Dixie Highway, Suite B, Cutler Bay, Florida 33157. Alearning resource center is available with internet access, computerstations, web-based resources, health reference books, andperiodicals. Medical labs are equipped with medical exam tables,computers, microscopes, stethoscopes, blood pressure cuffs, EKGmachines and other medical training equipment as applicable.Computer labs include student computer stations with internetaccess and word processing, presentation, spreadsheet, database,and medical billing/coding software applications. Lecture roomshave internet connectivity, media cabinets, and digital projectors.There is a student lounge with vending machines and microwaves.Library resources and library information services are available tostudents and faculty. Fortis College also uses the offices andlaboratories of local physicians, clinics, healthcare facilities andhospitals to provide on-the-job experiences for students.Administrative offices include academics, student and careerservices, financial aid, registrar, admissions, and business offices.There is a faculty workroom and faculty offices. All records received become property of FortisCollege.Associate Degree ProgramsNursing11111316(Applicants to the Associate Degree in Nursing program who holdcurrent PN licensure or who are graduates of a FortisCollege/Institute or MedVance Institute PN program are exemptfrom taking Wonderlic exam.)GENERAL ADMISSION REQUIREMENTSNote: In the event an applicant desires special accommodations forthe SLE, the applicant must follow the policies in this catalog torequest those accommodations. Campus staff members are notauthorized to approve accommodations for admissions testing.1. The applicant must be a high school graduate or possess therecognized equivalent of a high school diploma. The applicant mustprovide documentation of graduation from high school or college in theform of a valid high school diploma or higher earned degree,transcript, or other acceptable documentation which confirms that theapplicant meets or exceeds the academic achievements equivalent toa standard high school diploma earned in the USA as defined by theState where the diploma was earned. All documents from foreigncountries must also be translated into English and evaluated to beequivalent or higher than a USA high school diploma by a credentialevaluation service.7. Applicants must pay the enrollment fee and complete all tuitionpayment requirements.8. Accepted applicants must agree to and sign the Fortis CollegeEnrollment Agreement.ADDITIONAL ADMISSIONS REQUIREMENTS FOR THEMEDICAL ASSISTING WITH BASIC X-RAY OPERATION(MABXO) PROGRAM2. The applicant must be seventeen years of age or older at the timehe or she starts his or her program of study.Applicants must submit to and pass a healthcare student criminalbackground check prior to starting the program. Results must be inthe applicant’s admission file before starting the program. Theapplicant should note that a history of criminal convictions mayprevent the student from attending or completingtheclinical/externship requirements of the program or may prevent his orher credentialing in the profession. If there are any questionsregarding the results of the criminal background check, the applicantmust make an appointment with the program director to discuss theconsequences of the results which may include denial of programadmission.3. The applicant must complete an applicant information form.4. The applicant must be interviewed with an admissionsrepresentative and/or other administrative staff.5. Applicants who otherwise meet the requirements to pursue aselected program of study, will be given the opportunity to take theWonderlic Scholastic Level Exam (SLE). Applicants to the college whodo not achieve a passing score are eligible to immediately retake asecond version of the SLE. (See below for the SLE minimum scorerequirement for each program of study.) In the event that theapplicant fails to achieve a passing score on the secondadministration of the SLE, the applicant is eligible to take the SLE athird time using another version; however, a minimum of seven daysmust elapse after the second testing date before the third SLE may beadministered. In addition, this administration and subsequentadministrations requires approval by the Campus President. A fourthand final administration of another version of the SLE is permittedonly after a minimum of 180 days has elapsed since the date ofthe third test administration. Applicants who choose to take theentrance test for the fourth and final time are strongly encouraged topursue remediation in reading and math prior to testing for the fourthand final time to assist in strengthening their critical thinking skills.ADDITIONAL ADMISSIONS REQUIREMENTS FOR THEPHARMACY TECHNICIAN PROGRAMApplicants to the Pharmacy Technician program must submit to andpass a criminal background check and must be cleared per schoolpolicy. The applicant should note that a history of criminalconvictions may prevent the student from attending, or completingthe requirements of the program, or may prevent him or her fromobtaining appropriate credentials to work in the occupational field. Ifthere are any questions regarding this process, the applicant shouldmake an appointment to speak with the Program Director and/or theDean of Education.6. Once a passing score is earned, the SLE score is valid for threeyears from the date of administration. Applicants for re-admissionmust achieve the passing score on the SLE required of currentapplicants for admission to the selected program of study. If theapplicant for re-admission had previously achieved a passing scoreon the SLE, that score may be used for re-admission, provided theSLE test was administered and passed within three years (36 months)of the date of readmission and still meets the current minimumacceptable SLE score for the applicable program.ADDITIONAL ADMISSIONS REQUIREMENTS FORHYBRID PROGRAMSIn addition to the General Admissions Requirements, the following isan additional admissions requirement for students enrolling in ahybrid program.1.-6 -Theapplicant must signa StudentInformation

Applicants may be admitted to the nursing programwith an A2-RN score of 75% or higher on (1) English languagecomposite score comprised of reading, grammar, and vocabulary and(2) math composite score.ADDITIONAL ADMISSIONS REQUIREMENTS FOR THEASSOCIATE DEGREE IN NURSING PROGRAMIn addition to the General Admissions Requirements, the followingare additional admissions requirements for the Associate Degree inNursing (ADN) program.Applicants to the ADN program who are Licensed VocationalNurses (LVNs) or Licensed Practical Nurses (LPNs) mustmeet the following requirements:Applicants to the ADN program who are not licensedvocational or practical nurses must meet the followingrequirements:1. The applicant must be a graduate of a state-approved VocationalNursing (VN) or Practical Nursing (PN) program. Applicants who arenot graduates of a Fortis College/Institute or formerly MedVanceInstitute VN or PN program must have a cumulative grade pointaverage of 2.8 on a grading scale of 4.0. The applicant must providedocumentation of graduation in the form of an official transcript.Exceptions to this requirement may be considered in extenuatingcircumstances (such as transcripts only showing pass/fail;graduated from a military school and no GPA is provided). Therequest for a waiver must be submitted in writing to the Dean ofNursing, who will review and provide approval with guidance fromthe National Dean of Academics.1. After achieving a passing SLE score, applicants will be given theopportunity to take the Evolve Reach Admission Assessment Exam(A2-RN). Applicants may be admitted to the nursing program withan A2-RN score of 75% or higher on (1) English LanguageComposite Score comprised of reading, grammar, vocabulary and(2) math composite score.2. Prior to taking the A2-RN assessment, students will be providedan opportunity to purchase the study guide. Students are stronglydiscouraged from taking the assessment without completing thereview workbook. If the student is unsuccessful after his/her firstattempt, he/she should be directed to complete the HESI onlineremediation for at least one week before being allowed to take theA2-RN the second time. Such enrollment will allow the student,upon successful completion of the course, one more opportunity totake the A2-RN. Students can take this A2-RN assessment no morethan two times in a 12-month period. Applicants must take the A2RN exam at the campus they are requesting admission. Forapplicants who have previously taken the A2-RN exam within thelast 12 months and produce an A2-RN transcript, the fee to retakethe A2-RN exam will be waived.2. The LVN or LPN applicant must possess a current, validunencumbered Vocational or Practical Nurse license from the state inwhich he or she will be attending the ADN program.3. At the conclusion of the admission interview, the applicants whoare not Fortis College/Institute or formerly MedVance Institute VN orPN graduates will be given the opportunity to take the Evolve ReachAdmission Assessment Exam (A2-RN).
