Filing Claims And Pre-Treatment Estimates With AvesisAvesis wants to make your claim submission and paymentprocess as simple as possible. If your office is able tofile claims and pre-treatment estimates electronically, weencourage you to do so. If you have questions, pleasecontact your Provider Services representative and askhow you can get set up for electronic submission. Youmay also submit claims and pre-treatment estimatesdirectly to Avesis on the secure Avesis web portal atwww.avesis.com.Providers may also submit claims and pre-treatmentestimates on an ADA claim form to the following address:Avesis, PO Box 7777, Phoenix, AZ 85011–7777. Avesishas no special restrictions or requirements regardingwhich version of the ADA claim form providers maysubmit, as long as your office uses at least the CDT 2013codes when submitting claims and include the renderingprovider’s NPI number on the claim form. Failure to submit2your NPI number on the claim form will result in yourclaim being returned to you unprocessed. Please be surethat the information is entered clearly in the appropriatefields on the ADA claim form. Avesis scans all claim formsand they are entered into the system using OCR (Opticalcharacter recognition).If your information is not clearly entered in the appropriatefield, it is possible that characters may be misread or notrecognized as being present on the claim form at all. Youwill receive written notification of the status of both claimsand pre-treatment estimates in the mail. Should you haveclaims or pre–treatment estimates for which you do notbelieve you have received a response, you may check thestatus of those claims and/or pre-treatment estimates onthe secure Avesis web portal at www.avesis.com, or bycalling our Customer Service team at 888-209-1243.

UPMC Program ChangesUMPC for KidsThe Pennsylvania Insurance Department made somechanges to CHIP dental benefits for calendar year 2013.Those are as follows: A) Members enrolled in UPMC for Kids will no longerhave access to an extended dental benefit. Thisexpanded dental benefit (EDB) was only availablefor calendar year 2012. However payment will bemade on extended dental benefits authorized byPennsylvania CHIP in 2012 even if the services wereprovided in whole or in part after 2012. The annualmaximum for all non-orthodontia services is 1500.00per calendar year.All of these Special Needs Plans have the followingbenefits available to them:B) The orthodontic benefits will no longer be reimbursedat a case rate with all fees being paid in one lump-sumup front. Beginning with new cases banded January1, 2013 and after, Avesis will be paying based uponthe previous process of an initial banding fee followedby quarterly payments. All newly approved and billedorthodontic cases for 2013 will be reimbursed with abanding fee, up to seven quarterly fees and retention.This reimbursement will mirror the way that youare currently being reimbursed through the UPMCMedicaid benefits.UPMC Community Care; UPMC for Life Options;UPMC for You AdvantageEffective January 1, 2013, UPMC added the UPMCCommunity Care plan and updated the UPMC for LifeOptions and UPMC for You Advantage benefits. Thesethree plans comprise UPMC Health Plans Special NeedsPlan lineup. UPMC for You Advantage (HMO SNP)UPMC for You Advantage offers enhanced dentalbenefits for dual eligible beneficiaries. UPMC for Life Options (HMO SNP)UPMC for Life Options offers enhanced dentalbenefits for eligible and dual eligible beneficiaries.UPMC Community Care (HMO SNP)UPMC Community Care offers enhanced dentalbenefits for eligible and dual eligible beneficiaries. 0 copayment for the following preventive services: Oral exams up to 1 visit(s) every six months. Cleanings up to 1 visit(s) every six months. Dental x-rays up to 1 visit(s) every six months. Additional comprehensive benefits: Fillings and simple tooth extractions only.Program InitiativesBoth Avesis and UPMC are committed to putting forthevery effort to ensure that the UPMC population isfollowing AAPD (American Academy of Pediatric Dentistry)standards encouraging a dental visit by age one. As timeprogresses, Avesis will continue to update the networkon these initiatives and steps we are taking for itsimplementation.Special Needs ProgramAvesis has been working diligently with the PennsylvaniaDepartment of Public Welfare on a Special Needsinitiative. Your offices will receive a special needs surveyfrom Avesis. Please complete your survey and return it tothe Avesis Provider Services department.3

Claims and Provider Billing4Avesis would like to take this opportunity to advise allof our dental providers regarding issues that have beenfound on our office reviews. We want your office tofollow our guidelines to ensure that your billings are paidpromptly and accurately. Situations have been discoveredwhere dental offices are billing for services on one dateand then resubmitting using a different date if the claim isnot paid. Some of the reasons we have heard for this arebecause of Medicaid timely filing limitations or computermistakes.Another frequent issue is offices billing for servicesrendered by one dentist under another dentist’s name andprovider number. If your office has been doing this, youneed to immediately cease this practice unless you are ina Locum Tenens situation. If you are using a dentist asa Locum Tenens, then you must notify Avesis that youroffice is employing such a dentist, prior to the LocumTenens provider beginning to see patients, except in thecase of an emergency. Remember that use of a LocumTenens dentist is limited to sixty (60) days.We have also been made aware of billing for services notrendered. These are clear violations of the PennsylvaniaMedicaid rules and, if determined to be accurate, willresult in termination from the network and reporting to theappropriate authorities.If Avesis discovers offices billing under a providerother than the one rendering services we will performa complete review of all billings from your office foraccuracy and we will be required to report the situation toUPMC Health Plan and the appropriate state agency fortheir investigation. As a dentist in the practice, you areresponsible for all acts of your staff members regarding

Claims and Provider Billing (cont’d)claim submission. You should be aware if there are anyinappropriate claim practices occurring within your officeand should stop them immediately. Please understandthat these issues are some of the key issues that we willbe concerned about when reviewing your office’s claims.Any office in violation of these guidelines will be requiredto refund all payments made from Avesis and may riskbeing terminated from the dental network.Radiograph RequirementsWhen submitting images to Avesis for review, all imagesneed to be properly mounted, indicating right or left,date taken, and patient name. The provider’s name andaddress should be on the series or images. It is imperativethat the crown and apex of the tooth be visible onperiapical images.known as the BIN number - and merge it with the 10digit MasterCard account number provided in theemail/fax/mailed remittance to process the paymentthrough your credit card system as you would anyother credit card payment.Q: What is the maximum number of allowabletransactions that I can process on a card?A: There is one (1) per transaction, for the exact amountof the payment.Q: If I lose the card information, what do I do?A: You may contact your Avesis Provider ServicesRepresentative.Spore TestingSpore testing or biological monitoring of sterilizers isrecommended to be performed on a weekly basis. Forfurther guidance regarding sterilization procedures, pleaseconsult the CDC website (www.cdc.gov).Card Payment Services (CPS)Your office may have received a communicationconcerning the partnership between Avesis and CardPayment Services (CPS), to transmit future claimspayment transactions via the MasterCard network. You mustenter the 6 digit prefix, the 10 digit MasterCard account number along with the 3 digit securitycode and expiration date. If your payment is still notprocessing, you may contact Avesis Provider ServicesRepresentative.Q: Why do I have to run the card for the exact amount?A: This is a security feature of the program.Q: When will payment be available for processing?A: Payment will be available for processing as soon asyou receive your specific remittance information.Q: What charges will I incur by participating in theprogram?A: This payment is processed like any other creditpayment and the terms of the agreement that youcurrently have with your MasterCard processor inorder to accept payments through their system is alsoapplicable to these transactions.6Q: How will I be notified that a MasterCard paymenthas been assigned to the remit?A: You will receive the remittance advice via email/faxor mail. The remittance advice contains the details ofthe invoices being paid along with the 10 digit number,security code and expiration date. All of theseelements are necessary to process your payment.Q: How can I begin receiving payment notifications viaemail instead of mail?A: During our initial letter introducing the program, weprovided a form to fill out that asks for the emailaddress.
