Activity InformationActivity DescriptionThe sixth webinar in the series focused on the identification and integration of behavioral healthneeds within the Disability-Competent Care (DCC) Model. This activity was developed without commercial support.Chris Duff, MDiv, BS, presenter, directed a rehabilitation facility for persons with disabilities,and developed numerous community-based programs to support independent living from1978 through 1998. Mr. Duff subsequently founded and served as President and CEO of AXISHealthcare in 1998, a care management organization that partnered with UCare Minnesota todevelop and deliver the Minnesota Disability Health Options program, from 2001 through 2009.This partnership established one of the first demonstration programs in the country servicingadults with disabilities on Medicare and/or Medicaid. He established and led the DisabilityPractice Institute, sponsored by the Commonwealth Care Alliance of Massachusetts andIndependence Care System of New York in 2010. Mr. Duff has worked as a Consultant for theLewin Group, under their contract with the Medicare and Medicaid Coordination Office (MMCO)since 2011.Mr. Duff has worked in the field of rehabilitation with persons with disabilities for more than 40years. She is a Licensed Clinical Social Worker providing counseling for adultsand children at OCHS Provincetown Community Health Center. Before joining OCHS in 2014,Ms. Hansen served as the Clinic Director of the Family Counseling Center for the MassachusettsSociety for the Prevention of Cruelty to Children (MSPCC) in Hyannis, Massachusetts from 2011through 2014. She has maintained a private counseling practice since 1992.Ms. Hansen earned a Master of Social Work degree from Michigan State University and aMaster's Certificate in Global Mental Health, Trauma, and Recovery from Harvard University.Ms. Hansen has nothing to disclose.Andrew Jorgensen, MD, presenter, is the Chief Medical Officer at Outer Cape Health Services(OCHS) and is board certified in pediatrics and internal medicine.
