IntroductionWhat to Do in an EmergencyIf there is a need for cardiopulmonary resuscitation (CPR)or there is an immediate threat to your life or a limb, call911. If there is no need to call 911, go to the nearest hospitalor emergency room (ER). Please see Section 4A of yourMember Handbook for more information about what isand is not an emergency. Call the 24/7 Nurseline toll-free at1-877-213-2567 if you are not sure.Prior authorization is not needed for emergency services.You should call your Primary Care Provider (PCP) as soonas possible after receiving care from the ER to arrangefollow-up services. Do not use the ER in a non-emergencysituation. If you are a member receiving services at a CoreService Agency (CSA), you may also use your crisis plan forfurther instructions and contact your CSA crisis line. Beforean emergency arises, please contact your assigned CareCoordinator and ask about a personal crisis plan.How to Find Help in a CrisisYou can: Call 911. Go to the nearest ER, crisis center, or hospital. Contact the New Mexico Crisis Access Line (NMCAL):The crisis line is staffed by mental health professionalswho can respond to a crisis 24 hours a day, 7 days aweek. The line is available statewide and toll-free at1-855-NMCRISIS (1-855-662-7474). If you arespeech- or hearing-impaired, call 711 for TTY service. The NMCAL also offers a Peer-to-Peer Warmline at1-855-466-7100. The Warmline provides assistance andan understanding ear to New Mexicans struggling withsubstance abuse, grief, suicidal thoughts and otherbehavioral health issues. IntroductionWelcome toBlue Cross Community CentennialWelcome to Blue Cross Community Centennial, a health planadministered by Blue Cross and Blue Shield of New Mexico(BCBSNM). All of the doctors, hospitals and other providersin this directory participate in the network that supports yourBlue Cross Community Centennial plan. Specialists in thisdirectory are providers who you can visit without receiving areferral from your PCP. If you wish to visit a provider who isnot listed in this directory or who is not contracted directlywith BCBSNM for the Blue Cross Community Centennialplan network, you must receive prior authorization fromBCBSNM.The list is current as of the date it was published. BCSBNM gives youanother way to get care. Blue Cross Community Centennialmembers can access care for non-emergency health issuesand behavioral health needs through MDLIVE .Whether you are at home or on the road, you have accessto board-certified doctors and psychiatrists or licensedtherapists. Access is there for you 24 hours a day, sevendays a week. With MDLIVE’s virtual visits, you can speak toa doctor quickly or make an appointment based on youravailability. The average wait time is less than 10 minutes.Behavioral health consultations with a psychiatrist orlicensed therapist are available by appointment only, viasecure video. IntroductionCare CoordinationThe role of the Care Coordinator is to help memberswith special health care needs whether at home or in thehospital. Care is centered on the member, focused on thefamily, when needed, and sensitive to the member’s culturalbackground. Care coordination can help you better identifyyour medical needs. It also helps you get appropriateservices. This includes care coordination among providers inthe Blue Cross Community Centennial plan network, as wellas out-of-network providers. Your Care Coordinator can helpyou with your physical, behavioral, and long-termcare needs.If you have special needs, BCBSNM will assign you aCare Coordinator who speaks your preferred language and isresponsible for coordinating your health care services by: Giving you information about providers in BCBSNM’snetwork who may address those needs Coordinating medical, behavioral, and long-termcare services Assisting in coordinating care when you also haveMedicare or other coverage Getting help with different appointments, non-emergencytransportation, or other needs; or getting communityservices not covered by Centennial Care Making sure care coordination is provided when neededYou can call your Care Coordinator at 1-877-232-5518.Select option 3 and then option 2 to discuss your medical,behavioral, and long-term care needs. This is calledpost-stabilization care. You may receive post-stabilization(follow-up) care in a hospital or other facility. CentennialCare covers this care, but you may need to obtain priorauthorization from BCBSNM. For other follow-up care,such as medicine refills or having a cast removed, go toyour PCP’s office. IntroductionNon-Emergency TransportationIf you do not have a car or anyone to give you a ride, youmay be eligible for transportation to help you get to yournon-emergency medical, behavioral health, or long-termcare appointments. If you have an emergency and you needhelp getting to an ER, call 911. ModivCare coordinates nonemergency transportation for members, including food andlodging expenses, when you have to travel a long distance toget covered medical care.To request a ride to a scheduled appointment, call theModivCare Reservation Line at 1-866-913-4342. Call at leastthree working days before your visit, Monday through Fridayfrom 8 a.m. to 5 p.m. To return home or to arrange a rideafter hours (such as for urgent care), call the Ride Assistphone line. You can call 1-866-418-9829 toll-free24 hours a day, 7 days a week. Your PCP will alsoarrange your care with other doctors and hospitals. Pleasecall your PCP whenever you have a health need. You mustchoose a PCP upon enrollment. If you do not select a PCPwithin 15 calendar days of enrollment, we will choose one foryou and notify you in writing of the PCP’s name, location, andoffice telephone number. If you need help, we will help youfind a new PCP.Why is it important to get to know my PCP?Your PCP knows you best. Your PCP knows your medicalhistory and present state of health. This allows your PCP tomake the best decisions when you need medical care. YourPCP can also help arrange visits to specialists.How do I make an appointment to see my PCP?1. Call your PCP’s office.2. Tell the office that you are a Blue Cross CommunityCentennial member and have your ID card handy.3. IntroductionFrequently Asked Questions (continued)What is an out-of-network provider?An out-of-network provider is a provider who has not signeda contract with BCBSNM to see Blue Cross CommunityCentennial members.When do I need prior authorization?Certain services require approval from BCBSNM. Thisapproval is called prior authorization. If approval is notobtained, those services will not be covered. You must alsoget prior authorization any time you want to see an outof-network provider. You may have to pay for services ifyou did not follow prior authorization guidelines and stillreceived the service. See the Member Handbook for a listof services requiring approval. To request approval, youor your provider must call BCBSNM Health Services. Call1-877-232-5518, Monday through Friday, 8 a.m. to 5 p.m.,MT. Also,see if the services are covered by your plan before you makean appointment. Otherwise, you will be responsible for costsof services not covered. Even if you don’t need a referral forsome services with specialists, you may need prior approval.Ask your PCP. He or she knows your medical history.
