By routinely checking the equipment and insuring that it is functioning well andstructurally safe, you can decrease the chance of an accident at the loading dock area.Preventive maintenance is not only a factor in providing safe and useable equipment, it can alsomaintain productivity and efficiency levels for your business. Since the dock is the first and the lastpart of the product cycle in most facilities, your equipment should be performing at its highest levelall of the time.Page 1 of 11

Loading Dock EquipmentBENEFITS OF A PREVENTIVE MAINTENANCE PROGRAMOVERVIEW OF PREVENTIVE MAINTENANCE PROGRAMThe purpose of this program is to establish a consistent routine of maintenance for your loadingdock equipment, while providing for the purchase of repair parts and labor or emergency service asrequired. Longer life, more reliable product performance and early detection of any possibleoperational malfunctions are the objectives of our Preventive Maintenance Program. (PMP)As a property owner/manager, you are sensitive to maintenance costs and the need for reliabilityand the security provided by the loading dock equipment in your facility. If this equipment is out ofoperation, it can result in the necessity of added security expense or worse, a facility that is notsecure. It is essential that your loading dock equipment be properly maintained to minimize costlydowntime.Through this program you can expect to decrease the volume of dock equipment problems. Thiswill be accomplished through periodic inspections combined with proper lubrication andadjustments, as described in the following pages.To maximize the effectiveness of this program, a priority file will be established for your company.This file will contain the following data:A.A detailed map will indicate the physical location of all your dock equipment,identified by a number already established by your company or assigned by our PMteam personnel. This will simplify communications when requesting repair services.B.Specific information will be maintained on all dock equipment. Manufacturer,serial number, size, type, and similar important data in the file will allow promptresponse of personnel and equipment to properly perform service on the equipment.C.A detailed record of all inspections, results, actions taken and any possible requiredactions will be given to you after each inspection plus a copy will be put in your file.Experience has proven that periodic preventive maintenance and careful inspection can reduceboth downtime and repair expenses while at the same time extending your equipments useful lifeand operating efficiency.This periodic maintenance program is not a guarantee against failure of, or damage to, thedesignated dock equipment, but rather it is an important asset in the overall effective managementof your facility. This program provides for routine inspections in accordance with agreed termsand conditions. This program does not offer extended product warranty nor does it relieve theowner/manager from his daily responsibilities of housekeeping and proper operation of the loadingdock equipment.Page 2 of 11

Loading Dock EquipmentBENEFITS OF A PREVENTIVE MAINTENANCE PROGRAMCONDITIONS OF AGREEMENTConditions of the maintenance and periodic inspection agreement are as follows:1) The program covers normal maintenance limited to routine inspections, adjustments, lubrication and tightening offasteners. It does not include any replacement parts or the labor for their installation, if required after inspection.Periodic inspections and maintenance as outlined herewith will be conducted as per schedule noted for loading dockequipment: (place an “L” for levelers. “S” for seals/shelters. “R” for restraints. “A” for ther (specify)This program will cover loading dock equipment. The total sum for periodic PMP program shall be OPTIONS:Please indicate which of the following options will apply to this agreement in reference to repair work that mightbe recommended after inspections are completed:[A] Necessary minor replacement parts up to the amount of will be installed when performingpreventive maintenance and invoiced as extra to the agreed upon periodic fee for preventive maintenanceinspection.[B] A detailed list of recommended replacement parts will be supplied after each inspection with a quoted pricefor parts and installation. Authorization must be obtained prior to proceeding with installation oradditional work.2) Work performed under this agreement will be accomplished during normal work hours betweenAM andPM Monday through Friday. This agreement does not cover emergency service calls, which will be handledat our emergency rate of per man-hour for calls dispatched between the hours ofPM andAM.Overtime rates shall commence when any call is received afterPM and will be performed at the rate of per man-hour. Special overtime rates will apply on weekends and holidays and when service work isperformed between the hoursPM andAM on any day, at a rate of per man-hour.3) The Preventive Maintenance Program (PMP) will NOT cover the following:a. Damage to loading dock equipmentb. Any electrical problem created by insufficient voltage or incorrect electrical hook-up.4), an authorized distributor of Poweramp, a Division of Systems, LLC, shall not be held liable for specific,or misuseincidental or consequential damage or losses caused by any acts beyond the reasonable control ofor abuse of loading dock equipment by others.5) Service will not be rendered under this agreement in any cases that the customer has a past due account.6) It is agreed that either party will have the right to cancel this program at any time with written notice.Page 4 of 11
