MICHIGAN SPEECH-LANGUAGE PATHOLOGY LICENSING GUIDESPEECH-LANGUAGE PATHOLOGY LICENSING GUIDELICENSURE REQUIREMENTS CHECKLIST(Listed below are the minimum requirements needed to obtain licensure in the Speech-Language Pathology field.)APPLICATION AND FEES Application for a Speech-Language Pathology License - Complete all fields, answer all questions andsubmit any supporting documentation. Paper application can be found at www.michigan.gov/speechlanguagepathology Application Fee (Must be paid by check or money order payable to The State of Michigan):Educational Limited – 183.80 (Valid for 2 years from date issued)Educational Limited Relicensure – 203.80Limited Relicensure (Certified Teacher Only) – 122.70Speech-Language Pathologist – 102.70Speech-Language Pathologist Relicensure – 122.70Application fees are valid for a year or less (unless otherwise stated) to get you into the renewal cycle foryour profession.THE FOLLOWING REQUIREMENTS APPLY TO ALL APPLICANTS (EXCEPT RENEWAL) Criminal Background Check – Once the application is received, you will be mailed an ApplicationConfirmation letter containing instructions to complete the Criminal Background Check (except thoseapplicants seeking relicensure, if the license expired within the last three years.) Good Moral Character Questions – Documentation will be required if you answer “yes” to either question toshow at the current time you have the ability to, and are likely to, serve the public in a fair, honest and openmanner, that you are rehabilitated, or that the substance of the former offense is not reasonably related tothe occupation or profession for which you are seeking a license. Human Trafficking – Beginning March 16, 2021, individuals seeking licensure must have completed trainingin identifying victims of human trafficking that meets the standards in Administrative Rule 338.604 prior tobeing issued a license. The training must only be completed one time. Social Security Number – An individual applying for licensure is required to provide his or her social securitynumber at the time of application. If exempt under law from obtaining a SSN or do not have a SSN, astatement in writing is required at the time the application is submitted. Verification of Licensure – If you hold a current license or ever held a license as a speech-languagepathologist, verification of licensure will be required to be submitted directly to this office by the licensingRevision Date 9/17/19Page 1 of 5

Theverification must include your name, the date your certification was issued, the expiration date of yourcertification and it must specify that your certification was issued in speech-language pathology. Thecertification can be emailed to bpldata@michigan.gov or mailed to the Bureau of Professional Licensing, POBox 30670, Lansing, MI 48909.OR Final, Official Transcripts – Transcripts must be submitted directly to our office from the school attended andmeet the standards set forth by the Board. Transcripts can be emailed to bpldata@michigan.gov or mailed tothe Bureau of Professional Licensing, PO Box 30670, Lansing, MI 48909. If the education was taught in a non-accredited postsecondary institution, in place of a transcript fromyour educational institution, you must submit a full course-by-course credential evaluation of yourSpeech-Language Pathology education certified by a credentialing agency accredited by NACES. A listof approved credentialing agencies may be found on their website at www.naces.org. If the education was not taught in English, you must obtain either a score of 570 on the writtenTOEFL exam, 230 on the computerized TOEFL exam and a score of 50 on the TSE exam, or anoverall score of 89 or above on the TOEFL iBT examination. Post-graduate Clinical Experience Form verifying 9 months (1,260 hours) of supervised post-graduateexperience. This form can be emailed to bpldata@michigan.gov or mailed to the Bureau of ProfessionalLicensing, PO Box 30670, Lansing, MI 48909. PRAXIS Series II Exam in Speech-Language Pathology must be completed. Contact ETS and have your PRAXIS Series II Test in Speech-Language Pathology score report sent to theMichigan Board using recipient code 7430.AND If licensed less than 5 years, submit the Post-graduate Clinical Experience form verifying 9 months (1,260hours) of supervised post-graduate experience.ADDITIONAL REQUIREMENTS FOR SLP RELICENSURE Proof of 20 hours of continuing professional development courses are required to be submitted, including 1hours in pain and/or symptom management. If the license has been lapsed more than 3 years and the applicant is not licensed in another state, passingresults of the PRAXIS Series II Exam is required. Human Trafficking – Beginning in 2018, and all renewals thereafter, licensees seeking renewal must havecompleted training in identifying victims of human trafficking that meets the standards in Administrative Rule338.604 prior to renewing their license.
