2015 ANNUAL REPORTIn 2015 Avalon was thrilled to welcome 100 chronicallyhomeless individuals and families into their new homes!Read on for more highlights.

Dear Friends,Avalon Housing had a really big 2015! As you read through thisreport, we hope you will be as excited and proud as we are aboutthe growth and new initiatives we began in 2015. These changeshave enabled Avalon to provide housing and supports to morethan 650 people in a variety of settings across the county. We areopening more doors than ever!Our Permanent Supportive Housing program provides peoplewith an actual place to call home, a community to be part of, andessential one-on-one supports grounded in dignity, compassionand a radical acceptance of all. 2015HIGHLIGHTSAccreditationAt the start of 2015, followinga year- long planning effort,Avalon received approval foraccreditation from COA—the CouncilOn Accreditation. This process helpedAvalon raise our standards, improve our systems, and perhaps mostimportantly it positioned us to utilize Medicaid funding for our supportservices.FUSE (Frequent Users Systems Engagement)The FUSE Initiative is an innovative integration of housing and health care thatis having a big impact in Washtenaw. In 2015, Avalon took over leadershipof FUSE—part of a national demonstration project targeting frequent utilizersof emergency health and crisis services. Now in it’s fourth year, the Initiativehas provided housing and care coordination for 100 of the county’s neediest,costliest and most vulnerable individuals. New York University evaluators willsoon release the data that we believe will demonstrate both cost-savings andpositive health outcomes for this challenging population.

New Development ProjectsReal estate development in the Ann Arbor area is a tough business fornonprofits—which is why new units on the horizon are so exciting! In 2015Avalon signed purchase agreements on 12 existing units, and on a parcelof land that will allow us to build up to 70 new permanently affordableapartments. We look forward to providing more info as plans develop.Arbordale Apartments Renovationents, Ann ArborArbordale ApartmAvalon was awarded Low Income HousingTax Credits to renovate ArbordaleApartments, a 39-unit complex thatAvalon purchased in 1998. This rehab willhelp Avalon maintain high qualityhousing units and guarantee ongoingaffordability.Miller ManorWe started up a new HUD-fundedservices team that provides casemanagement, peer support,community building and 24/7 frontdesk supports at Miller Manor, a106-unit public housing site. Thisproject marks the start of an excitingcommunity partnership betweenAvalon and the Ann Arbor HousingCommission, with commitments inplace to expand Avalon services toall public housing residents.Danielle’s StoryDanielle was living outside for over two yearsbefore moving into Miller Manor, and shehad struggled with addiction for much longerthan that. After only one year in PermanentSupportive Housing, Danielle has achievedsobriety, been reunited with her daughtersand just moved into a larger family unit withAvalon. In May, Danielle was hired by Avalonto provide residential supports to tenantsat another property—proving just how muchdifference PSH can make in one year!“One year ago,I didn’t knowwhere I would betoday, and whoknows where I willbe a year from now it’s amazing.
