3General Preceptor GuidelinesTo meet student learning objectives, The College of Nursing at the University of South Alabamaencourages preceptor(s) to:1. Provide supervision and guidance in the clinical arena2. Share theoretical and experiential knowledge with the student specific to the specialtypatient population3. Counsel and guide students in identified areas of weakness4. Facilitate the growth of assessment and decision-making processes from observational tosemi-autonomous5. Facilitate opportunities for the student to observe and actively participate in specialprocedures6. Support the student as a professional while functioning in the role of an advancedpractice nurse7. Notify faculty of poor student performance that may interfere with appropriate/safe careof the patientAs mentioned previously, the following documents are included in this packet:1. Practicum Attendance Record: The student will complete the identifying information atthe top of the form prior to starting clinical hours with a preceptor and will have oneindividual Practicum Attendance Record for each preceptor with whom the studentworks. The student will then log the date and time for each clinical experience (day) withthe preceptor and present this running log to the preceptor at the end of each day for thepreceptor to verify, sign, and date. The student is responsible for submitting the documentto faculty at the conclusion of each clinical rotation.2. Bi-weekly Self-Evaluation: The student will complete this brief self-evaluation everyother week, focusing on his/her strengths and weaknesses and the plan to addressweaknesses. The student will provide the document to the preceptor so he/she mayreview the self-evaluation, include any comments or feedback if necessary, and sign thedocument physically or digitally. The student is responsible for submitting the documentto faculty after it has been reviewed and signed by the preceptor.3. Preceptor Evaluation of Nurse Practitioner Student: The student will provide thiscomprehensive evaluation form to the preceptor at the culmination of the clinicalrotation. It may be completed and signed by the preceptor either manually or digitally.The preceptor may return the completed evaluation to the student or directly to thefaculty facilitator.
