RoomThe Hazel Crest Park District’s C.U.R.V.E. provides a structured, before and after school program, forchildren attending Hazel Crest School Districts #144 and #152 ½. This program includes transportationto and from school. First‐time registration will require the completion of participant forms and proofof residency. Forms of residency accepted are a valid driver’s license, utility bill and mortgage/rentalagreements.Please note that monthly registration is required each month and must be received on or before the5th of every month with full payment. Registrations received after the 5th of the month will be accessed a 10 late charge. Monthly registration is also required for those families who receive financial assistance from the State of Illinois. A 100 per child co-payment is also due at time of registration. This fee is required to subsidize our daily rates not fully reimbursed by the state and administrative costs associated with accepting state assistance.A non‐refundable, one‐time registration fee of 30.00 is due each school year. PAGESummer Camp 2019 Mark Your Calendars!Ages 3‐12Community CenterThe Hazel Crest Park District Summer Camp provides a fun camp experience with a wonderfulgroup of age‐ appropriate themes, activities and field trips for our two organized groups forChildren 3‐5 and 6‐12! Regular activities will include a weekly swimming/water activity (pleasenote that a weekly swimming day may be cancelled due to availability of the swimming pooland is beyond our control) as well as games, crafts, fitness game/activities as well as fieldtrips. Additional field trip fees may be incurred, but parents will be notified as soon aspossible. We are sure your child will look forward to coming to camp with the large numberof planned activities! For our working parents, the pre‐camp and post‐camp hours will beprovided and are included in the camp fees. This is a wonderful benefit for our workingparents.
