COMMITMENT TO YEARBOOK Please fill in, read, and sign the following statement. I, , understand that by completing this application, I am absolutely committing my time and talents to the FLEC Yearbook Staff. I will be responsible to learn academic skills and apply them to the yearbook. I also understand that I am required to be a positive and productive member of yearbook staff. I also may be required to spend time outside of school hours to meet my deadlines. I understand that being a part of the yearbook staff is a major commitment. It is NOT just a way to get out of class to take pictures. Yearbook is a serious responsibility, and I must be on task without redirection from Mr. Fortune or lead yearbook staff. I am fully aware that no one else is assigned my tasks, so if I do not do them on time I am hurting my co-staff, and the yearbook will suffer. By signing this application, the above named student and parent/guardian agree to abide by the rules of yearbook staff. I also understand that there are a limited number of staff positions and the selection of the staff will be based on the content and completion of this application and teacher recommendation forms. I understand that I may not be selected for any of the following reasons: Negative comment on teacher recommendation forms, or not enough information from teachers Failure to complete/turn in application on time, or to follow directions on application Lack of basic skills, as proven on this application or by teacher’s evaluation Low grades, discipline issues, or issues getting along with others (as reported by school counselors/administrators) I also understand that once selected for yearbook staff, I can be suspended from yearbook activity for the following reasons: Missing important deadlines, or not making significant progress on your assigned pages Not asking for help or letting the teacher know if you are confused about what to do Discipline issues (in yearbook class or other classes) Negative attitude, being difficult to work with, isn’t a team player Failure to stay on task in yearbook class, or using yearbook class time to do other things (playing on your phone) Altering someone else’s work without their permission Not willing to help with other yearbook work when needed Completion of the application DOES NOT GUARANTEE selection onto the yearbook staff.
