PrefaceThe American Nurses Association (ANA)Social Policy Statement of 1980 was the firstto define nursing as the diagnosis and treatment of human responses to actual and potentialhealth problems. This definition, when combinedwith the ANA Standards of Practice, has providedimpetus and support for the use of nursing diagnosis.Defining nursing and its effect on client care supportsthe growing awareness that nursing care is a key factorin client survival and in the maintenance, rehabilitative, and preventive aspects of healthcare. Changesand new developments in healthcare delivery in thelast decade have given rise to the need for a commonframework of communication to ensure continuity ofcare for the client moving between multiple healthcare settings and providers.This book is designed to aid the student nurseand the practitioner in identifying interventionscommonly associated with specific nursing diagnosesas proposed by NANDA International (formerly theNorth American Nursing Diagnosis Association).These interventions are the activities needed toimplement and document care provided to the individual client and can be used in varied settings fromacute to community/home care.Chapter 1 presents a brief discussion of the nursing process and introduces the concept of evidencebased practice. Standardized nursing languages(SNLs) are discussed in Chapter 2, with a focus onNANDA (nursing diagnoses), NIC (interventions),and NOC (outcomes). NANDA has 167 diagnosislabels with definitions, defining characteristics, andrelated or risk factors used to define a client need orproblem. NIC is a comprehensive standardizedlanguage providing 514 direct and indirect interven-tion labels with definitions and a list of activities anurse might choose to carry out each intervention.NOC language provides 330 outcome labels withdefinitions, a set of indicators describing a specificclient, caregiver, family, or community states relatedto the outcome, and a 5-point Likert-type measurement scale that can demonstrate client progress evenwhen outcomes are not fully met. Chapter 3 addressesthe assessment process using a nursing framework fordata collection such as the Diagnostic DivisionsAssessment Tool.A creative approach for developing and documenting the planning of care is demonstrated inChapter 4. Mind Mapping is a new technique orlearning tool provided to assist you in achieving aholistic view of your client, enhancing your criticalthinking skills, and facilitating the creative processof planning client care. For more in-depth information and inclusive plans of care related to specific medical/psychiatric conditions (with rationalesand the application of the diagnoses), the nurse isreferred to the larger works, all published by theF. Davis Company: Nursing Care Plans: Guidelines for Individualizing Patient Care, ed 6 (Doenges,Moorhouse, & Geissler-Murr, 2002); PsychiatricCare Plans: Guidelines for Individualizing Care, ed 3(Doenges, Townsend, & Moorhouse, 1998); andMaternal/Newborn Plans of Care: Guidelines forIndividualizing Care, ed 3 (Doenges & Moorhouse,1999).Chapter 6 contains over 800 disorders/healthconditions reflecting all specialty areas, with associated nursing diagnoses written as client problem/need statements to aid you in validating the assessment and diagnosis steps of the nursing process.vii

In Chapter 5, the heart of the book, all the nursingdiagnoses are listed alphabetically for ease of referenceand include the diagnoses accepted for use byNANDA through 2004. The alphabetization of diagnoses follows NANDA’s own sequencing, wherebydiagnoses are alphabetized first by their key term,which is capitalized. Subordinate terminology ordescriptors of the diagnosis are presented in lowercasewords and are alphabetized secondarily to the keyterm (for example, chronic Pain is alphabetized underP). Each approved diagnosis includes its definitionand information divided into the NANDA categoriesof Related or Risk Factors and Defining Characteristics. Related/Risk Factors information reflectscausative or contributing factors that can be useful fordetermining whether the diagnosis is applicable to aparticular client. Defining Characteristics (signs andsymptoms or cues) are listed as subjective and/orobjective and are used to confirm actual diagnoses,aid in formulating outcomes, and provide additionaldata for choosing appropriate interventions. We havenot deleted or altered NANDA’s listings; however,on occasion, we have added to their definitions andsuggested additional criteria to provide clarificationand direction. These additions are denoted withbrackets [].NANDA nursing diagnosis labels are designed tobe multiaxial with 7 axes or descriptors. An axis isdefined as a dimension of the human response thatis considered in the diagnostic process (see theAppendix). Sometimes, an axis may be included in thediagnostic concept, such as ineffective communitycoping, in which the unit of care (i.e., community) isnamed. Some are implicit, such as activity intolerance,in which the individual is the unit of care. At times, anaxis may not be pertinent to a particular diagnosis andwill not be a part of the nursing diagnosis label. Forexample, the time frame (e.g., acute, intermittent) orbody part (e.g., cerebral, oral, skin) may not be relevant to each diagnostic situation.Desired Outcomes/Evaluation Criteria are identified to assist you in formulating individual clientoutcomes and to support the evaluation process.Suggested NOC linkages to the nursing diagnosis areprovided.Nursing priorities are used to group the suggestedinterventions, which are directed primarily to adultcare, although interventions designated as across thelife span do include pediatric and geriatric considerations and are designated by an icon. In general, theinterventions can be used in multiple settings—acuteviiicare, rehabilitation, community clinics, or home care.Most interventions are independent or nursing-originated; however, some interventions are collaborativeorders (e.g., medical, psychiatric), and you will needto determine when this is necessary and take theappropriate action. Icons are also used to differentiatecollaborative interventions, diagnostic studies, andmedications as well as transcultural considerations.All of these “specialized” interventions are presentedwith icons, rather than being broken out under separate headings, to maintain their sequence within theprioritization of all nursing interventions for the diagnosis. Although all defining characteristics are listed,interventions that address specialty areas outside thescope of this book are not routinely presented (e.g.,obstetrics/gynecology/pediatrics), except for diagnoses that are infancy-oriented, such as ineffectiveBreastfeeding, disorganized Infant Behavior, and riskfor impaired parent/infant/child Attachment. Forexample, when addressing deficient Fluid Volume,isotonic (hemorrhage), the nurse is directed to stopblood loss; however, specific direction to performfundal massage is not listed. Additionally, in supportof evidenced-based practice, rationales are providedfor the interventions, and references for these rationales are cited.The inclusion of Documentation Focus suggestions is to remind you of the importance and necessityof recording the steps of the nursing process.As noted, with few exceptions, we have presentedNANDA’s recommendations as formulated. Wesupport the belief that practicing nurses andresearchers need to study, use, and evaluate the diagnoses as presented. Nurses can be creative as they usethe standardized language, redefining and sharinginformation as the diagnoses are used with individualclients. As new nursing diagnoses are developed, itis important that the data they encompass are addedto the current data base.
