Gaps and Barriers Analysis OverviewThe Gaps and Barriers Analysis for Treatment Services in Erie County compares the provision oftreatment services within each ZIP code area to selected risk indicators. Gaps are identified where thereis a mismatch between the treatment services and risk level (e.g. no/low treatment provision & highrisk). First, the distribution of treatment services was evaluated by compiling information about allcertified NYS Office of Alcoholism and Substance Abuse Services (OASAS) treatment programs and theirlocations through the OASAS website. These data were tallied by ZIP code for each program type.Maps were developed from these data to evaluate the spatial distribution of programs in Erie Countyand then to examine existing gaps in service provision against the areas of highest risk. The maps oftreatment programs are broken down into five categories that correspond to none, one, a few (2 or 3),some (4 or 5) and many (more than 5). The gaps and barriers analysis for treatment programs comparesthe highest level of aggregated risk for ZIP codes in Erie County with the number of treatment programs.Risk Indicators Database (RIDB) measures were summed and then re-quartiled for this aggregated riskmeasure; the locations with the highest level of risk are those with aggregated measures that fall in thefourth quartiles of either ZIP codes in Erie County excluding City of Buffalo and ZIP codes in the City ofBuffalo only.Accessibility was measured using definitions instituted by the U.S. Department of Agriculture EconomicResearch Service through research on food access. In urban areas, accessibility is defined as being within1 mile; and in rural areas, within 10 miles. Maps showing ZIP codes of highest risk were derived bycreating straight line distance buffers around the geocoded locations of OASAS treatment programlocations for both Erie County excluding Buffalo and the City of Buffalo only. These locations includedprograms from neighboring counties if they were within 10 miles of the Erie County border.Gaps and Barriers Analysis Results Descriptive Maps of Treatment Program Locationso Many ZIP codes in Erie County do not contain any type of treatment program,particularly in the rural eastern and southwestern portions of the county. However theCity of Buffalo and first ring suburbs have a substantial allocation of programming, witha majority having at least one program.o Crisis and inpatient programs are found primarily within the City of Buffalo while opioidprograms are within the City of Buffalo and two suburban locations (Orchard Park andAmherst).o Outpatient programs are the most prevalent type of program in the county, thoughcoverage is still lacking in the eastern and southwestern portions of the county. There isintermittent coverage in the city, with programs primarily available in the West Side,parts of the East Side and South Buffalo. Overall, first ring suburbs have a gooddistribution of coverage but there are notable locations that lack any programs includingLancaster and Hamburg.o Residential programs are generally concentrated in the city but programs are alsoavailable in West Seneca and Eden.Maps of Highest Risk Locations with Treatment Programso Erie County Excluding City of Buffalo

ZIP codes with highest aggregated risk have few or no treatment programs ofany type. ZIP code 14223 in the Town of Tonawanda has no coverage, andlocations in the Southtowns (14219 and 14085 [Hamburg] and 14006 [Evans])are also lacking program coverage. Other highest risk locations have at least oneprogram, with ZIP codes 14150 and 14225 having a few programs. Outpatientprograms are similarly distributed compared to all programs. None of the highest risk ZIP codes in Erie County, excluding the City of Buffalohave crisis, inpatient, methadone, or residential programs.o City of Buffalo Only Overall, some ZIP codes with highest levels of aggregate risk have good programcoverage across all types with 14203, 14212 and 14215 having at least 4programs. However, ZIP codes 14211 and 14206 on the East Side have noprograms within their boundaries. The availability of crisis, inpatient and opioid treatment programs are limitedwithin the highest risk ZIP codes in the City. Opioid programs are notably notaligned to any of the identified highest risk ZIP codes within the City of Buffalo.Accessibility Mapso Erie County Excluding City of Buffalo Overall accessibility in Erie County based on all treatment programs and types ishigh and well aligned to areas of highest risk. All of Erie County has good accessibility to outpatient treatment programs. Thehighest risk ZIP codes have access to these programs within ten miles. Most of the county has access to residential treatment programs. Only a smallportion of highest risk ZIP code 14006 lacks access to these programs within tenmiles. Though most of the county does not have access to crisis, inpatient, or opioidtreatment programs, only the ZIP codes in the rural southern portion of thecounty with highest aggregate risk lack access (14006 [Crisis, Opioid], 14085[Crisis, Inpatient]). Access to these programs types are concentrated around theCity of Buffalo and the surrounding Northtowns and includes access to programsof these types outside of Erie County in Niagara County.o City of Buffalo Only Overall accessibility in Buffalo for all program types is well aligned to areas ofhighest risk. Only limited portions of ZIP code 14211, 14206, and 14203 arelacking access within one mile. The most prevalent program type in Buffalo is outpatient and areas lackingaccess within one mile are similar to overall accessibility. Inpatient, crisis and opioid program access is limited for nearly all areas of thehighest risk ZIP codes. Residential programs are aligned to portions of highest risk ZIP codes on theEast Side, however substantial portions of ZIP codes 14203, 14206 and 14211lack access.Change in Accessibility Maps (2017 to 2019)o Erie County Excluding City of Buffalo

Overall accessibility in Erie County has increased to cover nearly all of thecounty area outside of the City of Buffalo. Similarly, nearly all of Erie County has good accessibility to outpatient treatmentprograms with all highest risk ZIP codes having access to these programs withinten miles. Crisis program access has expanded slightly in the county since 2017, nowincluding all of Grand Island, though this additional coverage is derived fromprograms available in Niagara County. Inpatient program access has increased, with additional coverage within thecounty in the southwest corner (from programs in Chautauqua). Programcoverage for this type has decreased (due to changes in programming in NiagaraCounty) in the northern fringes of the county. Opioid program access has expanded since 2017 and now includes substantialportions of first and second ring suburban communities in the County. However,further outlying communities, including the highest risk ZIP code 14006, remainwithout access within 10 miles. Most of the county continues to have access to residential treatment programswith no change in the coverage since 2017.City of Buffalo Only Overall accessibility in the City of Buffalo has stayed relatively constant withsome loss of programs in the West Side/Blackrock area and in the UniversityHeights in the northeastern corner of the City. However, there has also been anincrease in coverage to serve a portion of Larkinville in 14210. Outpatient programs show similar coverage in 2019 as they had in 2017, withadditions in 14215 and 14210, with a loss of coverage in the University Heightsin the northeastern corner of the City. Coverage for residential, crisis, inpatient and opioid program coverage remainsthe same as in 2017. o
