NBIC Part 2, Section 55.2 REPLACEMEINT OF STAMPING DURING IINSERVICE INSPECTION5.2.1 AUTHORIZATIONa) When the stamping on a pressure-retaining item becomes indistinct or thenameplate is lost, illegible, or detached, but traceability to the original pressureretaining item is still possible, the Inspector shall instruct the owner or user to havethe stamped data replaced. All re-stamping shall be done in accordance with theoriginal code of construction, except as modified herein. Requests for permission tore-stamp or replace nameplates shall be made to the Jurisdiction in which thepressure-retaining item is installed. Application must be made on the Replacementof stamped Data Form, NB-136 (see NBIC Part 2, 5.3.2). Proof of the originalstamping and other such data, as is available, shall be furnished with, the request.Permission from the Jurisdiction is not required for the reattachment of nameplatesthat are partially attached. When traceability cannot be established, the Jurisdictionshall be contacted.b) When there is no Jurisdiction, the replacement of stamped: data shall be5

authorized and witnessed by a National Board Commissioned Inspector and: the completedForm NB-136 shall be submitted to the National Board.5.2.2 REPLACEMEINT OF STAMPED DATAa) The re-stamping or replacement of data shall be witnessed: by a National BoardCommissioned Inspector and shall be identical] to the original stamping.b) The re-stamping or replacement of a code symbol stamp shall be performed only aspermitted by the governing code of construction.c} Replacement nameplates shall be clearly malted "replacement.“5.2.3 REPORTINGForm NB-136 shall be filed with the Jurisdiction (if required) or the National Board by theowner or user together with a facsimile of the stamping or nameplate, as applied, and shall]also bear the Signature of the National Board Commissioned Inspector who witnessed thereplacement.5

NBIC Part 3, Section 55.11 REMOVAL OF ORIGIONAL STAMPING OR NAMEPLATEIf it becomes necessary to remove original stamping, the Inspector shall, subject tothe approval of the Jurisdiction, witness marking of a facsimile of stamping, theobliteration of old stamping, and transfer of stamping to the new item. Whenstamping is on a nameplate, the Inspector shall witness transfer of the nameplate tothe new location. Any relocation shall be described on the applicable NBIC “R”Form. The re-stamping or replacement of a code symbol stamp shall be performedonly as permitted by the governing code of construction.9

5.12.2 REPAIR NAMEPLATEWhen a pressure relief valve is repaired , a metal repair nameplate stamped with theinformation required: below shall be securely attached to the valve adjacent to theoriginal manufacturer's stamping or nameplate. If notmounted directly on the valve, the nameplate snail be securely attached so as not tointerfere with valve operation and sealed in accordance with the quality system.a) Prior to attachment of the repair nameplate, the previous repair nameplate, ifapplicable, shall be removed from the repaired valve.b) As a minimum, the information on the valve repair nameplate (see NBIC Part 3,Figure 5.7.5-e) shall include:1) The name of the repair organization preceded by the words “repaired by";2) The "YR" repair symbol stamp and the "YR" certificate number;3) Unique identifier (e.g., repair serial number, shop order number, etc.);4) Date of repair;5) Set pressure;6) Capacity and capacity units (if changed from original nameplate due to setpressure or service fluid change);15

7) Type/Model number (if changed from original nameplate by a conversion. See NBIC Part3, S7.2); and8) When an adjustment is made to correct for service conditions of superimposed backpressure and/or temperature or the differential between popping pressure between steam andair (see NBIC Part 3, 4.52), tine information on the valve repair nameplate shall include the:a. Cold Differential Test Pressure (CDTP); andb. Superimposed Back Pressure (BP) (only when applicable).15

CSP-53 POLICY ON PROTECTION OF ASME MARKS(a) Policy on the Protection of ASME MarkAll ASME accreditation programs that specify the use of an ASME mark shallprovide for designated oversight of those activities that affect the proper utilizationof ASME marks. Only ASME accreditation programs with measures established fordesignated oversight may utilize ASME marks.(b) Policy on Designated OversightThe measures established for designated oversight shall comply with ASMEdeveloped criteria. The criteria shall provide oversight measures that establishreasonable assurance that the activities (of the entity authorized by ASME to use itsmarks) that affect the use of ASME marks are accomplished in accordance with theappropriate ASME codes or standards. Where such activities are not accomplishedin accordance with the appropriate ASME codes or standards, the criteria shallprovide for notification of ASME and appropriate redress. ASME shall establishcriteria for and administer the qualification and certification of those agencies,organizations, and individuals authorized to provide designated oversight.(c) General Requirements for Designated Oversight:(1) Each ASME accreditation program that utilizes ASME marksshall provide for one (or more than one, where applicable) of the types of authorizeddesignated oversight given below.21

(i) An ASME accredited authorized inspection agency (AIA),i.e., an authorized insurance company or Enforcement Authority that provides designatedoversight through the use of authorized Inspectors (AI), qualified to criteria specified in theappropriate code or standard, and is not a qualified inspection organization, as described inPara. (c)(1)(ii) below.(ii) An ASME qualified inspection organization (QIO), i.e., anorganization that is qualified by ASME, to criteria specified in the appropriate code orstandard, to provide designated oversight through the use of qualified inspectors (QI); is notan authorized inspection agency, as described in Para. (c)(1)(i) above; and is not an entityauthorized by ASME to use its marks.(iii) A certified individual (CI), i.e., an individual employee ofthe accredited entity authorized by ASME to use its marks, the CI is qualified and certified bythe entity authorized by ASME to use its marks, to criteria specified in the appropriate codeor standard. The qualification and certification are subject to evaluation by ASME during theaccreditation survey or review. The CI is neither an AI nor QI, as described in Paras. (c)(1)(i)and (ii) above.(2) The use of all ASME marks shall be documented with data reports orcertificates of conformance that are signed by a responsible representative of the accreditedentity authorized by ASME to use its marks and the appropriate inspector or individualauthorized to perform the designated oversight activities (i.e., either an AI, QI, or CI).(3) ASME accreditation programs that utilize ASME marks shall provide forrecord retention of all data reports and certificates of conformance, such that the use of theASME marks is documented and traceable to the entity authorized by ASME to use its marks.All data reports and certificates of conformance shall be retained for a period established bythe appropriate code or standard.21

CAP-21 CRITERIA FOR REAPPLICATION OF AN ASMECERTIFICATION MARK1 After an item has been certified under an ASME standard, if the ASMEcertification mark (e.g. Code Symbol Stamp) becomes indistinct or the nameplate isillegible or lost, but traceability to the original certification can be established, thecertification mark may be reapplied to the item.2 Reapplication of certification marks shall only be permitted under the followingconditions:2.1 The owner has requested the reapplication.2.2 Where applicable, the Jurisdiction has granted the request for reapplication.2.3 The reapplication shall be performed by the original manufacturer of the Codeitem. Where responsibility to the original Code certification has been maintained,reapplication by a successor organization to the original manufacturer is permitted.2.4 The reapplication shall be authorized and witnessed by an Inspector from anASME accredited Authorized Inspection Agency, or by an authorized representativeof the Qualified Inspection Organization or a Certified Individual, as applicableunder the associated certification program.2.5 The reapplication of the mark shall be documented on a data report form which22

is retained as required for the original data report.3 Reapplication of the certification mark shall only be provided to restore evidence oforiginal compliance with the standard. Reapplication of the mark shall not be applied forcertification of the current condition of the item or for certification to different requirementsthan originally constructed.22

Where responsibility for the original Code certification has been maintained,reapplication by a successor organization to the original Manufacturer is permitted.24

(d) The reapplication shall be authorized and witnessed by an Inspector from an ASMEaccredited Authorized Inspection Agency, or by an authorized representative of the QualifiedInspection Organization or a CertifiedIndividual, as applicable under the associated certification program.(e) Reapplication of the Certification Mark shall be documented on a Certificate ofConformance for Reapplication of the Certification Mark as shown in Form III-1A. Thecompleted Certificate of Conformance for Reapplication of the Certification Mark shall beretained as required for the original Manufacturer's Data Report.III-3 RULESReapplication of the Certification Mark shall only be provided to restore evidence of originalcompliance with ASME Section I requirements. Reapplication of the Certification Mark shallnot be applied for certification of the current condition of the item or for certification todifferent requirements than originally constructed.24
