Franklin County Multiple Casualty Incident Response Plan FRANKLIN COUNTY MULTIPLE CASUALTY RESPONSE PLAN The following plan is intended to outline a process for proper pre-hospital care for accident victims of Multiple Casualty Incidents (MCI) in Franklin County of Massachusetts. If this plan conflicts with a town's Comprehensive Emergency Management Plan (CEMP), the CEMP plan takes precedence. I. NOTIFICATION AND DECLARATION NOTIFICATION OF A POSSIBLE MCI The first arriving responder, upon determining there are more victims than could be handled by one ambulance, should request an appropriate number of mutual aid ambulances (Appendix A). If the first responder estimates that the scene could possibly be considered an MCI, this information should be immediately relayed to their dispatch center; this information will be relayed to Shelburne Control if in an area not primarily dispatched by them. CONFIRMATION AND NOTIFICATION OF AN MCI The senior EMT on the scene, after determining that the scene is safe, will survey the area and provide the dispatch center a size-up notifying of the incident status and confirming if the incident is an MCI. If the scene is unsafe for EMT entry, but it is his/her determination that it is an MCI, he can confirm the MCI without entering the scene. An MCI declaration is indicated where the threshold level of patients is exceeded. The threshold level is the number of severely injured patients that can be routinely handled by the responsible ambulance service and its normal back-up service. A threshold level to be used as a guideline is: three (3) “priority one” patients or a combination of priority one and two patient that exceed the capacity of the primary ambulance and its first two back up units. “Priority one patients” are those in which there is an immediate threat to risk to life or limb if patient care is delayed. Franklin County Multiple Casualty Incident Response Plan In declaring an MCI the EMT will report the following to The Shelburne Control Communication Center: Type of incident Scene Safety: hazards which might interfere with rescue operations Extent of incident: number of injured Number of patients and how many are critical Severity of incident: Geographic area of incident Landing Zones to be used Mutual Aid Pre-Plan The initial Fire and EMS mutual aid system is the Tri-State Fire Mutual Aid System running orders as maintained by the Tri-State Fire Mutual Aid members in coordination with the dispatch center at Shelburne Control. The Tri-State Running Orders will list the first ten mutual aid ambulances to call in order for each community or district. Incidents that deplete ambulances beyond the ten Tri-State units listed will require additional resources; this may require deployment of additional single resource ambulances geographically based from the Franklin County MCI plan and/or deployment of a State Mobilization EMS Task Force. Note: - EMTs responding to an MCI should report to their station and respond to the scene as a unit, unless otherwise directed. - Ambulances should respond with additional EMTs if possible, however ambulance response should not be delayed due to this. - First Responders may be required to drive ambulances, if available, to free EMTs for patient care. - Upon arrival at scene, all personnel should report to the Designated Staging Area to insure accountability of responders. RESPONSIBILITIES OF THE SHELBURNE CONTROL COMMUNICATIONS CENTER In the event of an MCI in Franklin County, Massachusetts the Shelburne Control Communications Center is the designated DMAC - District Mutual Aid Center and referred hereafter as the Communications Center. All requests for personnel and equipment by the Incident Commander should be transmitted through the Communications Center. The Communications center should be immediately advised to the total number of patients and the corresponding number of patients tagged black, red, yellow, and green. Ambulances can typically transport 1 red tag patient and 1 yellow tag patient, or 1 red tag and 2 green tagged patients. The Communications Center should be advised when all patients have cleared the scene and must also be advised when the incident has been terminated. Upon confirmation of an MCI, the Communications Center is responsible for contacting: Fire and rescue agencies Local police State police Consider a priority traffic only announcement on the police frequencies for MCI event Additional EMS as requested (Appendix A) Consider EMS station cover Baystate Franklin Medical Center Additional hospitals (Appendix B) Activation of state HAZ-MAT plan if needed Activation of state DECON plan if needed Activation of WMEMS MCI trailer(s) if needed- Amherst fire (413-256-4899), or County Ambulance (in Pittsfield 413-499-2528) Ancillary agencies such as Mass Highways for traffic control Activation of the Massachusetts Fire and Emergency Medical Services Mobilization System (MFEMSMS) for ambulance resources, to facilitate requests for both fire and non-fire ambulance resources if the MFEMSMS Plan is activated the following shall apply: Notify the District Coordinator (Chief Annear of Orange Fire) of the request for outgoing assistance or the request for incoming resources. Coordinate the dispatch and tracking of requested resources within the District. Maintain an Emergency Resource Directory listing ALS and BLS transport resources and Task Force Leaders. Request additional resources under the Massachusetts State Fire and Emergency Medical Services Mobilization Plan according to prearranged procedures Mutual Aid Centers shall also follow Regional EMS notification protocols. (Tri-state Mutual Aid Run List) Notify MEMA at the State Emergency Operations Center of the activity and situation (if task forces are requested via NAWAS this notification should occur simultaneously). Franklin County Multiple Casualty Incident Response Plan III. DUTIES OF SECTION CHIEFS, COMMANDERS AND PERSONNEL The following sections provide general information about the organization of the MCI. The Incident Command System shall be used to coordinate the scene and provide for personnel and public safety. Checklist cards are included with this plan listing various Section Chief’s more specific roles and responsibilities in the incident. Incident Commander – The Incident Commander shall be dictated by the type of incident The Police Chief or senior police official, from the incident jurisdiction, or a State Police Officer, will be the Incident Commander (IC) in the following instances: - Civil Disturbances - Hostage or Sniper situations - Acts of Terrorism and or any crime scene The Fire Chief or senior fire official from the incident jurisdiction is the Incident Commander in: - All other situations The Incident Commander is identified by his/her “Incident Commander” reflective vest. He uses the Command Post as the scene communication center. The Command Post is made up of the first responding ambulance, first police car and a fire vehicle. The Command Post is identified by a white flag. (Appendix F: Frequency List) FOUO Page 6 of 84 5th Revision 2009

Franklin County Multiple Casualty Incident Response Plan The Incident Commander should delegate the duties of Fire, Police and EMS Section Chief. Duties of Incident Commander – Determines scene safety for responders. Decides if an evacuation is necessary. Oversees all scene operations making sure the efforts are as efficient and effective as possible. Establishes a Command Post Informs the Communications Center as to services enroute or already at scene before requesting additional services. Maintains communication with the Communications Center for requesting Mutual Aid, Ancillary Services and necessary equipment. Appoints a clerk to record scene developments and responders involved. Appoints, or acts as, a Public Relations Officer to communicate with the media. Communicates with the individual Section Chiefs: Fire, Police, EMS and in some situations Emergency Management and on-scene Ancillary Services. Determines the locations of the Command Post, Staging Area, Treatment/Patient Collection Area, morgue and helicopter landing zones. Notifies the Chief Town Official, through the Communications Center, of on-scene developments if necessary. Notifies the Communications Center and individual Section Chiefs of the termination of the MCI. Note: The ambulance Staging Area is to be located near the Patient Collection Area whenever possible. Staging Area may be established separately as needed for Fire and Police. These units will respond to specific parts of the scene as directed by radio. Staging Area for extra equipment can be established by the Incident Commander as needed. FOUO Page 7 of 84 5th Revision 2009

Franklin County Multiple Casualty Incident Response Plan Fire Section Chief – The Fire Chief from the jurisdiction of the incident or their designee is the Fire Section Chief. He is identified by his Section Chief's reflective vest. A designated fire unit becomes part of the Command Post and should be the last fire unit to leave the scene. Duties of Fire Section Chief FOUO Communicates with the Incident Commander and receives clearance to enter scene Contacts the Incident Commander concerning requesting additional equipment and personnel needs Oversees scene safety Recommends to Incident Commander if an evacuation is necessary Fire suppression, control and prevention Containment of hazardous or toxic materials Vehicle stabilization Coordinates heavy rescue equipment and techniques Victim extrication (but not patient removal) Provides lighting as necessary Manages Landing Zones for air medical resources May release fire personnel to control of EMS Section Chief for such purposes as driving ambulances or assisting in the removal of victims to the Patient Collection Area Is responsible for all fire personnel Notifies fire personnel of termination of MCI Page 8 of 84 5th Revision 2009

Franklin County Multiple Casualty Incident Response Plan Police Section Chief – The Police Chief, available senior police official from the town of the incident, or in some instances a State Police Officer, is the Police Section Chief. He is identified by his Section Chief's reflective vest. The first responding police car becomes part of the Command Post and should be the last police unit to leave the scene. Duties of Police Section Chief Communicates with the Incident Commander and receives clearance to enter scene After conferring with the Incident Commander, he can communicate with the Communications Center concerning: (a) what routes are to be kept clear for responding traffic and rescue egress, (b) needs for extra personnel or services Traffic management Oversees investigation of accident as needed Crowd control Incident area security Effecting any necessary evacuation of local citizens Security of equipment pool at Staging Area. Protects the bodies and belongings of the deceased and contacts Medical Examiner. Is responsible for all police personnel Notifies police personnel of termination of MCI FOUO Page 9 of 84 5th Revision 2009

Franklin County Multiple Casualty Incident Response Plan EMS Section Chief – The EMS Director/Manager, senior EMT of the first responding ambulance, having jurisdiction in the town of the incident, or their designee, is the EMS Section Chief. He is identified by his “EMS Section Chief” reflective vest. The first responding ambulance becomes part of the Command Post, and is the last ambulance to leave the scene. The ambulance may be relieved by a substitute vehicle with communication equivalent capability. If available, the first responding ambulance may later be moved to the Patient Collection Area for communications use by the Transportation Officer. Duties of EMS Section Chief Communicates with the Incident Commander and receives clearance to enter the scene Confirms MCI status through Communications Center Communicates the number and extent of casualties, including number of critical patients, to the IC for relay to the Communications Center Requests additional EMS resources through the Incident Commander Works with Incident Commander to locate the Patient Collection Area and Morgue Area Coordinates and insures the following positions roles are performed: Triage, Tagging, Treatment, Transportation, Communications and Staging Directs the efforts of any physicians on the scene, (physicians who arrive on the scene should follow Western Massachusetts EMS Pre-Hospital Protocols) Is responsible for all EMS and medical personnel Notifies EMS personnel of termination of MCI. Triage Officer – The junior EMT on the first ambulance is the Triage Officer. He/She will be identified by a “Triage” reflective vest. Duties of the Triage Officer Once the scene is cleared by the Incident Commander, he/she performs triage and tagging. Provide an approximate count of the total number of patients to the EMS Section Chief. Triage may be performed by a First Responder who has completed SMART Triage and MCI training if an EMT is unavailable. Directs movement of victims to the Treatment Area. Franklin County Multiple Casualty Incident Response Plan Treatment Officer – The Treatment Officer will be designated by the EMS Section Chief and will be a specifically trained EMT, if available. He/She will be identified by a “Triage” reflective vest. Duties of Treatment Officer Sets up a Treatment Area after communication with the EMS Section Chief. Coordinates activities at the Treatment Area. Communicates with EMS Section Chief if EMTs are needed to oversee treatment of individual patient category groups. Continually surveys victims and updates the victims’ status as necessary. Uses Patient Tally Sheet (Attachment B) to log all victims. Prioritizes patients for transport to hospitals and oversees ambulance assignment of victims according to prioritization. Communicates this information to the Transportation Officer. Loading/Transportation Officer – The Transportation Officer will be designated by the EMS Section Chief and will be a specifically trained EMT, if available. He will be identified by his reflective vest. (EMS Frequencies - Appendix F) Duties of Loading/Transportation Officer Oversees transport of victims according to prioritization established by the Treatment Officer. Uses Patient Tally Sheet (Attachment B) to log all victims who were transported. Coordinates patient distribution with hospitals to avoid overloading any one hospital. Directly handles all communications with nearby hospitals, unless patient condition worsens enroute. (i.e. ambulances in an MCI do not routinely provide radio reports to receiving hospitals) Coordinates communications with distant hospitals and helicopter transport via the Communications Center FOUO Page 11 of 84 5th Revision 2009

Franklin County Multiple Casualty Incident Response Plan Staging OfficerThe Staging Officer (if separate staging area is used): Responsible for units being held at staging area to insure an orderly response of resources when and as needed. Duties of Staging Officer – Insure the following are accomplished Transportation units are organized in a manner to insure they are readily available to respond to the needs of the Transportation Officer when requested. All vehicle crews shall stay in or with their units while at the staging area. Units are organized by vehicle type (ALS, BLS, Chairvan, etc) to assist in an orderly use of vehicles. The staging area shall be organized to allow the easy movement of vehicles and resources Request police presence if necessary to insure compliance with staging officer instructions FOUO Page 12 of 84 5th Revision 2009

Franklin County Multiple Casualty Incident Response Plan Appendix C - Hospital Mutual Aid The following are a list of hospitals to be contacted in the event that an MCI occurs in the named town. The listed hospitals should be contacted by the Communications Center, advised of the declaration of an MCI, and current status for accepting MCI patients obtained. This is also a guide for the Transportation Officer in determining patient distribution, based upon geographic considerations.
