Once a certainpressure is reached it then returnsto zero. However, IPPB starts with anegative pressure and does not deliverpositive expiratory pressure duringexhalation. Pressure does decreaseduring inhalation, but it always remainspositive, helping to open airways andre-inflate collapsed alveoli. (See figure2). EzPAP also provides the additionalbenefit of Positive Expiratory Pressuretherapy during exhalation offeringfurther rehabilitation to the patient.(See figures 4,5 &6)The combination of therapies offers asafe and effective alternative to IPPB forhospitalised patients. EzPAP is easy touse and inexpensive in comparison toIPPB. Wiersgella has also demonstratedthat EzPAP , when used post operativelyon coronary artery bypass graft patients,shows measurable improvements inatelectasis levels. 1 (figure 3)Positive Expiratory Pressure therapycan also be achieved using the acapella duet from Smiths Medical. The acapella duet is a vibratory PEP device thathas a built-in port for aerosolisedmedication via a small volume nebuliser.Medication such as bronchodilator ormucolytic treatments can be deliveredsimultaneously with PEP therapy, cuttingtreatment time to one session, savingtime and effort for both clinicians andpatients.Products that encourage PEP therapyare usually well tolerated by mostpatients and due to the option of selfadministering, are an ideal choice forpatients who are out of ICU. In contrastto traditional chest physiotherapy (CPT)options, PEP therapy offers effectivesecretion removal at a low cost. Bybeing independent of daily assistancefrom clinicians, the compliance levelis enhanced and this in turn can alsoreduce related health-care costs.2, 3

These devices,with the option of a mouthpiece or mask,are a fully versatile product for bothpatient and hospital.TheraPEP is an alternative productoffering PEP therapy. This therapy canalso be self-administered in half the timeof CPT.4 TheraPEP can accommodatevirtually any lung capacity and allowsinhalation and exhalation withoutremoval from the mouth. With a 22mmID connector to allow small volumenebulisers or MDI spacers and the optionof mouthpiece or mask, this is anothergreat choice for secretion clearanceand atelectasis reversal in the hospitalsetting or at home.For post-surgical patients, medicaldevices that help improve their lungsare an important function. Incentivespirometers encourage patients to takeslow and deep breaths to expand thelungs.
