Depending on your program of study, download and print the form for your program. This has instructions aboutexactly what Western Governors University requires you to have completed for compliance.4. If you are a student that has never had an ITS Account before:a. Click the ‘Online Registration’ button on the Main Pageb. Click ‘Online Registration’ button on the following pagec. Enter your personal informationd. Please enter your WGU student email address (American DataBank will be sending reminders to that email)e. Passwords are case-sensitive, so be careful with Caps Lock functions on your keyboardf. Pay the 30 month subscription via credit/debit cardg. You will only have to create ONE ITS account as long as you are at WGUNote: Your ITS Account is different than your AMS Account (for your Background Check)5. If you have already created an account:a. Click the ‘Member Login’ button on the Main Pageb. Click ‘Member Login’ button on the following page.c. Your login is your email address, and you selected your password upon creation of the account. If you do notremember your password, be sure to click the ‘Forgot Password?’ link6. Take your documents and scan them into the computer. If you do not have a scanner, frequently you can find one ata local library, or any copy store (FedEx Kinko’s, UPS Store, Office Depot, etc.). You can also use a digital camera orsmart phone to digitally capture your records. Make sure your copies are in grey-scale, and do your best to keep thesizes smaller than 1 Mb per file. This will help later.7. Login to your account and enter in your immunization information for verification. This is required; AmericanDataBank will not enter your information for you. Click one of the ‘Input Date’ buttons to get started.8. Be sure to enter a date of the item, a provider/hospital’s name, the date the item was proven/signed, and any results(if applicable) into the system. You can enter one item, Save AND Confirm, but you must finish the whole line for theitem to save correctly. If the item doesn’t have a ‘provider’, such as a CPR Card, put in the organization or hospital yougot it through. American DataBank will check all entries for accuracy when you submit documentation.9. Take your electronic documents and upload them directly into your profile. The ‘Add a Document’ link is near thebottom of the screen you are putting your dates into. There are help videos in the Help Menu to the left when you login,which will show this entire process start to finish.10. The ITS will only accept certain file types, they are jpg, gif, png, pdf, word, and txt. Most word processing softwarewill actually allow you to save as a Word Document. You can paste any images you have into the document and upload itthat way if you like.11. Click ‘Add a Document’. The screen will re-fresh and you will see a ‘Browse’ button.12. Click ‘Browse’ to add a document. You can add up 5 documents, totally 300 Kb. If your file is bigger, you will needto upload one at a time. Try to include what is in the file, to ensure it isprocessed accurately.14. Click ‘Add Document’ to upload the document. This might take a while if your file is very large.15. Click ‘Confirm Upload’ if your document is okay. You can click the little picture on the left to verify you uploaded thecorrect item. Once you ‘Confirm’ you will not be able to delete it without emailing American DataBank.16. All uploaded documents will appear immediately in the Supporting Documents section. This is viewable by AmericanDataBank, yourself, and authorized administrators at your school.17. If you were typing in items as well, be sure to ‘Save’, and ‘Confirm’. The ‘Save’ button is at the bottom of the screen.If you have any errors, the screen will refresh with red bars around the error sections and descriptions of the errors ontop of the screen.18. Once you correct any errors, you should see a Confirmation screen. You MUST click ‘Confirm’ to save. If you leavebefore clicking ‘Confirm’, your typewritten data will NOT save.19. Once you save and confirm correctly, your account will appear ‘Modified’ for each item you entered. That is how youknow you have done it right. Any new documents you added will appear as ‘DocReceived’ with your name and the timeyou uploaded them.20. Expect your account to be updated within 1-3 Business Days. American DataBank is not open on the weekend, andis closed on some Federal Holidays.21. Check back into your account, as it will have your most current status. Expirations will appear in the center column,and you will get emails sent automatically as items expire in your account.Expired Item? Be sure to submit new documentation to American DataBank. MMR (Measles, Mumps and Rubella) Vaccines or Titer: One TimeYou must submit either a or b for each disease:a) Proof of a 2-shot MMR series (or the equivalent shots for each disease) ORb) Immune Antibody IGG Titer for Measles, Mumps and Rubella.2. TDap (Tetanus, Diphtheria, and acelluar pertussis): Every Ten YearsYou must have a Tetanus, Diphtheria, and acellular pertussis (Tdap) vaccination within the last ten years forcompliance. If you are unable to provide appropriate documentation to prove you have had a vaccination, youmust get another vaccination.3. Varicella Vaccines or Titer: One TimeYou must submit either a or b:a) Proof of a 2-shot Varicella series ORb) Immune Antibody IGG Titer for Varicella.4. Hepatitis B: One TimeYou must have either a or b:a) Proof of the 3 shot series for Hepatitis B (or in the process of receiving the series) ORb) Immune Antibody IGG Titer for VaricellaSubmit Documentation on Items 5-115. TB Testing (PPD): One TimeYou must submit documentation of a current PPD.**If you have never had a PPD you must have documentation of a 2 step PPD (with PPD #2 given no less than 5days after PPD 1 but no further than 10 days after PPD #1).*If you have a Positive PPD:You must submit proof of a Chest X-Ray (Every Two Years) AND a TB Screening Report (Annual—SEE PAGE 5)from your provider. The report must state that you, the patient, are ‘negative for signs and symptoms oftuberculosis’. This report must be submitted annually, with the newest within 12 months of admission to WGU.6. Seasonal Flu Vaccination: AnnualA seasonal flu vaccination is required on an annual basis. Declining the flu shot may preclude you from beingable to attend clinical.7. CPR Certification: Expires every Two YearsYou must have a CPR Certification from the American Heart Association (AHA) on file before the beginning ofyour first clinical nursing course.
