All other benefits described herein are offeredby Allianz to You under an individual policy which numbercorresponds to the last 4 digits of Your BMO Mastercardnumber (referred to herein as the “individual Policies”).The insurance described in this Certificate of Insurance isfor eligible Mastercard Primary Cardholders of BMO whoseAccounts are in Good Standing and where specified, theirSpouses, Dependent Children and/or certain other persons(referred to herein as “You” or “Your”). This insurance is1

administered by Allianz Global Assistance through theOperations Centre. BMO receives compensation from thesale of this insurance.This Certificate of Insurance is effective on the date BMOreceives and approves the Primary Cardholders requestthat this insurance be included as a feature of theirMastercard.Only BMO may determine who is a Primary Cardholder,whether an Account is in Good Standing and whetherthe insurance pursuant to this Certificate of Insurance hascome into or is in force.No person is eligible for coverage under more than onecertificate of insurance providing insurance coveragesimilar to that provided hereunder. In the event that anyperson is recorded by Us as an “Insured Person” undermore than one such certificate, such person shall bedeemed to be insured only under the certificate whichaffords that person the greatest amount of insurancecoverage. In no event will a corporation, partnership orbusiness entity be eligible for this insurance coverage.This Certificate of Insurance supersedes any certificatepreviously issued to You.In this Certificate of Insurance, certain terms havedefined meanings. Defined terms are capitalizedthroughout this document. Those defined terms areas follows.1 DefinitionsAccidental Bodily Injury means bodily injury causeddirectly and independently of all other causes by externaland purely accidental means. The accident must occurduring the Coverage Period and the loss to which theinsurance applies must result within three hundred andsixty-five (365) days of the date of the bodily injury andmust not result from any of the exclusions.Account means the Primary Cardholder’s Mastercardaccount, which is in Good Standing.Actual Cash Value means We will pay the lesser of:: the actual purchase price of a similar item; the actual cash value of the item at the time of loss,which includes deduction for depreciation (for itemswithout receipts the insurance will pay up to 75% of thedetermined depreciated value); or the cost to repair the item.Benefit Amount means the Loss amount applicable at thetime the entire cost of the Passenger Fare(s) is charged toYour Mastercard Account.Certificate of Insurance means a summary of thebenefits provided under the Policy issued to BMO coveringaccident and sickness and the individual policy ofinsurance for all other benefits.Common Carrier means any land, air or water conveyancefor regular passenger service, which is fully licensed tocarry passengers for compensation or hire and whichundertakes to carry all persons indifferently as to whomay apply for passage, so long as there is room and thereis no legal excuse for refusal.Coverage Period means the time insurance is in effect,as indicated in the various sections of this Certificate ofInsurance.Departure Date means the date on which You depart onYour Trip.23

Dependent Child(ren) means an unmarried natural,adopted or stepchild of a Primary Cardholder dependenton the Primary Cardholder for maintenance and supportwho is:Operations Centre means the Operations Centremaintained by Allianz Global Assistance. From Canada andthe U.S. call 1-877-704-0341. From elsewhere call collect1-519-741-0782. twenty (20) years of age and under; or twenty-one (21) years of age or older and permanentlymentally or physically challenged and incapable of selfsupport and became so while eligible as a dependentchild.Passenger Fare means a ticket for travel on a CommonCarrier which has been completely charged to the Account.Tickets obtained through the redemption of loyalty pointsearned under the Mastercard reward program are eligibleproviding that all applicable taxes and/or fees have beencharged to the Primary or Family Cardholder’s Accountor paid through the redemption of loyalty points earnedunder the Mastercard reward program.Family Cardholder means a Primary Cardholder’sSpouse and/or Dependent Child who have been issued asupplemental Mastercard on the Account by BMO.Personal Effects means property normally worn ordesigned to be carried on or by an Insured Person solelyfor private purposes and not used for business.Good Standing means being in full compliance with allof the provisions of the cardholder agreement in forcebetween the Primary Cardholder and BMO, as amendedfrom time to time.Primary Cardholder means the cardholder who hassigned an application for a Mastercard, as primarycardholder, and for whom the Mastercard Accountis established and for whom BMO has received andapproved a request to include this insurance as a featureof the Mastercard. twenty-five (25) years of age and under and a full-timestudent attending a recognized college or university; orImmediate Family Member means the InsuredPerson’s Spouse, child including adopted children andstepchildren, parent, sibling, legal guardian, parent-in-law,grandparents, grandchildren, daughter-in-law, son-in-law,brother-in-law and sister-in-law.Insured Person means those persons covered for thebenefits described in this Certificate of Insurance asspecifically defined in each of the benefit sections.Loss means, with respect to a hand, complete severancethroughout or above the knuckle joints of at least four(4) fingers on the same hand; with respect to a foot,complete severance through or above the ankle joint. Wewill consider it a loss of hand or foot even if they are laterreattached.Mastercard means a Mastercard card issued by BMO andfor which BMO has received and approved the PrimaryCardholder’s request to include this insurance as a featureof the card.Mysterious Disappearance means when the article ofpersonal property in question cannot be located, and thecircumstances of its disappearance cannot be explainedand do not lend themselves to a reasonable inference thata theft occurred.Occupying means in, upon, entering into or alightingfrom.4Rental Car means a land motor vehicle with four wheels,that is designed for use mainly on public roads and whichYou have rented from a commercial rental agency for Yourpersonal use for the period of time shown on the RentalCar Agreement. Certain motor vehicles are not covered,please refer to Section 4.1.4.2. With regards to the CollisionDamage Waiver (CDW) Benefits described under Section4.1.1 a Rental Car may also include a commercial carsharing program of which You are a member.Rental Car Agreement means the entire written contractthat You receive when renting a car from a commercialrental car agency that describes in full all of the terms andconditions of the rental, as well as the responsibilities ofall parties under the Rental Car Agreement. With regardsto the Collision Damage Waiver (CDW) Benefits describedunder Section 4.1.1 a Rental Car Agreement may alsoinclude a commercial car sharing program of which Youare a member and the terms and conditions thereof.Spouse means the person who is legally married to thePrimary Cardholder; or if there is no such person, theperson who has been living with the Primary Cardholderin a conjugal relationship and who resides in the samehousehold as the Primary Cardholder and is publiclyrepresented as the spouse of the Primary Cardholder. Forthe purposes of this insurance the Primary Cardholder mayhave only one (1) spouse.5

Terrorism means the unsanctioned and illegal use of forcethat causes destruction of property, injury or death by anindividual or group for the express purpose of achieving apolitical, ethnic or religious goal or result.Ticket means evidence of full fare paid for travel on aCommon Carrier, which has been partially or completelycharged to the Primary or Family Cardholder’s Account.Ticket(s) obtained through the redemption of loyaltypoints earned under the Mastercard reward program areeligible for coverage.Trip means a defined period of travel of definite lengthfor which the full cost, or portion of the cost, of Your Tickethas been charged to the Primary or Family Cardholder’sAccount. Tickets obtained through the redemption ofloyalty points earned under the Mastercard rewardprogram are eligible for coverage.4 Coverage period anddescription of benefits4.1 CAR RENTAL BENEFITSCoverage EligibilityThe Car Rental Benefits apply when You enter into anon-renewable Rental Car Agreement for a four-wheelpassenger vehicle, where the total rental period does notexceed forty-eight (48) days and is subject to limitationsand exclusions (as outlined in Section 4.1.4) and thefollowing requirements:1.the Rental Car must be rented by the PrimaryCardholder, or a Family Cardholder; andWe, Our, Us means Allianz Global Risks US InsuranceCompany (Canadian Branch).2. the Rental Car must be rented from a commercial carrental agency; andYou or Your means the Insured Person.3. the full cost, or portion of the rental cost, must beeither charged to the Account or paid through theredemption of loyalty points earned under theMastercard reward program. An eligible Rental Carincluded in a pre-paid travel package is coveredif the full cost, or portion of the cost, of the travelpackage was charged to the Account or paid throughthe redemption of loyalty points earned under theMastercard reward program; and2 Insurance effective andtermination dateExcept as otherwise stated herein, this insurance shallcome into effect on the date BMO receives and approvesthe Primary Cardholder’s request to include the BMOTravel Protection as a feature of their Mastercard.Except as otherwise stated herein, this insurance shallterminate on the earliest of:1.The date the Insured Person is no longer eligibleto participate;2. The date the eligible Account is defined as ineligibleby BMO;3. 00:01 a.m. of the date the Primary Cardholder cancelsthis coverage or otherwise chooses to close theMastercard Account; or4. The date the Policy is terminated.3 EligibilityTo be eligible for this insurance You must be a resident ofCanada with a Mastercard Account in Good Standing.64. You must not rent more than one vehicle at a timeduring a rental period; and5. You must decline the collision damage waiver benefits(or similar provisions, such as “loss damage waiver”)offered by the rental agency (when not prohibited bylaw). If there is no space on the Rental Car Agreementto decline coverage, You must write on the contract“I decline the CDW provided by the Rental Agency.”If such coverage is not available from the rentalagency, then CDW benefits are not available under thisCertificate of Insurance; and6. the Rental Car must have been operated by thePrimary Cardholder, a Family Cardholder, the PrimaryCardholder’s Spouse or Dependent Child listed onthe Rental Car Agreement and authorized to operatethe Rental Car under the Rental Car Agreement inaccordance with its conditions when the loss occurs.Coverage PeriodFor a Rental Car to be eligible for coverage, this insurancemust be a feature of the Mastercard prior to entering into7

the Rental Car Agreement or prior to the Insured Persontaking control of the Rental Car. Insurance coverage beginsas soon as the Primary Cardholder, the Family Cardholder,the Primary Cardholder’s Spouse or Dependent Child who isauthorized to operate the Rental Car under the Rental CarAgreement takes control of the Rental Car. The total rentalperiod must not exceed forty-eight (48) consecutive days.In order to break the consecutive day cycle, a full calendarday must exist between rental periods. If the rental periodexceeds forty-eight (48) consecutive days, coverage underthis Certificate of Insurance will be void.Insurance coverage ends at the earliest of:1.the time when the rental agency assumes control ofthe Rental Car, whether it be at its place of business orelsewhere. Rental keys left in a locked drop box doesnot constitute that the rental agency has assumedcontrol of the Rental Car;2. the end of the chosen rental period; or3. the date on which the Primary Cardholder’s coverageis terminated in accordance with the “InsuranceEffective and Termination Date” provision set outabove.4.1.1 COLLISION DAMAGE WAIVER (CDW)BENEFITSInsured Person means the Primary Cardholder, FamilyCardholder, Primary Cardholder’s Spouse or DependentChild(ren).Coverage Benefits Subject to the terms and conditions,You are covered for Rental Cars with a Manufacturer’sSuggested Retail Price (MSRP), in its model year, up to amaximum of 65,000 for:1.damage to the Rental Car; and2. theft of the Rental Car or any of its respective parts oraccessories; and3. rental agency charges for valid loss-of-use while theRental Car is being repaired; and4. reasonable and customary charges for towing theRental Car to the nearest available facility.This coverage does not provide any form of thirdparty automobile, property damage or personalinjury liability insurance. It is the responsibility ofthe Insured Person to have adequate third partyinsurance, either through their own automobileinsurance policy or by accepting the liability portion ofthe insurance offered through the rental agency.8The amount of the benefit payable will be equal tothe cost of the repair (including loss-of-use) or thereplacement cost of Your Rental Car which has beendamaged or stolen, less any amount or portion of theloss assumed, waived or paid by the car rental agency, itsinsurer, or a third party insurer.In the event of a claim, the Insured Person must contactthe Operations Centre as soon as possible or within fortyeight (48) hours. We will need the following information: a copy of the driver’s license of the person who wasdriving the Rental Car at the time of the accident; a copy of the loss/damage report You completed withthe rental agency; a copy of the original police report when the resultingloss from damage or theft was over 500; a copy of Your Mastercard sales draft, and Yourstatement of Account showing the rental charge. Thischarge must appear on Your credit card statementwithin ninety (90) days of the incident; the original front and back pages of the opened andclosed-out Rental Car Agreement, or if applicable, acopy of Your membership agreement with the carsharing program, a copy of the visual inspection reportcompleted prior to assuming control of the vehicle andconfirmation of Your time booked; a copy of the itemized repair estimate, final itemizedrepair bill and parts invoices; original receipt(s) for any repairs for which You mayhave paid; and if the loss-of use is charged, a copy of the rentalagency’s daily utilization log from the date the RentalCar was not available for rental, to the date the RentalCar became available to rent.Please see section 4.1.4 for applicable limitations andexclusions.4.1.2 CAR RENTAL ACCIDENTAL DEATH ANDDISMEMBERMENT BENEFITSInsured Person means the Primary Cardholder and his/her Spouse and Dependent Child(ren) while Occupying aneligible Rental Car.Coverage Benefits Car Rental Accidental Death andDismemberment Insurance covers an Insured Person whosuffers a “loss”, as defined as follows, arising as a resultof an Accidental Bodily Injury to the Insured Person whileOccupying an eligible Rental Car.9

“Loss of hand or foot” means dismemberment bycomplete and permanent severance at or above the wristor ankle joint. “Loss of thumb and index finger” meanscomplete and permanent severance of the thumb andindex finger on the same hand. “Loss of sight” meanscomplete and irrecoverable loss of all visual acuity and itmust be the direct result of physical damage to the eyeand/or optic nerve. Legal blindness is not the standardfor determining “Loss of sight” under this Certificate ofInsurance. “Loss of speech or hearing” must be completeand irrecoverable.Loss means one of the following losses as definedherein:Amount of nLoss of Life 200,000 20,000Loss of Both Hands or Feet 200,000 20,000Loss of One Foot or OneHand and the Entire Sightof One Eye 200,000 20,000Loss of Entire Sight of BothEyes 200,000 20,000Loss of One Hand and OneFoot 200,000 20,000Loss of Speech and Hearing 200,000 20,000Loss of One Hand or OneFoot 100,000 10,000Loss of Entire Sight of OneEye 100,000 10,000Loss of Speech 100,000 10,000Loss of Hearing 100,000 10,000Loss of Thumb and IndexFinger on the Same Hand 50,000 5,000LossPlease see section 4.1.4 for applicable limitations andexclusions.Exposure and DisappearanceIf by reason of an accident covered by this Certificate ofInsurance an Insured Person is unavoidably exposed to theelements and as a result of such exposure suffers a lossfor which indemnity is otherwise payable hereunder, suchloss will be covered hereunder.If the body of an Insured Person has not been found withintwelve (12) months after the date of disappearance as theresult of the sinking or wrecking of a vehicle in which theInsured Person was riding at the time of the accident andunder such circumstances as would otherwise be coveredhereunder, it will be presumed that the Insured Personsuffered loss of life resulting from Accidental Bodily Injury.BeneficiaryAny accidental death benefit payable under this Certificateof Insurance will be paid to the Primary Cardholder, ifliving, otherwise to the estate of the Primary Cardholder,unless a beneficiary designation has been filed withAllianz Global Assistance. All other benefits are payable tothe Primary Cardholder.4.1.3 CAR RENTAL PERSONAL EFFECTS BENEFITSInsured Person means the Primary Cardholder, his/her Spouse and Dependent Child(ren) travelling with thePrimary Cardholder or Family Cardholder who has rentedthe Rental Car.Coverage BenefitsThis Personal Effects insurance covers theft or damageto Personal Effects belonging to an Insured Person whilesuch Personal Effects are in a Rental Car during a Trip forthe duration of an eligible rental period.Coverage during such rental period will be the ActualCash Value of Your Personal Effects up to a maximumof 1,000 for each Insured Person, per occurrence. Totalbenefits during each rental period are limited to 2,000per Account.Please see section 4.1.4 for applicable limitations andexclusions.The maximum benefit payable for any one accident is 300,000. If more than one of the described Losses issustained by an Insured Person in any one accident, thenthe total benefit payable for that accident is limited tothe greatest amount payable for any one of the Lossessustained.1011

4.1.4 CAR RENTAL BENEFITS AND EXCLUSIONS4.1.4.1 CAR RENTAL BENEFITS AND EXCLUSIONSThis insurance does not cover certain risks. We will notpay any of the Car Rental benefits if a claim is directly orindirectly a result of one or more of the following:Damage - wear and tear, gradual deterioration,mechanical or electrical breakdown or failure, insects orvermin, inherent flaw or damage, damage caused by theuse of incorrect fuel type;Loss of Vehicle Entry Device – loss, damage ormisplacement of vehicle entry devices;Diminished Value - the amount by which the resale valueof a damaged (or damage repaired) Rental Car has beenreduced for having a significant damage history;Violation of Rental Car Agreement - operation of theRental Car in violation of the terms of the Rental CarAgreement;Intentional Acts - damage due to intentional acts, whilesane or insane;Off-road operation - damage caused to the Rental Car byuse off of publicly maintained roads;Speed Contests - damage caused to the Rental Car whiledriving at a rate of speed that is a marked departure fromthe lawful rate of speed;Intoxication - any event which occurs while the InsuredPerson is under the influence of illicit drugs or alcohol(where the concentration of alcohol in the Insured Person’sblood exceeds eighty (80) milligrams of alcohol in onehundred (100) millilitres of blood) or when the InsuredPerson illustrates a visible impairment due to alcohol orillicit drugs;Drugs or Poison - any voluntary taking of poison, toxicsubstances or non-toxic substances or drugs, sedativesor narcotics, whether illicit or prescribed, in such quantitythat they become toxic, or voluntary inhalation of a gas;Disease - bodily or mental infirmity, sickness, illness, ordisease of any kind;Medical Complications - medical or surgical treatment orcomplications arising therefrom, except when required asa direct result of an Accidental Bodily Injury;Suicide - suicide, attempted suicide or self-inflicted injurywhile sane or insane;Illegal Trade - transporting contraband or illegal trade;12Criminal Offence - committing or attempting to commita criminal offence or dishonest or fraudulent acts, orcommitting or provoking an assault;War or Insurrection - declared or undeclared war, or anyact of war, riot or insurrection; or service in the armedforces of any country or international organization; orhostilities, rebellion, revolution or usurped power;Liability - other than for loss of, or damage to, the RentalCar;Expenses - assumed waived or paid by the commercialcar rental company or its insurers or payable under anyother insurance;Confiscation - confiscation by order of any government orpublic authority;Seizure or Destruction - seizure or destruction under aquarantine or customs regulation.4.1.4.2 COLLISION DAMAGE WAIVER (CDW)BENEFITS LIMITATIONS AND EXCLUSIONSIn addition to the General Car Rental Benefits Limitationsand Exclusions, these limitations and exclusions apply toCollision Damage Waiver (CDW) Benefits:1.There is no coverage for any vehicle with aManufacturer’s Suggested Retail Price (MSRP), in itsmodel year, over 65,000.2. There is no coverage for additional rental fees chargedby the rental agency for a replacement vehicle ifrequired by You for the remainder of the originalrental period.3. This coverage does not apply to Rental Cars when Yourrental period is more than forty-eight (48) consecutivedays, or Your rental period is extended for more thanforty-eight (48) days by renewing or taking out a newrental agreement with the same or another rentalagency for the same vehicle or other vehicles.4. This coverage will not pay for cost of any insuranceoffered by or purchased through the car rentalcompany, even if such cost is mandatory or included inthe price of the vehicle rental.5. Vehicles which belong to the following categories arenot covered: vans (except as defined below); trucks (including pick-ups) or any vehicle that canbe spontaneously reconfigured into a pick-up truck;13

campers or trailers; vehicles towing or propelling trailers or any otherobject; off-road vehicles (Sport Utility Vehicles are covered,provided they are not used as off-road vehicles,are driven on maintained roads and do not have anopen cargo bed); motorcycles, mopeds or motorbikes; expensive or exotic vehicles; antique vehicles; recreational vehicles or vehicles not licensed forroad use; and leased vehicles with buyback guarantee.Vans are covered provided that they:1.are for private passenger use with seating for no morethan eight (8) occupants including the driver; and2. do not exceed a “3/4 ton” rating; and3. are not designed for recreational use (such as but notlimited to camping, operation on roads not maintainedby a federal, provincial, state or local authority and isdesigned and manufactured for off road use); and arenot to be used for hire by others.Antique vehicles are ones which are over twenty (20)years old or when their model has not been manufacturedfor ten (10) years or more.Limousines are not covered. However, standard productionmodels of these vehicles that are not used as limousinesare covered provided that they have an MSRP, in theirmodel year, of 65,000 or less.4.1.4.3 CAR RENTAL PERSONAL EFFECTS BENEFITSLIMITATIONS AND EXCLUSIONSIn addition to the General Car Rental Benefits Exclusionsand Limitations, these specific limitations and exclusionsapply to the Car Rental Personal Effects Benefits.1.Personal Effects do not include money (whether paperor coin), tickets, consumable or perishable goods,bullion, banknotes, negotiable instruments or othernumismatic property.2. Benefits are not paid if loss results from MysteriousDisappearance.3. Reasonable effort must have been made by theInsured Person to protect their Personal Effects (e.g.locking Your personal items in the trunk of the Rental14Car instead of the front or back seat). If claimingas a result of theft, evidence of forcible entry intothe vehicle while all its doors, windows and otheropenings are closed and locked must be submitted.4. Personal Effects coverage is in excess of all otherapplicable valid insurance, indemnity or protectionavailable to the Insured Person in respect of the itemsubject to the claim.
