This isreferred to as airflow obstruction.Breathing difficulties are caused by long term damage to the lungs,usually because of smoking.COPD is one of the most common respiratory diseases in the UK. Itusually affects people over the age of 35. Over 900,000 people inthe UK have been diagnosed with COPD.Who are we?We are a small nursing team based in Ramsbottom but we cover thewhole of Bury as long as you have a Bury GP.We are not an acute service however we generally see patientsreferred to us within two weeks.2

Who is the service for?Patients with a confirmed diagnosis of COPD requiring advice,education and support to manage their chest condition. This can beafter an acute exacerbation (worsening) or for ongoingmanagement.We provide inhaler and respiratory medication advice and teachyou and your carers how to manage your condition and providing aflare up plan outlining advice on what to do when you becomeunwell.Although our primary focus is your breathing condition we assessyou as a whole and frequently refer onto other services that mayinclude: Dietician.Smoking cessation.Living well services.Occupational Therapy.Oxygen Assessment.Speech And Language Therapy.Pulmonary Rehabilitation.Physiotherapy.Palliative care.Referrals come from GPs, Practice Nurses, Respiratory Consultantsand acute wards at North Manchester General Hospital andFairfield.We also take self-referrals from patients who we have seenpreviously.3
