Before You Choose a PlanTo choose the best plan for your needs, you should:Understand metal levelsLook into financial assistanceUnder the Affordable Care Act (ACA) there arefour categories of health coverage—Bronze, Silver,Gold and Platinum—called metal levels. All healthplans fall into a metal level depending on the shareof healthcare expenses they cover. For example,bronze plans have higher deductibles than othermetal level plans.There are two types of financial assistance (alsocalled subsidies) available:In Maryland, CareFirst offers plans in the followingmetal levels: Bronze Silver GoldCareFirst also offers a Catastrophic plan(BlueChoice Young Adult) for individuals under age30 or individuals with a hardship exemption.Consider a Health Savings AccountA Health Savings Account (HSA) is a tax-exemptmedical savings account that can be used to payfor your own—and your dependents’—eligibleexpenses. HSAs enable you to pay for eligiblehealth expenses and save for future healthexpenses on a tax-free basis. We offer two healthinsurance plans that coordinate with an HSA. Lookfor HSA in the plan name.A tax credit to help pay your monthly premium—This subsidy helps reduce your monthly premium.Once you qualify, your tax credit will be sent toCareFirst and applied to your bill, reducing yourpremium. If you qualify for this type of assistance,you can use it toward the purchase of any plan—Bronze, Silver or Gold (excludes the BlueChoiceYoung Adult plan).A subsidy to lower your out-of-pocketexpenses—This subsidy helps limit how muchyou spend on out-of-pocket expenses like copays,coinsurance and deductibles. By lowering theseout-of-pocket costs, your health plan beginspaying 100% of your costs sooner than it wouldhave without the subsidy. If you qualify andwant to take advantage of this type of financialassistance, you must purchase a Silver planthrough the Maryland Health Connection atmarylandhealthconnection.gov.Note: If you are an existing member and you qualifiedfor financial assistance in 2021 and did not electautomatic reassessment, you need to contact theMaryland Health Connection. All individual and familyplans include:Prescription drug coverageAs a CareFirst member, your prescriptioncoverage includes: Prescription drug coverage Vision examination for membersover age 19A nationwide network of more than 69,000participating pharmacies. Dental and vision coverage for membersunder age 19 Enhanced diabetes benefitAccess to thousands of covered prescriptiondrugs on our formulary (drug list), divided intotiers. The price you pay for a drug is determinedby the tier it falls into.Enhanced diabetes benefit Generic Drugs (Tier 1)—Generic drugs areequally safe and effective as brand-namedrugs, but generics cost up to 85% less.* Askyour doctor if your prescription medicationcan be filled with a generic alternative.CareFirst has proactively taken steps to removefinancial barriers to diabetes medicationsand supplies. Insulin—CareFirst covers Preferred BrandInsulin at 0 with no deductible requirementfor all plans, including Health SavingsAccounts (HSAs). In addition, we’re coveringNon-Preferred Insulin, capped at 30 for a 30day supply and 60 for a 90-day supply. Preferred Brand-Name Drugs (Tier 2)—Brand-name drugs that may not yet beavailable in generic form, but have beenreviewed for quality, effectiveness, safety andcost by an independent national committeeof healthcare professionals. Diabetic supplies—for members in HSAplans, you’ll no longer have to meet yourdeductible first to get these supplies at nocost. Diabetic supplies will continue to be nocharge for all plans.† Tier 2 now includes Preferred BrandInsulin at 0. Non-Preferred Brand-Name Drugs(Tier 3)—These drugs often have a genericor preferred brand drug option where yourcost-share will be lower. You will pay morefor drugs in this tier. If you choose a nonpreferred drug when a generic is available,you will pay the non-preferred copay alongwith the difference in price between thegeneric and non-preferred drug. Preferred Specialty Drugs (Tier 4)**—Consistof drugs used to treat chronic, complex and/or rare health conditions. These drugs mayhave a lower cost-share than non-preferredspecialty drugs. That’s your rate.Buying a family planIf you are interested in a family plan, each familymember is rated individually and your rates arecombined to calculate your family premium.
