NY Connects Niagara County offers information and assistance, education and screening toindividuals, their families and/or caregivers, as well as people employed in the helping serviceprofessions. It assists Niagara County residents in making informed choices regarding theirlong term care needs by letting residents know who provides those services nearest to themand how to get in touch with service providers.The goal of NY Connects Niagara County is to assist Niagara County residents in identifyingavailable long term care options for them or their loved ones by providing accessible,coordinated, unbiased and person-centered information and assistance services. By providinginformation and assistance about available services, it is also our goal to enable a person toremain in their home as long as possible.Every effort has been made to ensure the information contained in this guide is accurate andcurrent. What is Long Term Care?Traditionally, long term care only meant care provided in a nursing home. Today, long termcare includes medical and non-medical services that are provided in a variety of settings. What are Home Health Care and Private Duty Nursing Services?Home Health Care programs make necessary medical services available in the homes ofpeople who are aged, ill, disabled or recuperating from an illness and/or surgery.Private Duty Nursing are programs whose home health care services are provided by alicensed vocational nurse and/or registered nurses who are not members of a hospital staff butare called upon to take special care of individual patients.Source: http://www.211taxonomy.org, February 1, 20088
