HEALTH PLANS- ANTHEM BLUE CROSS BLUE SHIELD The section below outlines the health plans offered in 2022, which are the same plans as offered in 2021.A comparison chart can be found at the end of this section. You can learn more about these plans in theBenefits Portal at wku.edu/hr/benefits/healthandwellness.THINGS TO CONSIDER WHENPICKING A HEALTH PLAN YOU’LL NOTICE WE’VE NAMED OUR HEALTHPLANS “HEALTHY ”.That’s because we hope you’ll participate in ourTopLife Employee Wellness Program. LOOKING FOR A DISCOUNT ON YOUR HEALTHPLAN PREMIUMS?If you elect to participate in our TopLife EmployeeWellness Program, we’ll give you a 15/month discount onEmployee Only and Employee Child(ren) coverage levelsor a 30/month discount for other coverage levels.OPTING OUT OF HEALTH INSURANCEDo you have health insurance already? You have anoption to opt-out of health insurance. We call this ourMedical Refusal option. We will open a Flexible Spending Account (FSA) on yourbehalf. The initial contribution is 500 and is pro-ratedmonthly for new hires.PLAN HIGHLIGHTS: The initial credit to the Medical Refusal FSA is 500.We prorate this monthly for new hires. WOULD YOU LIKE TO EARN SOME SPENDING MONEYFOR YOUR FAMILY’S MEDICAL, DENTAL AND VISIONEXPENSES?If you elect to participate in our TopLife EmployeeWellness Program, WKU will credit either an HRA or HSA(depending on your plan selection) with 500 (single) or 1,000 (other coverage levels). THERE'S A SINGLE DEDUCTIBLE WITHIN THEFAMILY DEDUCTIBLE:What does that mean? Each covered family memberworks toward their own single deductible. If one membermeets the single deductible, that member has satisfiedtheir deductible and will begin only paying the applicableco-insurance. Co-insurance paid after the single deductibleis met does not count toward the family deductible. Oncetwo or more covered members meet the family deductible, the whole family is considered as having met the deductible and will begin paying the applicable co-insurance. Thesame is true for the out of pocket maximum (OOP), eachperson works toward their own OOP.If one member meets the single OOP, that member’sclaims will be paid at 100% going forward. Once two ormore members meet the family OOP, the whole family’sclaims are paid 100%. ANTHEM BLUE CROSS BLUE SHIELD IS THETHIRD-PARTY ADMINISTRATOR OF WKU’SSELF-INSURED GROUP HEALTH PLANWKU’s health plan includes access to Anthem's BlueAccess PPO Network, which offers a large selection ofnationwide providers.88 Earn additional credits by participating in the TopLifeEmployee Wellness Program. You’re eligible for up toa 500 cash incentive. If you complete the VitalityHealth Review by December 17, 2021, we will add 250 toyour last March 2022 paycheck. If you earn a minimumof 5,000 total points by September 1, 2022, we’ll add anadditional 250 to your last October paycheck. New Hires through through May are eligible for theMarch 250 cash incentive and/or October 250 cashincentive if the Vitality Health Review is complete onsource reports used for administering the payouts. When you elect the Medical Refusal option, we’ll askif your family contributes to a Health Savings Account(HSA). If you do, you’ll need to select the LimitedPurpose Flexible Spending Account. This means you canuse your FSA for dental and vision expenses only. If you’d like, you can also open your own FSA andcontribute up to 2,750 pre-tax dollars (subject tochange based on IRS guidelines).
