This resource guide was compiled by Wheeler Clinic’s Senior EmotionalHealth Outreach Plus Care Program (SEHOP-CP). Call SEHOP-CP, a United Way-funded, FREE, in-home caremanagement assessment program. SEHOP-CP will help provide in-homeservices that will allow seniors to stay in their homes longer.
