The PHNs provide public healthfield nursing assessment and services including health and tobaccoeducation, short term medical case management, and linkage toresources.The SHOPP Behavioral Health Clinicians provide engagementservices, crisis intervention, assessment, and linkage to resourcesfor adults 60 years and older experiencing a functional impairmentrelated to a mental health issue.Substance Abuse Resource Team (START)START is an in-home service for adults 60 years and older who have aserious mental illness and who are misusing substances such as alcohol,recreational drugs, over the counter and prescription medications. STARTserves older adults unable to access traditional services geared towarddrug/alcohol abuse.
