How to Use the ICD-10-CMStructure of ICD-10-CMThe new structure will allow further expansion than was possible withICD-9-CM as more positions in more chapters are left open and availablefor future modifications. This is the updated draft version of ICD-10-CM.However, the codes in this ICD-10-CM are not currently valid for anypurpose or use. There is now an anticipated implementation date forthe ICD-10-CM of October 1, 2013 and implementation will be basedon the process for adoption of standard under the Health InsurancePortability and Accountability Act of 1996.ICD-10-CM has an index and tabular list similar to ICD-9-CM; however,the alphabetic index is more in-depth. ICD-10-CM also uses an indentedformat for both the index and tabular list. The Tabular List is similar to theICD-9-CM version of the Tabular List, containing categories, subcategories,codes and descriptors, arranged numerically within the 21 separate chapters.The coding guidelines outlining coding conventions for ICD-10-CM are inthe index, which consists of two parts: the index to diseases and injury and theindex to external causes of injury. The former ‘V’ and ‘E’ code classificationsthat were previously considered supplemental to ICD-9-CM are incorporatedinto the Tabular List in the ICD-10-CM code set as individual chapters. TheNeoplasm Table and the Table of Drugs and Chemicals are located in theAlphabetic Index to Diseases and Injury. Additionally, the Index to ExternalCauses has been included.Alphabetic Index of Diseases and InjuriesThe Alphabetic List contains terms in the ICD-10-CM code set and thecorresponding code. This List is divided into two parts: ‘Index to Diseasesand Injuries’ and ‘Index to External Causes of Injuries’. Within the Indexthere is a Neoplasm Table and a Table of Drugs and Chemicals.Alphabetic Index Instructional NotationsAbbreviationsNEC Not elsewhere classifiable – represents ‘other specified.’ Whena specific code is not available for a condition, the Indexdirects the user to the ‘other specified’ code in the TabularList.Tabular List of Diseases and InjuriesThe Tabular List consists of categories, subcategories, and valid codes. ValidICD-10-CM codes contain letters (alpha) and numbers (numeric) and maybe 3-7 digits long. The following describes the content of the specific digits: First Digit – alpha. The only letter not used is the letter ‘U’, whichis reserved for newly discovered diseases of unknown etiology. Second Digit – numeric Third Digit – numeric or alpha (not case sensitive) Fourth through Seventh Digits – numeric or alpha (not casesensitive). Note: For some ICD-10-CM codes, an ‘x’ is used as a4th-6th digit character place holder.The Letter ‘x’ as a Place HolderIf the code indicates there is a required 7th digit and there are less than six (6)characters in the code, a place holder of ‘x’ should be assigned for all missingcharacters, in order to keep the 7th character of a code always in position of the7th digit. These placeholders have been included in the Tabular List to enableusers to code more accurately, although they may appear to be out-of-order. The icon noted in front of a code indicates the code has a 7th character, such asan encounter, sequela, tophus, gestations for fetus unspecified, etc.Example:W11.xxxA Fall on and from ladder, initial encounterThe ICD-10-CM system also utilizes the ‘x’ as a 5th character place holderwith certain 6-digit codes to allow for future expansion. An example ofthis is found in the poisoning, adverse effect, and underdosing codes,categories T36-T50. Where a placeholder exists, the ‘x’ must be used inorder for the code to be considered valid.Punctuation[]()Punctuation()Parentheses – used in the Alphabetic Index to enclosesupplementary words that may be present or absent inthe statement of a disease/condition without affecting thecode number to which is is assigned. The terms within theparentheses are referred to as nonessential modifiers.Fainting (fit) R55Manifestation codesConditions/diseases that are considered manifestation codes have a specificentry structure. These codes are derived from the second editionof the International Classification of Diseases for Oncology (ICD-O).
