ASCEND CV – IT GuidelinesASCEND CV Release 7.0Backup GuidelinesAs part of technical discovery at the site, ASCEND HIT specifies the virtual machine infrastructure(cores, memory, disk) required for their VMs. It is the responsibility of the client site’s MIS/ITstaff to allocate sufficient resources on their VSphere (ESXi) hosts to support the requiredservers for the ASCEND solution.This document specifies what portions of the ASCEND CV running image need to be backed upso they can be restored to the time of backup.Important: As per the ASCEND CV licensing agreement, it is the responsibility of the customer toimplement a backup and restore program for the ASCEND CV application servers and associateddata. These need to be in accordance to the site’s own internal guidelines and procedurespertaining to systems backup, restore, and disaster recovery. ASCEND CV recommends usingdrives other than C:\ to store SQL data and logs, such as a D:\ drive for data and L:\ drive forlogs.It is strongly recommended that:1. ASCEND CV is deployed in a VMWare VCenter Server Environment that includesVMotion and HA (high-availability) so that any physical hardware failure results in a hotfailover to another ESXi virtual host without interrupting the functionality of theASCEND CV application.2. The capabilities in (1) require a high-performance SAN device that employs RAID,allowing for physical disk failures and replacement without impacting any of the virtualmachines running in the VCenter managed environment, including the ASCEND CVservers.3. The SANs environment needs to provide a capability for replication to a remote datacenter or co-location environment, providing for disaster recovery of all enterpriseapplication environments (VMs) and data en masse.4. VM Backup Management software is utilized to backup and/or replicate individual VMsto protect against data loss or corruption on individual virtual appliances such asASCEND CV.Representative packages include “Veeam Backup” Backup and Replication or “DellAppAssure”, that are both designed to: Manage and maintain a current (typically to within the last 5-10 minutes)backup/replication of VMs either onsite for quick restore, offsite for disasterrecovery, or both. Provide a rolling window of snapshots so that VMs can be restored to previouspoints in time over the last 24-48 hours; to protect against and allow forrecovery from system or data corruption.If all or some of these capabilities are not available, as a minimum requirement the customersite IT must take and preserve a snapshot of the ASCEND CV server VMs at the point and timeBusiness Confidential to ASCEND HIT LLCVersion 7.04

The following section is an inventory of ASCEND CVsystem configuration and application (patient data) that needs to be backed up and available forrestore.ASCEND CV Application Server EnvironmentFor the purposes of the following section, X:\ will serve as a placeholder for the drive where therequisite application has been installed.ASCEND CV Configuration and Log FilesLog files are required to be backed up for forensic reasons as they can help determine ordiagnose the reasons for the system or application failure that led to the need for recovery frombackup.On the ASCEND CV Reporting Server(s): X:\Program vice\bin\*.configX:\Program Files\ASCEND .configX:\inetpub\logs\LogFilesInbound File Drop AreaFor device data import mechanisms that leverage a file drop, this is the area where the filedrops occur. In practice, these directories will have little content as the files dropped here aretransitory in nature and all imported data preserved in the Ensemble Caché database isdescribed later; but the contents of this directory can have historical or forensic value.On the ASCEND CV Integration Server: X:\EnsembleTOMTEC Import Measurement Service (TMMS)These directories contain the configuration and licensing files for the TOMTEC supplied servicethat handles importing of DICOM SR measurements from various Echo Ultrasound carts.On the ASCEND CV Interface Server: X:\TMMS * \ImageArena40\GlobalConfigX:\TMMS *\ImageArena40\LocalConfigBusiness Confidential to ASCEND HIT LLCVersion 7.05

ASCEND CV – IT GuidelinesASCEND CV Release 7.0InterSystems Ensemble Site Specific Configuration and Library DatabasesThis directory is required for ASCEND CV’s Ensemble Integration Engine environment to be fullyrestored as configured for the client site, except for the database that stores the ASCEND CVdata, which is backed up and restored separately and described later.On the ASCEND CV Integration Server: X:\InterSystems\HealthShare – excluding all CACHE.DAT filesThe AscendAdmin Service User AccountThis is the user account used by Ascend to service the ASCEND CV application servers. It isrequired to be an AD account that is also a local machine administrator account. This account’suser directory should be preserved so that any notes or content related to installation history orsite configuration are preserved.On all ASCEND CV Servers: C:\Users\AscendAdminBusiness Confidential to ASCEND HIT LLCVersion 7.06

ASCEND CV – IT GuidelinesASCEND CV Release 7.0ASCEND CV Patient and Clinical DataASCEND CV retains data related to Cardiovascular Patients and the clinical proceduresperformed on them in several SQL Server databases, as well as an InterSystems Ensemble Cachédatabase file. Suggested backup and restore guidelines for these are described in the followingsection.ASCEND CV SQL Server Database Files and Transaction LogsASCEND CV requires several SQL Server Databases: an AscendCV databasean AscendAnalytics Databasean AscendDataMart Databasean AscendStratus DatabaseReportServer and ReportServerTempDB databases required to support SQL ServerReporting Services reports that support ASCEND Analytics reportsa TTADB database utilized by the TOMTEC TMMS service in support of its DICOM-SRimport activitiesthe standard SQL Server System DatabasesEach of these databases consist of a data file and transaction log, which are typically placed onseparate Logical Unit Number storage areas, or LUNs, on a SAN, and share the SQL ServerSystem tempdb, which should also be placed on its own LUN. The ASCEND CV IT Requirementsdocument describes how these LUNs could be assigned to specific drive letters, but this is up tothe preferences of the client site.All these LUNs should be backed up frequently, with the transaction logs backed up mostfrequently — (e.g. every hour) — as a combination of a data file backup and the correspondingtransaction log can be used to restore a database to any point in time covered by thetransaction log.It is suggested you follow your facilities own standard SQL server database backup guidelinesor those recommended by Microsoft. ASCEND CV – IT GuidelinesASCEND CV Release 7.0Maintenance PlansAscend CV DatabasesASCEND CV requires 5 production databases: TTADBAs well as ReportServer and ReportServerTempDB required by SSRSThe estimated total disk space consumed by these databases per year of online data iscalculated and provided separately in the IT Requirements document.Ascend CV SQL Maintenance Plan for the Stand-AloneDatabase OptionThe following maintenance plan is an example of what we have used at a client site that preferswe deploy with our standalone database option vs. their own centrally managed Enterprise SQLServer.This site preferred to provision multiple drives that contains the following: Production SQL Database Data Files (Data Store) in D:\MSSQL\DATASQL Database Backups in R:\MSSQL\BACKUPSo Hourly transaction logo Daily Incrementalo Weekly FullTempDB in T:\TEMPDBTransaction Logs in L:\TRANSLOGThis D drive must be backed up by the client site, according to their own data redundancy anddisaster recovery policies, to offsite storage typically using their own licensed BackupManagement Software solution.The ASCEND CV stand-alone database server maintenance plans consist of three primaryscheduled sets of tasks:Business Confidential to ASCEND HIT LLCVersion 7.013

The ‘Frequent’ plan runs once each hour.There is also a daily and weekly task.The SQL maintenance plans for ASCEND CV run as follows: Every hour from 12:30AM to 11:30 PM:1. Transaction log backup of all databases2. Cleanup/Delete transaction log backup files older than 76 hoursEvery day of the week at 12:00AM except Sunday:1. Check database integrity of all databases2. Rebuild indexes of all production databases (AscendCV, AscendAnalytics,AscendDataMart, ReportServer, TIMS)3. Update statistics on all databases’ tables and views4. Perform a differential backup of all databases5. Cleanup/Delete full database backup files older than 9 days6. Cleanup/Delete daily incremental backup files older than 3 daysOnce a week on Sunday at 12:00AM:1. Rebuild indexes of all databases2. Update statistics on all databases’ tables and views3. ASCEND CV – IT GuidelinesASCEND CV Release 7.0Ascend CV Reporting Server Scheduled TaskOn the ASCEND CV Reporting Server a task, Reset ASCEND CV Services, is scheduled once perday that restarts the ASCEND CV Reporting Services to clean up memory due to .NET memoryfragmentation. World Wide Web Publishing Service – Hosts the ASCEND CV web applicationInterface Server VMWhen an ASCEND CV Interface Server VM is restarted or the application is not functioning, thefollowing services should be confirmed to be in a running state:1. ASCEND CV – IT GuidelinesASCEND CV Release 7.0Appendix A: Optional Backup of CachéDatabasesInterSystems Ensemble Caché Database FilesThe ASCEND HIT Caché database file (CACHE.DAT) contains the Ensemble Integration EngineProduction that implements the interfaces to the site’s HL7 Order (ORM, ORU) and ADTmessage feeds and also provides most of the device import and data export capabilitiesprovided by ASCEND CV.This database also contains a transaction history of all recent message and data flow, allowingfor diagnosis, reconfiguration, and replaying of message traffic.In the ASCEND CV installation, the default path for the ASCENDHIT database isX:\InterSystems\HealthShare\mgr\ascend hit, as in the above figure where X is a place holderfor the Ensemble install drive. In practice, this Caché database directory should be moved to theD: (data file) LUN described above.There are also Caché journal files that store all database transactions since the last completedbackup, so that a restore to the most recent point in time before a failure can be achieved.These should typically be placed on a separate LUN and associated drive letter (e.g. “J”). ASCEND CV – IT GuidelinesASCEND CV Release 7.0InterSystems Caché Online Backup ProceduresFor relative simplicity, effectiveness, and for providing minimal downtime, it is suggested thatInterSystems Caché “Online Backup Procedures” be followed for ASCEND CV.In this case, the System Backup should include the following HSAP/Ensemble directories withexclusions:On the ASCEND CV Integration Server: X:\InterSystems\HealthShare – excluding all CACHE.DAT filesThere are 3 different types of online backup: full backup of database(s), cumulative since thelast full backup and incremental since the last backup of any type.When restoring a system from an online backup, first restore the most recent full backup,followed by the most recent cumulative backup, and then all incremental backups taken sincethe cumulative backup.In our case, where the ASCEND HIT database is not that large, we recommend a combination offull backups once a week, with incremental backups occurring daily. No cumulative backup isrequired.Database-Only Versus Full System RestoreDatabase-only restore: A restore where the ASCEND CV Caché database becomes corrupted orunusable and needs to be restored to an earlier point in time – along with a few other keyInterSystems ‘bookkeeping’ databases. This procedure is described later in this document.Full system restore of HSAP/Ensemble (Caché): When required, this consists of a re-installationof HSAP on the server followed by a database-only restore.Configure InterSystems HSAP/Ensemble for Backup Of The Ascend HitDatabaseConfiguration of the Database Backup List, the location the backup should be stored, and settingup the scheduled tasks to perform the regular backups is done in the HSAP Management Portal.To access the Management Portal, right click on the HSAP icon in the system tray:And launch the portal:Business Confidential to ASCEND HIT LLCVersion 7.019
