Initially, SAGE grew out of a twoyear study by the social studies department of the SummitCollege Club. A local council on aging emerged from theclub’s commitment to educate itself and the communityon issues affecting older residents. Council representationcame from civic groups, churches, social agencies, andinterested residents and resulted in the formation of SAGE– Summit Association for Gerontological Endeavor.SAGE Eldercare is distinguished among its field as beingone of the oldest eldercare agencies in New Jersey and alsoone of a few nonprofit eldercare agencies in the UnitedStates that offers a comprehensive array of services underone roof.SAGE is a pioneer in providing care for older adults,beginning with its HomeCare services in 1954. SAGEstarted one of New Jersey’s first Meals on Wheels programsin 1966 and opened New Jersey’s first adult day healthcare program in 1975. Many of the services and programsprovided by SAGE have served as models for those used byother public and private agencies. ASAGE ADULT DAY PROGRAMSpend your days at SAGEand your nights at home.With more than four decades of experience, theSAGE Spend-A-Day Adult Day Medical Health Centerprovides an interactive environment for clientsto enjoy memory enhancement activities, groupdiscussions, active games, exercise programs, andholiday celebrations. Individual needs and interestsare incorporated to provide a comprehensiveprogram for each participant. Spend-A-Day alsoprovides an excellent opportunity for increasedsocialization. Judging by the number of smiles wesee each day, we must be doing something right!In addition, medical research continues to advocatefor the benefits of structured social, physical andcognitive activities for individuals with, or at risk for,memory challenges. SAGE ADULT DAY PROGRAMWHO SHOULD CONSIDER THESPEND-A-DAY PROGRAM?Spend-A-Day supports older adults living in their home, orwith a caregiver, by providing a safe home away from home.It also provides a welcome respite for spouses, adult children,their families, and other caregivers. Spend-A-Day may be anespecially good option when your loved one:A TEAM OF PROFESSIONALSSAGE’s Spend-A-Day Adult DayMedical Health Center is staffed bycaring, experienced, interdisciplinaryprofessionals, including:· Cannot be safely left alone in the home· A full-time Registered Nursewho provides monitoring andassistance with medications,blood pressure, diabetes, andweight management· Would benefit from care related to dementia(including Alzheimer’s), post-stroke, post-surgery,cardiac illness, diabetes management, or otherconditions· A full-time licensed GeriatricSocial Worker who providesassistance and support toclients, families, and caregiversTHE PROGRAM INCLUDES:Care plans are developed bynursing, social services, therecreation team, and caregiver inputunder the guidance of the client’sprimary physician. CSAGE CARE PLANNINGThe right plan can makeall the difference.Our Guidance, Planning, and Support (GPS) Servicesprovide personalized care planning to maximizeindependence, safety, and well-being. Figuring outwhat is right can be difficult; our program helps youmake the best choices by evaluating your specificsituation.EXPERIENCED, TRAINED PROFESSIONALSSAGE’s GPS Care Navigators are Registered Nurses(RNs) who specialize in geriatric care. We beginwith a detailed assessment of your physical andemotional health, support networks,and community resources. The RNs will review yourmedications and healthcare regimen, conduct ahome safety evaluation, and assess your individualneeds. You will then be given a detailed writtencare plan as well as guidance and support to helpyou implement the plan.8 www.sageeldercare.org

SAGE CARE PLANNING“GPS service has been a lifesaver to me because I was sooverwhelmed. I could not imagine caring for my parentswithout GPS support. GPS is a godsend and worth itsweight in gold.WHO SHOULD CONSIDER GPS SERVICES?Anyone who has concerns about a loved one’s care shouldconsider GPS. This service is especially helpful during a time oftransition, for example:· Returning home after a hospital or rehabilitation stay· Receiving a new diagnosis and/or medication regimen· When a loved one is experiencing medical and/orbehavioral changes· When a loved one can no longer remain at home alone· Planning for future care· When assistance is needed for long-distance care givingAn assessment includeseverything from amedication review tochecking to make surenutritional needs arebeing met. MSAGE MEALS ON WHEELSMore than a meal.At the core of SAGE’s Meals on Wheels service isa nutritious meal, companionship, and a watchfuleye on the health and safety of our clients whoare frail, homebound, or disabled. This includespeople recuperating from an illness, cognitivelyor physically impaired, without family or friendsto shop for them, or people needing a special diet.There are no income requirements.10 www.sageeldercare.org“The caregivers who deliver ourmeals are wonderful, cheerful,caring and just unbelievable. SAGE MEALS ON WHEELSSAGE Meals on Wheels provides thorough in-home support withancillary services like licensed Social Work Support, Meals withFriends, Grocery Shopping, Errands, and an Emergency FoodPantry. Caregivers also gain peace of mind and respite as SAGEpartners with them in caring for their loved ones.Meals are planned by a registered dietician. In some areas, weare able to provide special diets, including diabetic and renalmeals. All meals are relatively low in fat and sodium. The mealsare delivered Monday through Friday and can be scheduled fora short time or indefinitely. Extra food for the weekend can bedelivered with the Friday meal for those in need.Caring Meals on Wheels volunteersdeliver meals and visit briefly withprogram participants between11:00 am and 12:30 pm.Clients must be home to receive their meals, unless specialarrangements are made. GSAGE GROCERY SHOPPING& ERRAND SERVICENeed things done?Let SAGE do the run.If you are over the age of 60 and need helpwith your grocery shopping, errands, or laundry,SAGE can do it for you. This service includes:· Grocery shopping· Errands to the post office, library, city hall,and pet store· Laundry and dry cleaning pick upand delivery· Check-in/safety visits· More – just ask! SAGE GROCERY SHOPPING & ERRAND SERVICE“The service has been extraordinary. Thevolunteers are very courteous, sincere andfriendly. I just can’t thank them enough.They are beautiful, caring people.”SHOPPING SERVICEDeliveries can be once a week or once in a while, whatever youneed.
