initial full modal premium must be submitted on all applications (except for faxed and Express App applicationswhere the bank draft authorization can be completed for premium).Due to the USA Patriot Act’s broad anti-terrorism measures, UTA’s policy is to prohibit money laundering throughdetection, deterrence and prevention. Therefore, we do not accept currency (cash), foreign currency, Cashier’schecks, money orders or Travelers checks as premium payments. A check drawn on the payer’s own account, suchas a personal check, is not considered cash. Third Party checks and/or representative payees are not acceptable forpayment of any contract premium, unless Group/Association Direct/List Bill status is utilized.6Policy Details

Underwriting guideLInesUNDERWRITING GUIDELINES FOR ALL UTA CANCER PRODUCTSThe medical conditions listed below are not insurable with Cancer plans: Have or ever have had internal cancer or malignant melanoma. Have or ever have had cancers of the blood including leukemia, lymphoma, mylelodysplastic syndrome,myeloma and myeloproliferative disorder. Have been diagnosed with Familial Adenomatous Polyposis. Have been diagnosed with Sarcoidosis. Are suspected of having or have symptoms or signs of cancer. Have been diagnosed with AIDS, ARC or who are HIV positive. Have been diagnosed as having a terminal illness. Have been diagnosed with Alzheimer’s disease or Dementia. Are bedridden, inpatient of a hospital, or confined to a nursing home or other health care facility. Are scheduled for surgery in the next 60 days and/or had surgery within the past 30 days. Have or ever have had Prostate-Specific Antigen (PSA) levels greater than 4.0.UNDERWRITING GUIDELINES FOR ALL UTA heart products and the ICU RiderThe medical conditions listed below are not insurable with Heart plans and the ICU rider: Have been diagnosed with AIDS, ARC or who are HIV positive. Have had any of the following circulatory diseases: cardiovascular disease (e.g. heart attack/bypass surgery),cerebrovascular disease (e.g. stroke/transient ischemic attack), or peripheral vascular disease (e.g. atherosclerosisof the aorta/aortic aneurysm). Have been diagnosed as having a terminal illness. Have been diagnosed with Alzheimer’s disease or Dementia. Have diabetes mellitus. Have been diagnosed with Sarcoidosis. Have renal disease or failure. Are currently receiving, or been advised to undergo, chemotherapy or radiation treatment for cancer. Have had or have been advised to have a heart, liver or kidney transplant. Have or have had uncontrolled hypertension at any time after the period of initial diagnosis and treatment.Uncontrolled hypertension is defined as blood pressure readings of 140/90 and above. Are bedridden, confined to a wheelchair, inpatient of a hospital, or confined to a nursing home or other healthcare facility. Are scheduled for surgery in the next 60 days and/or had surgery within the past 30 days. Are obese (see chart on page 9).7Policy Details

Additional Underwriting Guidelines for the ICU Rider (Continued)The medical conditions listed below are not insurable with the ICU rider: Have had, or been treated for, alcohol/drug dependency in the past five (5) years. Are incapable of taking care of their own basic activities of daily living. Are or have been on Social Security Disability or other Disability benefits in the past 12 months. Have sickle cell anemia, cystic fibrosis, Down’s Syndrome or other congenital disease or birth defects thataffect mortality. Have a totally disabling musculoskeletal, neurological or mental disorder. This includes Muscular Dystrophyand Multiple Sclerosis. Have a history of leukemia, lymphoma or other cancers of the blood even if considered now to be in remission. Are using oxygen or have been advised to use oxygen in the past twelve months. Have a history of tumor of the brain, lung, liver or connective tissue, whether benign or malignant. Have been diagnosed with Familial Adenomatous Polyposis.
