ITEM 10Radiation Safety ProgramDusty Nagle has been designated as the company Radiation Safety Officer and will assume theduties and responsibilities that include the following:A.To ensure that all terms and conditions of the license are being met and that theinformation contained in the license is up-to-date.B.To ensure that the equipment has been leak tested in the required timely mannerand that the leak test is performed in the manner prescribed by the equipmentmanufacturer.C.To ensure that the use of the equipment is only by individuals that have beenauthorized by the Radiation Safety Officer and that all users wear personnelmonitoring equipment when utilizing the equipment. Personnel monitoringequipment will consist of Thermoluminescent Dosimeter supplied by TroxlerMonitoring Services on a quarterly exchange period.D.To maintain the records as required by the license and the regulations. Theserecords shall include personnel exposure records, leak test records and trainingcertificates for all users.E.To ensure that the equipment is properly secured against unauthorized removal atall times when it is not in use.F.To serve as a point of contact and give assistance in case of emergency such asequipment damaged in the field or theft and to notify the proper authorities in caseof emergency.G.To ensure that all users have read and understand the radiation safety operatingand emergency procedures.Operating Procedures:Transportation of Equipment:A.Care shall be taken to ensure that the equipment is tightly secured to thetransporting vehicle and positioned as far as practical from the occupantcompartment (s). When transporting in an enclosed vehicle, the gauge will besecurely fastened and locked to the truck bed. Additionally, the gauge will becovered by a locked bed cover.

ITEM 10 (cont.)B.The gauge will be transported in the Troxler transportation case. Department ofTransportation requires that the gauge be transported in a properly labeledcarrying case.C.At all times during transport, the operator will have a properly completed Bill ofLading for each gauge.Utilization proceduresA.When the gauge is in the field, you as the authorized user must maintain controlover the gauge at all times. The gauge must never be left unattended.B.When not making measurements, the gauge should be placed in the transportationcase and returned to its permanent storage area as soon as possible. The gauge isto be used for its intended purpose only. By doing so, you will maintain anyradiation exposure to as low as reasonably attainable.C.When using the equipment, you will wear personnel monitoring device that hasbeen assigned to you. When you are not using the equipment, your monitoringdevice is to be stored in the radiation free area that has been designated in theoffice.Maintenance and Leak Test Procedures.A.Periodic maintenance will include cleaning the gauge. During any maintenance,you must wear your personnel monitoring device.B.No maintenance will be performed in which the radioactive source is removedfrom the gauge. For this type of maintenance, the gauge will be returned to themanufacturer.C.The leak test will be performed using the Troxler Model 3880 Leak Test Kit. Theleak test will be performed using the manufacturer's instructions. Again thepersonnel monitoring device will be employed. Gauges will be leak tested atintervals not to exceed six (6) months.

ITEM 10 (cont.)Emergency Procedures.In the event of physical damage to a gauge, the following will be performed:A.Immediately cordon off an area around the gauge. An area radius of 15 feet willbe sufficient.B.If a vehicle is involved, it must be stopped until the extent of contamination, if any,can be established.C.A visual inspection of the gauge is to be made to determine if the source housingand/or shielding has been damaged.D.At the earliest possible time, when the situation is under control, you must contactthe Radiation Safety Officer at (304) 664-2223 ( main office),(home)Describe the present conditions and follow the instructionsorof the Radiation Safety Officer.E.In the event the gauge is lost or stolen, immediately notify the Radiation SafetyOfficer at the numbers listed above.PERSONAL INFORMATION WAS REMOVEDBY NRC. 11O COPY OF THIS INFORMATIONWAS RETAINED BY THE NRC.
