IntroductionBenefits ofClinicalRemindersClinical Practice GuidelinesPurpose of Guidelines Assure that the appropriate amount of care is provided(addressing both under & over-utilization) Reduce errors and promote patient safety Ensure predictable and consistent quality Promote learning and research Facilitate patient and family educationClinical Reminders, Performance Measures, andClinical Practice GuidelinesEach Veterans Integrated Service Networks (VISN) must comply withperformance measures that address Prevention Index/Chronic DiseaseIndex (PI/CDI), as well as with the Health Promotion And DiseasePrevention Program Handbook 1120.2, which states that each VHAfacility shall have a program to educate veterans with respect to healthpromotion and disease prevention and to provide veterans withpreventive medical care that includes screening and other clinicalservices.The Clinical Reminders package offers tools to help clinicians complywith these performance measures and guidelines on a patient-by-patientbasis. The use of these tools leads to improved patient care.Providers can work with their local Clinical Application Coordinatorsto set up customized reminders based on local and national guidelinesfor patient education, immunizations, skin tests, measurements, exams,laboratory tests, mental health tests, radiology procedures, and otherprocedures.The Office of Quality and Performance oversees the VA’s performancemeasure plan. Each year the Performance Measurement Workgroup(PMWG), recommends the annual Network Performance Plan to theUnder Secretary for Health. IntroductionPatch 6 Updates toClinical RemindersNOTE: Most dialogs thatuse Mental Health testswon’t exhibit manychanges until CPRS GUIV27 is released.Clinical Reminders Patch 6Patch 6 contains modifications to integrate the Clinical Reminderspackage with the new version of the Mental Package called MHA3.The Clinical Reminders package will support use of new mentalhealth surveys, instruments, and forms for clinical collection,reminder evaluation, patient list building and reporting. Thesemodifications will be distributed at the same time as MHA3(YS*5.01*85).This functionality is needed so that Clinical Reminders can be used tohelp sites meet the Performance Measure requirements related to astandardized set of Mental Health Instruments that will be availablein the YS*5.01*85 patch. The standardized instruments are AUDC,BDI2, PHQ-2, PHQ9, and PC-PTSD.GMTS*2.7*77, bundled with PXRM*2.0*6, provides two newHealth Summary Components to view administered mental healthtests and scores: MHAL - MHA Administration List and MHAS MHA Score.In order to use all of the dialog functionality available with MHA3and PXRM*2*6, Version 27 of the CPRS GUI will need to beinstalled. Clicking on oneof them will open your web browser at that site.Clicking on Reminder Icon Legend will bring up a display that showswhat the various reminder icons mean. Using Clinical RemindersChapter 1: CPRSand RemindersOverviewClock ButtonAnother place you can interact with Clinical Reminders is by clickingon the reminders button in the upper right hand corner of the CPRSGUI. The reminders button looks like an alarm clock and correspondsto the status of the reminder, as indicated in the icon legend shown onthe previous page.This brings up the Available Reminders window, which shows thesame tree view as seen in the Reminders drawer.This window has two menus: View and Action.View MenuThe View menu lets you determine which categories of reminders willbe displayed in the tree view. Those with a checkmark to the left of thiswill be displayed. You can toggle the checkmark on or off by leftclicking on the icon. Note: as soon as you click on an icon the Viewmenu will disappear and the tree will be updated to match your currentselection. A processed reminder is one whosedialog has been pr cessed by checking off items; a checkmark appearsby the reminder ic n. The option that is selectable out of these threeoptions depends onwhat has been selected on the reminders tree. When youclick on the Notes tab and open a new note, a Reminders tab appears.You or your site candetermine the folder view,and whether the folders areopen or closed when youfirst open the reminders tab(also called a drawer).Reminders DrawerUsing a dialog to resolve aclinical reminder isdiscussed in Chapter 2.When you click on the Reminders drawer, a list of reminders isdisplayed, categorized by Due, Applicable, Not Applicable, and OtherCategories. Reminders that have an associated dialog have a specialicon (see the previous Reminder Icons Legend). Using Clinical RemindersChapter 2:Processing/Resolving ClinicalRemindersSummary of Steps to Process RemindersThese are the basic steps for processing reminders from the Notes tab inCPRS. These steps are described in more detail, as they relate to MentalHealth, in Chapter 3.1. Start a new progress note. To process a reminder, start a newprogress note. When you begin a new progress note, the remindersdrawer appears.NOTE:Your site can determinethe folder view – whichreminders andcategories/folders appearin the reminders drawer.2. Open the reminders drawer. When you click on the remindersdrawer, you see several folders containing reminders for thispatient. Possible folders include Due, Applicable, Not Applicable,All Evaluated, and Other Categories. These folders may contain ahierarchy of folders and reminders within folders. The view offolders is customizable by you (see Appendix C). The folders andsubfolders in the Reminders Drawer are sometimes called the “treeview.”3. Choose a reminder. Open a folder (if necessary) and click areminder that you wish to process.
