qualityImprovementMethodist Hospitals Leads The Waywith Groundbreaking Research on SafetyImprovements in Intravenous Therapy.Devices called disinfection caps are often used with central lineIVs to reduce the incidence of blood stream infections. Methodistnurses proposed their use as one of the interventions to helpreduce these infections. Infection control and nursing collaboratedto conduct research that demonstrated that the use of thesedevices throughout the hospital on peripheral, as well as central IVlines, resulted in 45% fewer overall infections. This research waspublished in the Journal of the Association for Vascular Access.This publication and other initiatives have led major nationalhealth providers to begin implementing similar practices intheir systems.8 - Annual Report 2014

Methodist Hospitals’ highest priority is the safety and quality of the care we deliver topatients. And we continuously strive to improve our quality and safety performance.Some of these efforts are highlighted below.Value Based Purchasing (VBP):Partnership for Patients:The Federal Government evaluates the performanceof hospitals on key quality measures, including safety,patient experience and outcomes of care, creatinga “Total Performance Score” for each hospital.Penalties and bonuses for individual hospitals areassessed based on this score.This American Hospital Association-affiliatedprogram aims to reduce inpatient harm by 40% andhospital readmissions by 20% in 11 target areas.Methodist Hospitals made significant strides towardmeeting these goals in 2014.Methodist Hospitals’ VBP total performance scorein 2014 exceeded both state and national averages,earning a bonus of 200,000. Methodist was amongonly 25% of hospitals nationally to earn a VBPbonus payment.VBP Total Performance verage In three areas, Methodist, not only achieved a 40%or greater reduction but engaged in improvementinitiatives that go beyond the program goals,making it a potential Mentor Hospital:– Preventing venous thromboembolisms– Preventing central line-associatedblood stream infections– Preventing early elective deliveries Methodist is rated as demonstrating sustainedhigh performance or a national benchmarkin preventing:– Adverse drug events– Pressure ulcers– Adverse obstetrical events– Patient injuries from falls and immobility– Surgical site infections– Readmissions2014ANNUALREPORT Methodist was rated as demonstrating outstandingimprovement in preventing catheter-associatedurinary tract infections. Methodist is currently working on improvementsin preventing ventilator-associated events andsubmitting data on outcomes.Total Joint ReplacementQuality Initiative:One of several service line-specific improvementefforts, the work of a multidisciplinary team hasresulted in significant improvements: 23% decrease in complications 35% drop in length of stay Maintenance of 0% mortality rate andreadmission rate below 5%9

In 2014 Methodist madea significant effort to help community memberstake advantage of the Health Care Marketplacesestablished by the Affordable Care Act and the stateHIP 2.0 program to expand Medicaid. accomplishmentshighlights of 2014Methodist Hospitals is First inNorthwest Indiana to Achieve Designationas a Level III Trauma CenterA decade-long aspiration and goal for Methodist Hospitalswas realized in 2014 when official approval came from theIndiana State EMS Commission for Methodist Hospitals’Northlake Campus to be considered an “in the process”Level III trauma center. With this approval, the NorthlakeCampus Emergency Department now becomes the state’s16th trauma center and the first in Northwest Indiana.Methodist Hospitals’ team displays the Trauma Designation.12 - Annual Report 2014This designation means that EMS providers may nowtransport the most seriously injured patients within a45-minute radius to Methodist Hospitals’ Northlake Campus.In addition, trauma center status, with its emphasis on thecoordination of care from the patient’s first contact with EMSthrough all phases of care, has a ripple effect that raises thelevel of care across the whole organization. That is good newsboth for Methodist Hospitals patients and for the hospitals’commitment to providing the very best care possible.

The coreservices offered in the Multi-Specialty Centerare those of the Orthopedic and Spine program,including orthopedic surgeons, spine surgeons,specialists in podiatry and podiatric surgery andclinics devoted to the treatment of conditions suchas osteoporosis and scoliosis. The Multi-SpecialtyCenter will also include specialists in other highneed areas, such as general surgery and neurology.2014ANNUALREPORTMethodist HospitalsHome Health Named oneof the Top 500 in the USMethodist Hospitals was named tothe Top 500 of the 2014 HomeCareElite , a recognition of the top-performing homehealth agencies in the United States. The HomeCareElite identifies the top 25 percent of agencies andhighlights the top 100 and top 500 agencies overall.Winners are ranked by an analysis of publiclyavailable performance measures in quality outcomes,best practice (process measure) implementation,patient experience (Home Health CAHPS ),quality improvement and consistency, and financialperformance. In order to be considered, an agencymust be Medicare-certified and have data for at leastone outcome in Home Health Compare. Out of9,994 agencies considered, 2,501 are elite.The Midlake Campus Multi-Specialty Center13

Methodist Hospitals invested 91.3 million in community benefit in 2014.Helping Youth MakeHealthy Life ChoicesSupporting Regional Civic andHealth OrganizationsMethodist Hospitals partnered with the GarySchool Corporation and Workforce DevelopmentServices to bring a 5-week healthy choices programto Roosevelt College and Career Academy and WestSide High School students. The program addressedhigh profile health issues in the teenage population.Methodist supports a wide variety of communityorganizations serving the Northwest Indianacommunity, including the Indiana HospitalAssociation, the Northwest Indiana HealthDisparities Council, the Urban League of NorthwestIndiana, the Crossroads, Gary, Lake Shore andValparaiso Chambers of Commerce. The hospitalis also a leader in the support of health advocacyorganizations, including the American HeartAssociation, the American Cancer Society, the PinkRibbon Society, the Multiple Sclerosis Society, theParkinson’s Foundation and the March of Dimes.Taking a StandAgainst Youth ViolenceMethodist Hospitals is committed to the preventionof youth violence, offering programs for local youthwith partners such as Indiana University Northwest,Community Organizations for Families and Youth(COFFY), Merrillville and Gary high schools and thePOP on Violence Foundation.Promoting Women’s Health –Annual First Ladies Health InitiativeMethodist Hospitals is a partner and continuoussupporter of the First Ladies Health Initiativeto better meet the health needs of women andtheir families. Mayor Karen Freeman-Wilsonproclaimed September 28th as “First Ladies HealthDay”, throughout the communities of Gary andMerrillville.YWCA & YMCA PartnershipsCommunity Health InitiativesMethodist Hospitals demonstrates its commitmentto improving community health through a wide arrayof community outreach programs, including dozensof regular screening programs, support groups andclinics, as well as free community health fairs andeducational seminars led by physicians and otherexperts.Introducing High School StudentsHeart Disease and Stroke: A total of 35 eventsprovided education and screenings to more than2,500 community members.Educating Organized Labor GroupsMethodist provides educational programs for locallabor groups on topics such as health care reform,workforce development and health care career trends.REPORTAssessment Outreach targets healthconditions identified as presenting barriers to thegood health of community members.Cancer: More than 2500 people were reachedin 2014 with screenings, education and riskassessments in the effort to improve early detectionand treatment of skin, colon, breast, lung andprostate cancers.– In 2014, Methodistcontinued its intern program to give studentscomprehensive exposure to clinical and non-clinicalcareers in health care.ANNUALCommunity Health NeedsMethodist Hospitals’ outreach efforts at the GaryYWCA and Valparaiso Family YMCA provide aseries of educational sessions that address geriatrichealth, prevention of heart disease, diabetes, stroke,and offer information about nutritional health.Methodist Hospitals distributes health literature,offers health education, conducts workshops andmakes referrals to its services and programs.to Health Careers2014Diabetes: 22 programs reach more than 2000 adultsand children. The reach of our Diabetes Center wasextended to Porter County.A summer series of Farmer’s Markets were heldto improve the availability of fresh produce inthe Gary community.Other programs were developed to address kidneyhealth, teen pregnancies and infant mortality.15
