Health Net Medicare Advantage (PPO) members Health Net Violet Option 1 (PPO) and Health Net Aqua (PPO) plans include benefits for complementary/ alternative care including routine chiropractic, acupuncture and naturopathy services. Additionally, some Health Net Medicare Advantage PPO plan members have access to non-Medicare covered dental and/or routine vision services through their plan and/or an optional supplemental benefit package with a separate premium. Your Health Net Medicare Advantage PPO plan gives you two levels of benefits and many choices of providers. In-network coverage – applies when you receive covered services from providers in our PPO network. Complementary/alternative, dental and routine vision care providers who are contracted with Health Net are called in-network or network providers. In-network providers are listed in this directory. Out-of-network coverage – applies when you receive covered services from providers who are qualified to provide the benefit in question and who accept the plan’s terms and conditions of payment, but do not participate in our WellNet network. These providers are referred to as out-of-network providers. It is important to note that utilizing an out-of-network provider will typically result in higher out-of-pocket costs for covered services. The level of benefits or coverage you receive is based on the provider you choose. The in-network providers listed in this directory have agreed to provide you with your complementary/ alternative, dental and routine vision care coverage. You may go to any of our in-network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider. You receive your plan’s in-network benefits when you receive covered services from a Health Net network provider and follow any applicable guidelines, such as obtaining prior authorization when it is required. Refer to your Health Net Medicare Advantage PPO Evidence of Coverage to determine which in-network services require prior authorization. Complementary/alternative care services received out-of-network must be furnished by a provider qualified to provide the benefit in question and who accepts the plan’s terms and conditions of payment. For out-of-network services, prior authorization is recommended but not required. When you receive emergency or urgently needed care from a provider who is not part of our network and you pay the entire amount yourself at the time you receive the care, you need to ask us to pay you back for our share of the cost. For Health Net Medicare Advantage (HMO/HMO SNP) members Emergency care can always be obtained in or out of the service area from the nearest available provider. In an emergency, call “911” or have someone take you to a hospital emergency room. In an emergency, you may use an in-network or out-of-network emergency care facility. When in the service area, you must use in-network providers for urgent care. When out of the service area, urgent care may be obtained from the first available provider.
