How to Use Your DirectoryYou will find and easily locate a variety of in-network dentists within this Directory.The example below will help you understand how to read the Directory.Your plan features a variety of different types of health care providers for your oral health. Dentist – this is a general dentist. Once you select a dental office (see the next page for instructionson how to select a dental office), you can see any of the dentists within that office.Orthodontics – these are specialists that focus on Braces. No referral is necessary.Denturist – these are individuals that specialize in the fabrication of dentures. No referral isnecessary. Visiting a Denturist can often reduce your copayment for dentures and denture services.Periodontics, Endodontics, Pediatric Dentistry and Oral & Maxillofacial Surgery – these arespecialties requiring a referral for plans that include specialty coverage (see your Schedule of CoveredServices and Copayments). Please consult your General Dentist to determine if a referral is necessary.
Selecting your dentist prior to receiving careYou have the choice. Select any of the conveniently located dental offices listed in this directory. In orderto finalize your selection, you will need to provide the Dental Office number to Dental Health Services.To make your initial selection or change dentists contact a Member Service Specialist at 800-637-6453, oronline through www.dentalhealthservices.com. The selection of the dental office must be made prior toreceiving care.All dental care is received from the general dentist you select, except in the case of an out-of-areaemergency. You may receive care from any of the Orthodontists or Denturists listed in the directorywithout a referral. For plans that include specialty coverage (see your Schedule of Covered Services andCopayments), you can receive the service of Endodontists, Periodontists, Oral Surgeons, andPedodontists with a referral which can be obtained from your general dentist.EnglishIMPORTANT: Can you read this? Immediately call your Participating Dentist for care. They are available to assist you.If you are unable to reach your selected participating dental office, you can seek emergency palliative carefrom any participating dentist or any licensed dentist. No prior authorization or referral is needed. Youwill be reimbursed for the cost of emergency palliative treatment less any copayments that apply.Details can be found in your Evidence of Coverage and/or your Schedule of Covered Services andCopayments.Your Member Service Specialist is available at 800-637-6453 to answer your questions.
