Introduction to the Competency Assessment TemplateWhy do we have this document?Providing high quality research requires a trained and competent workforce (ICH GCP 1996).One of the ways of validating staff training is to carry out a competency assessment (EdCaN2008, Epstein & Hundert 2002). Often the competency assessments have to be written ‘fromscratch’ because there is none available that matches the activity which has to be assessed.The UKCRF Network Education Group were tasked by the UKCRF Network Lab ManagersWorkstream to create competency documentation that could be adapted for the assessementof laboratory skills. It became clear at an early stage that there was value in having a genericcompetency assessment template. Subsequently, the Education Group established a projectgroup, the Competency Assessment Template Project Group, to explore and create a genericcompetency assessment template that could be populated for any relevant activity thatrequired competency assessment.The literature search for this project yielded a plethora of specific competency assessmentdocuments in a range of vocational education settings, for example: Canadian Society ofRespiratory Therapists (2011), NHS Health Education North West (2013), the University ofNorth Carolina (2012). However, it appeared that there had not been any work carried out tocreate a nationwide generic template to document evidence of competence for clearly definedpractical tasks undertaken by clinical research staff.With no existing generic template we could reuse, we agreed to develop our own, resulting inthe UKCRF Network Competency Assessment Template.What is the Competency Assessment Template?The Competency Assessment Template (CAT) presented here by the UKCRF NetworkEducation Project Group is a tool that can be utilised in the workplace of clinical research. Itspurpose is to provide a recommended structure which may be populated by staff who have towrite a competency assessment for a given activity in Clinical Research.The boundaries of the Competency Assessment TemplateFor the purpose of this document a definition of “competency” has been adapted from theCompetency Framework for Clinical Research Nurses (Competency Framewok Group 2011):The ability to demonstrate the application of knowledge, understanding, practicaland thinking skills to achieve effective performance in a practical skill.The CAT has been designed for those instances where a competency assessment is requiredand where there is no existing documentation in place. Where there is already aTrust or Board policy that determines how competency is assessed it is recommended thatthese are used if they are available and appropriate for the task.1

Miller’s (1990) pyramid model forassessing clinical competency (seeFigure 1) is widely referred to in theliterature on competency assessment.The CAT has been designed to allowassessment at a level approximatingto the “shows how” or level 3 ofMiller’s pyramid (Miller 1990). At thislevel the member of staff has fullknowledge and comprehension of theprocedure, shows that they candiscuss the principles in detail, canperform the procedure competentlyFigure 1: Miller GE (1990)without supervision or assistance, butrecognises their own limitations and seeks appropriate advice when necessary.The CAT is to be used in conjunction with any professional code of conduct relevant to thestaff member and in accordance with ICH GCP. All staff remain accountable for theirprofessional conduct at all times during training, supervised practice, competencyassessment and in practice following sign-off.It is recognised that Standard Operating Procedures (SOPs) assist in reproducibility andconsistency of performance of tasks (Amare 2012). However, SOPs do not ensure thatcompetence has actually been achieved. This does not mean that all SOPs require to befollowed up by a competency assessment. Such a decision will be made locally and will takeinto consideration factors such as risk and quality assurance.Please note the CAT should not be confused with a Competency Framework, for example,that for Clinical Research Nurses (Competency Working Group, 2011). Such frameworksprovide a broadly defined combination of competences that mark out an entire staff group inthe clinical research field. In contrast the CAT was designed so it could be populated andserve as an assessment document for a well-defined task or activity which a member of staffis required to undertake in Clinical Research.Introduction to using the Competency Assessment TemplateFor ease of use the project group decided to issue the CAT in the following two forms:Single Competency Assessment to be used for tasks which require a singlecompetency assessment prior to sign offMultiple Competency Assessment to be used for tasks which require multiplecompetency assessments prior to sign offThe CAT is a blank template designed to be populated with the various components thattogether make up the specified task for which a member of staff has to be assessed, forexample, this could be the task of being competent in using a Gilson Pipette. Thesecomponents will fall into the categories of knowledge, skills and behaviour.2

We have provided two examples where a CAT has been populated with the wording for acompetency assessment. One is for Preparation of Blood Films and the other is forMeasuring Adult Height with Portable Leicester Stadiometer. During the draft stages of thisproject it has proved very useful to look at such an example while trying to understand theinstructions for the CAT.If the person populating the template is not the designated expert in the subject, the expertmust authorise the content of the template. A system recording expert authorisation should beimplemented locally.Ideally the CAT will be produced alongside an SOP for the same task, but the CAT may onoccasion be produced as a stand-alone document. The components of the competencyshould reflect the content of any related SOP.It is recommended that the populated assessment document is reviewed at least every2 years or when there is a significant change in practice or a change in the related SOP –whichever is earlier. It is suggested that staff members will not need to be reassessedimmediately following the production of a new SOP and CAT but should be made aware ofthe change. In the case of significant change in practice, appropriate training should be givento staff members who have already been signed off as competent and arrangements forreassessment made as appropriate.Dependent on the task, it may be sufficient to complete a single competency assessment inorder to be signed off as competent, e.g. use of the centrifuge. For a more complex task itmay be necessary to complete multiple competency assessments prior to being signed offas competent, e.g. venesection, receiving informed consent.Detailed guidance has been created outlining how to populate the template prior todistribution and how to use the template during assessment (see pages 26-29).TrainingThe Competency Assessment Tool (CAT) has been designed to aid competency assessmentfollowing appropriate training and supervised practice. It is not a training document, thoughevidence of training may be required as part of the competency assessment i.e. as evidencethat a competency component has been achieved. Adequate training should be sourced asappropriate to the task. Should a period of supervised practice be required following initialtraining, this can be supported by another member of staff who has already been assessedand signed off as competent in the task. A member of staff will only be considered safe andcompetent to perform the task autonomously once they have been assessed and signed offas competent by an assessor.3

Roles and responsibilitiesAssessee - the member of staff undergoing competency assessment.An assessee:is accountable for their own practice and competency in the task at every stage of thetraining and assessment processis accountable for the ongoing maintenance of their skills (following sign off ascompetent)Assessor - An individual who has full knowledge and comprehension of the task and hasfully developed their skill without the need for supervision and assistance in more unusual orcomplicated situations. They have been deemed fit to act as an assessor by the expert. Thecriteria for who can become an assessor will be a local decision.An assessor:is accountable for signing off members of staff who demonstrate sufficient knowledgeand skill during competency assessmentmust give adequate feedback to those who do not show competence on assessmentmust complete all CAT documentation fully at the time of assessmentmust devise an action plan for re-assessment if needed, in collaboration with theassessee’s line manager as appropriateExpert - An individual who has extensive knowledge and experience in the field in which theyare regarded as an expert and can demonstrate sufficient evidence to support this. Thecriteria for who is regarded as an expert will be a local decision.An expert:is accountable for ensuring that the template is populated accurately prior to usemust ensure that all assessors have appropriate experience in order to act asassessorscan also act as an assessorLine managers - organisational line manager of member of staff.A line manager:must allow access to training and ensure time for any supervised practice neededprior to competency assessmentwill liaise with the assessor, as appropriate, to devise an action plan for reassessment if neededSupervisor – any member of staff who has already been assessed and signed off ascompetent in the task may supervise another member of staff prior to completing theircompetency assessment.A Supervisor:must ensure that the member of staff is practising according to local policy andprocedureis accountable for their own practice whilst supervising others4

Some explanations about assessment, feedback and record keepingPractical assessmentThis will usually take place during normal practice but may take place as a simulationexercise as appropriate e.g. role play of the informed consent process. Assessment shouldnot be undertaken until the assessee has had sufficient training and opportunity forsupervised practice. In order for sign off following competency assessment all components ofthe task must be achieved as set out in the CAT. Feedback must be given promptly followingthe assessment.FeedbackIf competency is not achieved following assessment then performance feedback should begiven in order to help the assessee to gain the further knowledge or practice required. A planfor re-assessment should be made and documented. Feedback following achievement ofcompetency may also be helpful and should be given as appropriate.Re-assessment following non-achievement of competency components onassessmentMechanisms and time-scales for re-assessing should be decided locally. Review withassessee’s line manager should be considered.On-going competency assessmentCompetency will usually be re-assessed after a set period of time, two yearly isrecommended but this will depend on the task and local decision. The appropriate reassessment period should be clearly marked on the populated competency assessmentdocument.Record keepingA system for maintaining records of competency assessment, whether by individual staffmembers or centrally, should be put in place. It is useful to have an SOP outlining the localprocedure for competency assessment and record keeping and this may include the localdecisions regarding criteria for who can take the role of expert, re-assessment mechanismsetc.5
