WINTER 2019Next GenerationNCLEX NEWS Clinical JudgmentMeasurement ModelThe Next Generation NCLEX News is a quarterly publication that provides thelatest information about the research being done to assess potential changes tothe NCLEX Examinations. In this issue, you will find information related to clinical judgment. Clinical judgment is defined as theobserved outcome of critical thinking and decision making. It is an iterative process that uses nursingknowledge to observe and assess presenting situations, identify a prioritized client concern andgenerate the best possible evidence-based solutions in order to deliver safe client care.NCSBN research identified a list of contextual factors that play a role in the quality of nursing clinicaljudgment (Dickison et al, 2016). These factors may be divided into conditions that are internal (education, experience, knowledge, communication, consequences/risk, emotions/perceptions, professionalorientation) or external (task complexity, time pressures, distractions, interruptions, professional autonomy)to the nurse.Continued on next page

Recognizing that it was necessary to ascertain whether clinical judgment is more thanjust possessing nursing knowledge, NCSBN conducted a pilot study in 2016 (Munteanet al. 2016 AERA presentation). Results from this study found that while knowledge isessential, it is not enough to substantiate the clinical judgment essential to safe nursingpractice. The study also indicated that the average ability of a nurse to demonstrate thedifferent steps in the clinical judgment process (cue recognition, hypothesis generation, hypothesis evaluation, taking actions and evaluating outcomes) is progressive. Thus, a nurse’s ability to recognize cues, develop hypothesesand take appropriate actions does not guarantee the abilityto evaluate the outcomes of the action taken. Ultimately,no single element of clinical judgment adequately predictsa nurse’s clinical judgment ability; it is the combinationof all the elements that add validity and reliability to themeasurement of a nurse’s clinical judgment ability. In short,“Because we know that nurseclient care and nurse errorshaving content knowledge does not always translate tohaving clinical judgment skills.can be improved by enhancingThe NCSBN Clinical Judgment Measurement Model (CJMM)clinical judgment skills in novicerepresents a fundamental shift from the current dichotomousnurses, it is imperative that wemeasurement models in which something is either right orfind a way to assess the degreewrong. Layer 4 in the CJMM is onethat has not been introduced in any psychometric models before now – the context. The question is whether you can put context around items in a way that you actuallymake it more real.NCSBN continues to develop item prototypes, collect data and do research onmeasuring clinical judgment and measuring the layers of the CJMM.Continued on next pageNext GenerationNCLEX NEWS WINTER 2019 · 2

The Layers of the Clinical Judgment Measurement Model1. Recognize Cues – Identify relevant and important informationfrom different sources (e.g., medical history, vital signs).Recognize CuesIdentify relevant and important information fromdifferent sources (e.g., medical history, vital signs). What information is relevant/irrelevant? What information is most important? What is of immediate concern?Do not connect cues with hypotheses just yet.2. Analyze Cues – Organizing and linking the recognized cuesto the client’s clinical presentation.Analyze CuesOrganizing and linking the recognized cues to theclient’s clinical presentation. What client conditions are consistent with the cues? Are there cues that support or contraindicate aparticular condition? Why is a particular cue or subset of cues of concern? What other information would help establish thesignificance of a cue or set of cues?Consider multiple things that could be happening.Narrowing things down comes at the next step.Continued on next pageNext GenerationNCLEX NEWS WINTER 2019 · 4

Prioritize Hypotheses – Evaluating and ranking hypotheses accordingto priority (urgency, likelihood, risk, difficulty, time, etc.).Prioritize HypothesesEvaluating and ranking hypothesesaccording to priority (urgency, likelihood, risk, difficulty, time, etc.). Which explanations aremost/least likely? Which possible explanationsare the most serious?Item development should focuson ranking the potential issuesand should use phrases such as"most likely."4. Generate Solutions – Identifying expected outcomes and usinghypotheses to define a set of interventions for the expected outcomes.Generate SolutionsIdentifying expected outcomes and usinghypotheses to define a set of interventionsfor the expected outcomes. What are the desirable outcomes? What interventions can achieve those outcomes? What should be avoided?Focus on goals and multiple potentialinterventions—not just the best one—thatconnect to those goals. Potential solutionscould include collecting additional information.Continued on next pageNext GenerationNCLEX NEWS WINTER 2019 · 5

Which intervention or combination of interventionsis most appropriate? How should the intervention(s) be accomplished(performed, requested, administered, communicated, taught, documented, etc.)?For “how” questions, ensure that specificelements from the scenario are whatdetermines approach. Avoid memorizedor “textbook” procedures. The itemstem and/or the responses shouldinclude action verbs.6. Evaluate Outcomes – Comparing observed outcomes againstexpected outcomes.Evaluate outcomesComparing observed outcomes againstexpected outcomes. What signs point to improving/declining/unchanged status? Were the interventions effective? Would other interventions have beenmore effective?Item development should focus onthe efficacy of the intervention(s)from the previous items.Continued on next pageNext GenerationNCLEX NEWS WINTER 2019 · 6

NGN TalksIn order to provide information regarding aspects of the Next Generation NCLEX (NGN)project such as the topic of the Clinical Judgment Measurement Model (CJMM), the NCSBNExaminations Department has begun recording a series of videos entitled “NGN Talks.”NGN Talks are available for public viewing. The videos include 5-7 minutes of informationaldetails about various aspects of the NGN project. NGN Talks are presented by the ExaminationsDepartment’s staff and include topics such as an introduction to NGN, item development, testvalidity, and many other aspects of the NGN research project. The first NGN Talk provides anintroduction to the NGN project, and is now posted on the NCSBN website.Access NGN TalksLooking for updates about the NGN Project?Sign up for the email list to receive future issues of this publicationNext Generation NCLEX News is published byNational Council of State Boards of Nursing (NCSBN)JOIN OURMAILING LIST111 E.
