EFFECTIVE DATEWorldwide Automatic Travel Accident, Flight Delay,Baggage Delay, Excess Lost or Stolen Baggage andCar Rental Travel Accident insurance coveragesas well as Travel Assistance Services take effecton October 24, 2011 and will cease on the date theapplicable Master Policy is terminated (in whichcase you will be notified by Bank of Montreal(“Diners Club”)) or on the date that your DinersClub † Professional Card account ceases to be in goodstanding, whichever occurs first. Your policies ofinsurance for Car Rental Collision, Loss and Damageand for Purchase Assurance and Extended Warrantycoverages shall come into effect on October 24, 2011and will cease on the date the Qualified Travellers areno longer eligible to participate or on the date yourDiners Club Corporate Account ceases to be in GoodStanding, whichever occurs first.CERTIFICATES/POLICIES OF INSURANCEAND SUMMARY OF TRAVELASSISTANCE SERVICESThese Certificates/Policies of Insurance and theSummary of Travel Assistance Services provide asummary description of the principal provisions ofthe insurance coverages and travel assistance servicesavailable to eligible Cardmembers. WorldwideAutomatic Travel Accident insurance coverage isunderwritten by Chubb Insurance Company of Canadaand administered by DFS&A Insurance AgencyInc. (U.S. administrator) and CSI Brokers Inc. (planadministrator). Car Rental Travel Accident insurancecoverages are underwritten and administered byChubb Insurance Company of Canada. Flight Delayand Baggage Delay, Excess Lost or Stolen Baggageinsurance coverages are underwritten by ChubbInsurance Company of Canada and administered byWorld Travel Protection Canada Inc. (“WTP”).Complete provisions pertaining to these insurancecoverages are contained in the applicable MasterPolicies issued to Bank of Montreal (the “Policyholder”)and referenced in these Certificates of Insurance.If a statement in a Certificate of Insurance and anyprovision in the applicable Master Policy differ, subjectto applicable law, the Policy will govern. The InsuredPerson and any claimant under this insurance mayrequest a copy of the Master Policies, subject to certainaccess restrictions. Travel Assistance Services areprovided and administered by WTP. Travel AssistanceServices are not insurance benefits.2

Car Rental Collision, Loss and Damage and PurchaseAssurance and Extended Warranty insurancecoverages are underwritten by Allianz Global Risks USInsurance Company (Canadian Branch) and offered toyou under an individual policy. The Insured Person andany claimant under this insurance may request a copyof the policy subject to certain access restrictions. Yourpolicy number is your Diners Club card number. Thisinsurance is administered by AZGA Insurance AgencyCanada Ltd. operating as Allianz Global Assistance.SUMMARY OF INSURANCE BENEFITSAND TRAVEL ASSISTANCE SERVICESWORLDWIDE AUTOMATICTRAVEL ACCIDENT - CERTIFICATE OFINSURANCEThe coverage outlined in this Certificate of Insuranceis effective as of October 24, 2011 and is provided to alleligible Diners Club Professional Card Cardmembersby Chubb Insurance Company of Canada (the“Company”) under Master Policy number 6405-91-10(hereinafter referred to as the “Policy”) issued by theCompany to Bank of Montreal (hereinafter called the“Policyholder”).As a Diners Club Professional Card Cardmember,you, your Spouse and your Dependent Child(ren) willbe automatically insured against accidental loss oflife, limb, sight, speech or hearing while riding as apassenger in, entering or exiting any licensed CommonCarrier, provided the entire cost of the passengerfare(s), less redeemable certificates, vouchers,coupons, or free flights awarded from frequent fliercards, has been accumulated or charged to your DinersClub Professional Card account.If the entire cost of the passenger fare has been chargedto your Diners Club Professional Card account prior todeparture for the airport, terminal or station, coverageis also provided:a) for Common Carrier travel (including taxi, bus,train or airport limousine, but not includingcourtesy transportation provided without a specificcharge), immediately, preceding your departure,directly to the airport, terminal or station;b) while at the airport, terminal or station;c) for Common Carrier travel immediately followingyour arrival at the airport, terminal or station ofyour destination.3

If the entire cost of the passenger fare has not beencharged to your Diners Club Professional Card accountprior to your arrival at the airport, terminal or station,coverage begins at the time the entire cost of thetravel passenger fare is charged to your Diners ClubProfessional Card account.Benefit Amount means the benefit amount payableunder either the basic amount or the optional levelof coverage, if you are enrolled, of Worldwide TravelAccident Insurance at the time that the entire cost ofthe passenger fare(s) is charged to your Diners ClubProfessional Card account. If you are enrolled for anoptional level of coverage, the applicable WorldwideTravel Accident Insurance benefit amount will beindicated on your letter of confirmation.Common Carrier means any land, water or airconveyance operated by those whose occupationor business is the transportation of persons withoutdiscrimination and for hire. This coverage does notinclude commutation which is defined as travelbetween the Insured’s permanent residence andregular place of employment.Dependent Child(ren) means unmarried children,including adopted children, those children placedfor adoption and step-children, who are primarilydependent upon the Cardmember for maintenanceand support and who are under the age of 25 andreside with the Cardmember, beyond the age of 25 whoare permanently mentally or physically challengedand incapable of self support, or up to the age of 25 ifclassified as a fulltime student at an institute of higherlearning.Insured means the Diners Club Professional CardCardmember, his or her Spouse and his or herDependent Child(ren), provided the entire cost ofthe passenger fare(s), less redeemable certificates,vouchers, coupons, or free flights awarded fromfrequent flier cards, has been accumulated or chargedto the Cardmember’s Diners Club Professional Cardaccount.Loss means, with respect to a hand, completeseverance through or above the knuckle joints of atleast 4 fingers on the same hand; with respect to a foot,complete severance through or above the ankle joint.The Company will consider it a loss of hand or footeven if they are later reattached.Member means a hand or foot. One quarter of theBenefit Amount is payable for the accidental loss of thethumb and index finger of the same hand.Spouse means either a person to whom theCardmember is lawfully married, or the commonlaw spouse of a Cardmember. Common-law spouse4

shall mean a person (of the same or opposite sex) whohas been living with a Cardmember continuously forat least one year and is publicly represented as theCardmember’s partner.ELIGIBILITYThis Worldwide Automatic Travel Insuranceis provided to Diners Club Professional CardCardmembers automatically when the entire costof the passenger fare(s) are charged to your DinersClub Professional Card account while the insuranceis effective. It is not necessary for you to notify theadministrator or the Company when tickets arepurchased.BENEFICIARYThe loss of life benefit will be paid to the beneficiarydesignated by the Insured. If no such designationhas been made, that benefit will be paid to the firstsurviving beneficiary in the following order:a) the Insured’s spouse;b) the Insured’s child(ren);c) the Insured’s parents;d) the Insured’s brothers and sisters;e) the Insured’s estate.All other benefits will be paid to the Insured.THE BENEFITSThe full Benefit Amount is payable for accidental lossof life, two or more Members, sight of both eyes, speechand hearing or any combination thereof. One half of theBenefit Amount is payable for accidental loss of: oneMember, sight of one eye, speech or hearing.The Loss must occur within one year of the accident.The Company will pay the single largest applicableBenefit Amount.In no event will duplicate request forms or multiplecharge cards obligate the Company in excess of thestated Benefit Amount for any one Loss sustained byany one individual Insured as the result of any oneaccident.The Benefit Amount for an Insured whose coveragehas become effective shall be: 300,000 Travel Accident Insurance BenefitIn the event of multiple accidental Insured deaths percredit card account arising from any one accident, theCompany’s liability for all such Losses will be limited5

to a maximum limit of insurance equal to three timesthe applicable Benefit Amount for loss of life.Benefits will be proportionately divided among theInsureds up to the maximum limit of insurance.EXCLUSIONSThis insurance does not cover loss resulting from:1) an Insured’s emotional trauma, mental orphysical illness, disease, pregnancy, childbirth ormiscarriage, bacterial or viral infection (exceptbacterial infection caused by an accident orfrom accidental consumption of a substancecontaminated by bacteria), or bodily malfunctions;2) suicide, attempted suicide or intentionally selfinflicted injuries;3) declared or undeclared war, but war does notinclude acts of terrorism;4) commutation which is defined as travel betweenthe Insured’s permanent residence and regularplace of employment.This insurance also does not apply to an accidentoccurring while an Insured is in, entering, or exitingany aircraft while acting or training as a pilot orcrew member, but this exclusion does not apply topassengers who temporarily perform pilot or crewfunctions in a life threatening emergency.CLAIMS PROCEDURESAs a condition to the payment of benefits under thisinsurance, the Company will need certain informationfrom you. This information will include, at a minimumand is not limited to, the following (as applicable): A copy of the invoice for your travel by CommonCarrier showing your Diners Club Professional Cardas the method of payment for the passenger fare(s); Certified Death Certificate; Medical records pertaining to the accident; and Police report or any other accident reports filed.For claims or questions, please contact:DFS&A Insurance Agency Inc. Complete provisions pertainingto this Worldwide Automatic Travel Accident Insuranceare contained in the Policy on file with the Policyholder.Any terms of this Certificate of Insurance which are inconflict with the applicable provincial statutes, lawsor regulations of the province or territory in whichthis Certificate of Insurance is issued are amendedto conform with such statutes. If a statement in thisCertificate of Insurance and any provision of the Policydiffer, subject to applicable law, the Policy will govern. 500 FLIGHT DELAY COVERAGE CERTIFICATE OF INSURANCEThe coverage outlined in this Certificate of Insuranceis effective as of October 24, 2011 and is provided to alleligible Diners Club Professional Card Cardmembersby Chubb Insurance Company of Canada (the“Company”) under Master Policy number 6405-98‑99(hereinafter referred to as the “Policy”) issued by theCompany to Bank of Montreal (hereinafter called the“Policyholder”).7

PERSONS COVERED – “QUALIFIED TRAVELLERS”The following persons (“Qualified Travellers”) areeligible for coverage: any person named on an airline ticket purchasedwith a Diners Club Professional Card and whotravels on a regular, special or chartered flight bya scheduled airline; or on a published, regularlyscheduled flight by a chartered airline; any non ticketed infant up to two (2) years of ageprovided that the infant is endorsed on a ticket andthe ticket is purchased with the Cardmember’sDiners Club Professional Card.COVERAGEQualified Travellers are eligible for reimbursement forreasonable meal and living accommodation expenses(including ground travel immediately to or from theairport) incurred during the period of the flight delaysubject to a maximum of 500 provided that: the full fare for the delayed flight was paid with aDiners Club Professional Card; the delay lasted in excess of four (4) hours from thescheduled time of departure; such reasonable expenses are limited to meals,living accommodations, and ground travelimmediately to or from the airport for which theQualified Traveller can provide receipts; such unavoidable flight delay was the result ofstrike by airline personnel, quarantine, civilcommotion, hijack, natural disaster or inclementweather.EXCLUSIONSCoverage does not apply when the delayed flight wasdue to: mechanical breakdown of the aircraft; air traffic delays caused by congestion in the skies; any criminal act by the Qualified Traveller.Flight Delay coverage does not apply to Diners ClubProfessional Card Cardmembers travelling on free ornon-revenue tickets obtained through any frequentflyer program or promotion.CLAIMSTo file a Flight Delay claim, you must contact theClaims Administrator (World Travel Protection CanadaInc.) as soon as reasonably possible to request a claimform:8

Toll-free: 866-865-2617 (In Canada & U.S.)Collect: (416) 977-7743 (from elsewhere)Completed claim forms should be returned to theClaims Administrator within 90 days from the date ofthe incident or the loss.Completed claim forms must be accompanied by thefollowing documentation and forwarded by mail to theClaims Administrator: proof of the flight delay from the airline, includinglength of delay; original itemized receipts for all expenses incurredas a result of the delay (limited to meals, hotelaccommodations and ground transportationimmediately to or from the airport); a copy of your airline ticket or Diners Club billingstatement showing proof that the airline ticket forthe delayed flight was purchased with your DinersClub Professional Card. 500 BAGGAGE DELAY COVERAGE CERTIFICATE OF INSURANCEThe coverage outlined in this Certificate of Insuranceis effective as of October 24, 2011 and is provided to alleligible Diners Club Professional Card Cardmembersby Chubb Insurance Company of Canada (the“Company”) under Master Policy number 6405‑98‑99(hereinafter referred to as the “Policy”) issued by theCompany to Bank of Montreal (hereinafter called the“Policyholder”).PERSONS COVERED – “QUALIFIED TRAVELLERS”The following persons (“Qualified Travellers”) areeligible for coverage: any person named on an airline ticket purchasedwith a Diners Club Professional Card and whotravels on a regular, special or chartered flight bya scheduled airline; or on a published, regularlyscheduled flight by a chartered airline; any non ticketed infant up to two (2) years of ageprovided that the infant is endorsed on a ticket andthe ticket is purchased with the Cardmember’sDiners Club Professional Card.COVERAGEQualified Travellers are eligible for reimbursement forthe reasonable cost of emergency cash outlays for thepurchase of necessary clothing and personal hygienearticles, required as a result of a covered baggage delay,subject to a maximum amount of 500 provided that: the full fare for the flight on which the baggage9

was checked was paid for with the Diners ClubProfessional Card; such baggage was unavoidably delayed by theairline for more than six (6) hours; such baggage was in the custody of the airline; the baggage delay occurred at an airport other thanin the city or metropolitan area of the city in whichthe Qualified Traveller is resident or during the lastleg of their return destination.EXCLUSIONSCoverage does not apply: when purchases related to the delayed baggagewere made after the return of the baggage; when purchases related to the delayed baggagewere made more than ninety-six (96) hours afterthe arrival of the Qualified Traveller’s flight.Baggage Delay coverage does not apply to DinersClub Professional Card Cardmembers travelling onfree or non-revenue tickets obtained through anyfrequent flyer program or promotion.CLAIMSTo file a Baggage Delay claim, you must contact theClaims Administrator (World Travel Protection CanadaInc.) as soon as reasonably possible to request a claimform:Toll-free: 866-865-2617 (In Canada & U.S.)Collect: (416) 977-7743 (from elsewhere)A claim form will be forwarded to your attention forcompletion.Completed claim forms should be returned to theClaims Administrator within 90 days from the date ofthe incident or the loss.Completed claim forms must be accompanied by thefollowing documentation and forwarded by mail to theClaims Administrator: copy of the delayed baggage report filed with theairline; copy of the confirmation receipt from the airlineshowing the date and time that your baggage wasreturned to you; original itemized receipts for all expenses incurredas a result of the delay (limited to necessaryclothing and personal hygiene items); a copy of your airline ticket or Diners Club billingstatement showing proof that the airline ticket waspurchased with your Diners Club Professional Card.10

1,000 EXCESS LOST OR STOLENBAGGAGE COVERAGE - CERTIFICATEOF INSURANCEThe coverage outlined in this Certificate of Insuranceis effective as of October 24, 2011 and is provided to alleligible Diners Club Professional Card Cardmembersby Chubb Insurance Company of Canada (the“Company”) under Master Policy number 6405-9899 (hereinafter referred to as the “Policy”) issued bythe Company to Bank of Montreal (hereinafter calledthe “Policyholder”).PERSONS COVERED – “QUALIFIED TRAVELLERS”The following persons (“Qualified Travellers”) areeligible for coverage: any person named on an airline ticket purchasedwith a Diners Club Professional Card and whotravels on a regular, special or chartered flight bya scheduled airline; or on a published, regularlyscheduled flight by a chartered airline; any non ticketed infant up to two (2) years of ageprovided that the infant is endorsed on a ticket andthe ticket is purchased with the Cardmember’sDiners Club Professional Card.COVERAGEQualified Travellers are eligible for reimbursement ofup to 1,000 of which no more than 100 will apply toall jewellery (including watches) and of which no morethan 250 will apply to golf clubs (including bags) forloss, theft or damage of checked or carry on baggageand personal property contained therein provided that: the full cost of the airline ticket has been charged toyour Diners Club Professional Card; the baggage was checked with the airline (coveragefor checked baggage begins with curbside check-inwith airline designated personnel) or carried onboard by the Qualified Traveller.Coverage is in excess of all other insurance availableto the Qualified Traveller (i.e. airline or personal homeinsurance).Payment is based on actual replacement cost of any lostor stolen article without deduction for depreciation,provided that the article is actually replaced; otherwise,payment is based on the actual cash value of the articleat the time of the loss.11

EXCLUSIONSCoverage does not apply to: specific items of personal property: animals,sporting equipment (except golf clubs and bags upto a combined total of 250), cameras and accessoryequipment, furs, tickets, eye glasses and contactlenses, prosthetic devices including dentures,valuable papers and documents, securities,negotiable instruments and money; loss resulting from confiscation, expropriation ordetention by any government, public authority,customs or other officials; loss resulting from nuclear fuel or waste from thecombustion of nuclear fuels.Lost or Stolen Baggage coverage does not applyto Diners Club Professional Card Cardmemberstravelling on free or non-revenue tickets obtainedthrough any frequent flyer program or promotion.CLAIMSTo file a Lost or Stolen Baggage claim, you must contactthe Claims Administrator (World Travel ProtectionCanada Inc.) as soon as reasonably possible to requesta claim form:Toll-free: 866-865-2617 (In Canada & U.S.)Collect: (416) 977-7743 (from elsewhere)A claim form will be forwarded to your attention forcompletion.Completed claim forms should be returned to theClaims Administrator within 90 days from the date ofthe incident or the loss.Completed claim forms must be accompanied by thefollowing documentation and forwarded by mail to theClaims Administrator: copy of the lost or damaged baggage report filedwith the airline; proof of settlement from the airline for items lost orstolen; proof of settlement from your personal insurancecompany; itemized receipts for replaced items; a copy of your airline ticket or Diners Club billingstatement showing proof that the airline ticket waspurchased with your Diners Club Professional Card.12

CAR RENTAL COLLISION, LOSS ANDDAMAGE INSURANCE - POLICYOF INSURANCEThe coverage outlined in this Policy of Insurance iseffective as of October 24, 2011 and is provided to alleligible Diners Club Professional Card Cardmembersby Allianz Global Risks US Insurance Company(Canadian Branch). Your individual policy numberis your Diners Club card number.ELIGIBILITY – “QUALIFIED TRAVELLERS”The following persons (“Qualified Travellers”) areeligible for coverage: any Diners Club Professional Card Cardmemberwho charges the full car rental fee of a RentedAutomobile on a Diners Club Professional Card,who operates the Rented Automobile in accordancewith the terms of the rental contract and who, at thetime of the rental, declines any optional collisiondamage insurance or waiver (CDW) offered by thecar rental company; all additional drivers whose names appear on thecar rental contract, or who are covered as permitteddrivers under the terms of the rental contract.COVERAGEThe Diners Club Professional Card Cardmember willbe reimbursed for sums paid for repair of damage orloss to the Rented Automobile due to collision, fire,theft or vandalism, up to the full value of the RentedAutomobile provided: the full car rental fee for the Rented Automobilewas paid for by a Diners Club Professional Card; the Cardmember declined at the time of rental anyoptional collision damage insurance or waiver(CDW) offered by the car rental company.Coverage is: primary and allows for full recovery of loss ordamage costs without having to file a claim withyour personal insurance company; Worldwide unless local laws prohibit this typeof coverage. Any deductible imposed on theCardmember in this case will be reimbursed wherenot prohibited; in effect for car rentals of up to a maximum of48 consecutive days.13

Definition of Rented Automobile:“Rented Automobile” means a four-wheeled privatepassenger motor vehicle designed for travel on publicroads and rented from a licensed rental companylocated anywhere in the world, for no more than48 consecutive days. It does not include four wheeldrive vehicles (when being used off normal pavedor maintained road surfaces), trucks, pickup trucks,campers, trailers, recreational vehicles or motorvehicles propelling or towing a trailer or any otherobject, vans or mini-vans that are manufactured to seatmore than eight (8) occupants (including the driver)or when the vehicle is used to carry, haul or transportany type of cargo or property or passengers for hire.Off-road, antique, limited edition, special interest,high value motor vehicles and exotic cars are excluded(including Aston Martin, Bentley, limousines, Daimler,Excalibur, Ferrari, Jensen, Lamborghini, Lotus,Maserati, Porsche, Rolls Royce or similar vehicles).Antique motor vehicles are defined as any vehicleover 25 years old, or any vehicle which has not beenmanufactured for 10 years or more. Back to back rentalsof more than 48 consecutive days are excluded. Back toback rental is defined as two or more rentals of the sameor different vehicles within 120 km radius with the firstending and the next beginning within a 24 hour period.EXCLUSIONSCoverage does not apply to loss: resulting directly or indirectly from any dishonest,fraudulent or criminal act by the Cardmember; resulting from forgery; resulting from violation of the car rental contract bythe Cardmember; resulting from the Cardmember being legallyintoxicated or under the influence of any narcotic,unless prescribed by a licensed physician; resulting from intentional damage to the RentedAutomobile by the Cardmember; resulting from war; resulting from radioactive contamination, nuclearreaction or nuclear radiation.Coverage also does not apply to: damage or loss of personal effects in the RentedAutomobile; damage to tires, unless damaged by fire, maliciousmischief or vandalism, or stolen, or unless the lossis coincident with and from the same cause asanother loss covered by this insurance; damage which is due and confined to wear andtear, freezing, mechanical or electrical breakdown14
