PROPOSAL PROCESSComplementary and fully insured, ArmadaCare’s products are dynamically underwritten to thedemographics of the covered census. GETTING STARTED WITH USLICENSING AND APPOINTMENTSWe do not require brokers to become appointed inorder to receive a pricing proposal. When you sellyour first case, we make the appointment processpart of our group set-up. At that time, you willcomplete the following: Request for Appointment Form Direct Deposit Authorization Copy of Your Agency’s License (provided by you) W-9 (required annually)BROKER OF RECORD (BOR) CHANGESIf you are the new producer for an existingArmadaCare client and not yet appointed with theappropriate carrier, you’ll need to submit theappointment paperwork (listed at the left). All BORchanges require completion of this Broker of RecordChange Form, as well as a letter from the client oncompany letterhead.We will implement the BOR change for the 1stof the following month that we receive it along withall appropriate paperwork. For example, if we get aBOR change on August 15 with an effective date ofAugust 1, it will go into effect for ArmadaCare onSeptember 1 once all paperwork has been providedand reviewed. Retroactive changes will not bemade.8
