Better Health for AllThe Santa Clara Valley Health & Hospital System has been, andwill continue to be, Santa Clara County’s healthcare safety net.But our public healthcare system must do and be more.To succeed in a changing healthcare environment, a transformation in the way healthcare isdelivered is needed. And to meet the goals of national healthcare reform, the Institute forHealthcare Improvement described an approach to enhance health system performance.The approach is called the “Triple Aim” and it targets quality, cost and population health.Hospitals and health systems, large and small, have adopted it as framework for improvement.The approach and objectives outlined by the Institute have already generated meaningfulchange. But there is more work to be done and Santa Clara Valley Health & Hospital System(SCVHHS) is committed to carrying out the Triple Aim objectives: Improve the patient experience of care (quality) Reduce the cost of healthcare per person (cost) Improve the health and well-being of the community (population health)Over the last five years, Santa Clara Valley Health & Hospital System has made a numberof changes and achieved a great deal. You will read about the accomplishments in thesepages. But more importantly, you will see examples of how the Triple Aim objectives arebeing reached.SCVHHS has undertaken this work for the well-being of our community, and for the healthof residents, patients, clients, and employees. Growing access tothese services is an importantstep in protecting the health ofmothers and infants.VHC Milpitas OpensFree Whooping Cough VaccinesThe health center offers Obstetricsand Gynecology, Pediatrics, InternalMedicine and Family PracticeMedical Homes, pharmacy andlaboratory services.When whooping cough reachedepidemic levels in California, thePublic Health Department offered freevaccinations to prevent the spread ofthis highly contagious disease.Rehabilitation CenterCelebrates 40 YearsRecognized nationally as a center forexcellence, the Rehabilitation Centertreats patients with severe brain, spinalcord and stroke related injuries.Trauma CenterReceives VerificationRecognizing the Trauma Center’sdedication to providing the best care forinjured patients, the American Collegeof Surgeons re-verified the Center asa Level I Trauma Center and a Level IIPediatric Trauma Center.Stroke System of CareServices for VeteransThe Emergency Medical Services Agencydesignated SCVMC as a Primary StrokeCenter. All stroke centers, includingSCVMC, specialize in the treatment ofacute stroke patients.The Adult System of Care, withthe Drug Treatment Court, createdspecial services for veteransincluding residential treatmentand case management services.By providing great care, passing local ordinances on limitingtobacco use and preventing childhood obesity, we areimproving the health and well-being of the community.

2011 Better Health for AllNew Nutritional StandardsBurn Center Nationally VerifiedEcstasy Drug PreventionDiabetes Center RecognizedNew Technology PilotedThe Public Health Department, based on national bestpractices, develops standards that apply to food andbeverages offered, purchased, or served at Santa ClaraCounty facilities, or by the County’s departments or agencies.The American Burn Association and the American College ofSurgeons Committee on Trauma congratulated the center for itscommitment to excellence and for providing quality burn careto both adult and pediatric patients.The Department of Alcohol & Drug Serviceslaunched outreach to schools. This includedlesson plans for teachers and a Train-theTrainer program to educate youth and theirparents about the risks.The prestigious American DiabetesAssociation awarded the EducationRecognition Certificate to theDiabetes Program for havingexcellent diabetes self-managementeducation programs.eLEGS, a wearable robotused by patients with paralysisor weakness in the lowerextremities, was tested atSCVMC. This collaborative conferencewas an essential step in translating regeneration research intoclinical treatments.School-Linked ServicesThis system of coordinatedand effective health and socialservices on school campuses andin the community help childrenthrive at home, in school, and intheir communities.Volunteers Celebrate 75 Years ofDedication and ServiceOver the years, thousands of volunteers have been partof our team making patients their top priority. In the last10 years alone, over 2,000 volunteers have given 450,000hours of service to patients, visitors and staff.Our electronic medical record system gives physicians andmedical staff immediate information about their patients.More timely and effective care is provided, and healthcarecosts can be reduced.

TheBurn Center scored in the top 10%nationally in overall quality of careand being rated as excellent bytheir patients.Breastfeeding is a crucial first step in protecting the healthof mothers and infants, nourishing infants and building afoundation for a healthy immune system. SCVMC rankedone of the top hospitals in the state for breastfeeding rates.The Public Health Department partnered with FIRST 5 SantaClara County and the Santa Clara Valley Water District to bringwater bottle filling stations to schools, community centers, parks,and other public gathering spaces. The PublicHealth Department worked with SCVMC and otherlocal hospitals to properly train staff, monitoredtravelers returning from the affected countriesand put systems in place for a strong publichealth response.The Behavioral Health Departmentran a campaign to raise awarenessabout crisis and suicide preventionservices. Througha public-private partnership,this new center will enhancethe maternity experience andcontinue to provide the best careto newborns, moms, and families.Number ofUninsured ReducedIn December, 2013there were 35,141patients without anyhealth coverage orinsurance. Thenew department will offer bettercoordination of services, key toimproving the health and well-beingof the community.The Violence Prevention Programlaunched awareness efforts to increaseknowledge and skills for buildinghealthy, non-violent relationships withyoung people ages 11–17.US News & World Report ranked SCVMC one of thebest hospitals in the San Jose area in 2013–14.The Rehabilitation Center was also ranked a highperforming specialty. The Burn Center receiveda 5 Star Excellence Award from PRC for providingexemplary care, demonstrating their commitmentto compassionate care at the highest level.The Medical Respite Program has saved over 2,500 hospitalbed days and 6 million in costs to area hospitals. 2015 Better Health for AllIt’s in our hands.SCVMC Lowest Mortality RatesNew Clinical Trial BeginsNew Leaders NamedSCVMC had the lowest death rates forheart attacks, stroke and pneumonia ofthe nine hospitals in Santa Clara Countyaccording to the Agency for HealthcareResearch and Quality.SCVMC is the first public hospital inthe nation designated to provide aregenerative medical treatment fornewly injured spinal cord patients.Bruce Butler was named CEO ofValley Health Plan. Dr. Sara Cody,Health Officer for Santa ClaraCounty remained in that positionand was named the Director ofthe Public Health Department.Trauma Center Reaches 20,000 Preventing Heart Attacksand StrokesFor 15 years the Trauma Center hasparticipated in the “Every 15 Minutes”Program. Over 20,000 high school juniorsand seniors have been challenged to thinkabout drinking, driving, and the impact oftheir decisions.With a grant from KaiserPermanente, SCVMC is part of aneffort to prevent heart attacksand strokes among people withdiabetes. Activities like this assistpatients in developing the skills, knowledge and behaviors theyneed to rebuild their lives. When they achieve the highest degree ofindependence possible, the burden of their injury is greatly reduced.Santa Clara County Emergency Medical Services Agency,with the Fire Chief’s Association, Fire Departments andSheriff’s Office, introduced this public safety campaignto save lives.Providing innovative medical treatments; preventing heart attacks andstrokes; and supporting efforts to get more children, adults and familieshealthcare coverage, all contribute to improving the health of thecommunities we serve.
