Parish NurseServicesThe Parish Nursing Program promotes healthand wellness for seniors and vulnerablepopulations in our community, includinghomeless individuals and families, people wholack access to health care and disenfranchisedveterans. Directing people to appropriateproviders, linking them to primary medicalhomes and identifying appropriate resourcesto meet their health needs are part of ourParish Nurses’ responsibilities. Throughpartnerships with community agencies, clinicsand churches, the program also providesopportunities to socialize, be with peers andimprove family relationships. Services includehealth education and emotional support,health care management, health assessments,observation and referrals into care. Servicesare provided at four church locations and ahomeless facility.In fiscal year 2014, we provided Parish Nurseservices to 5,326 clients. We are currentlyexpanding this program and are seekingpartnerships with nurses who provide similarservices in their faith communities.If you are a nurse interested in a partnership,please call our Faith Community Coordinatorat 954-267-9551.

MobileHealth UnitProgramThe Mobile Health Unit has been providinghealth care in our community since 2009and is dedicated to expanding accessto health services for the homeless andminority populations in targeted areas.Services onboard the van include:» Rapid HIV and OraSure testing» Rapid hepatitis C testing» Health exams by a doctor or an advancedregistered nurse practitioner (ARNP)» Health screenings (cholesterol, diabetes,blood pressure and pregnancy)» Care management» Links to health care services andaffordable health care navigators» Childhood immunizations» Flu vaccine shotsThe Mobile Health Unit operates up tosix days per week. In fiscal year 2014,it served 2,497 people.

GrowingHealthy KidsGrowing Healthy Kids is dedicated to improvingknowledge, attitudes and behaviors towardgood nutrition, fitness and self-image. Itsprograms target underserved, lower-income,at-risk elementary school students in parochialand public schools. We partnered withcommunity organizations such as the 4-H Club,Florida Agriculture in the Classroom and theFlorida Farm Bureau. As a result, 159 classroomtrainings and healthy lifestyle tools wereprovided to 391 teachers and 3,745 studentsand parents in fiscal year 2014.The Pilates for Girls program aims to improve themental and physical health of female studentsand their teachers. MELT (myofascial energeticlength technique) classes were also providedto 274 school faculty members, parents andseniors. MELT enhances body awareness,rehydrates connective tissue, decreases stuckstress that accumulates from daily living andreturns the body to a more ideal state.Growing Healthy Kids also guided schools inplanting 83 on-site produce gardens that werecultivated and harvested by students. Thisexperience gave students, their families andschool communities new attitudes towardfood, nutrition, the earth, green programs andsustainability.

HomelessServices andHunger InitiativesWe participate in Broward County’s annual Point-inTime (PIT)—a reliable count of people experiencinghomelessness in our community. The data isused by government agencies to properly allocateassistance funds. The 2014 PIT count identified2,766 homeless individuals.Our response to our community’s homelessnessissues included:» Placing a Parish Nurse at a homeless day shelterand at church-based homeless feeding programs» Providing outreach screening services: breasthealth, blood pressure, blood sugar, cholesterol,and HIV and hepatitis testing (as well asassistance with necessary follow-up services)» Partnering with Hope South Florida to create aWellness Center with on-site health care servicesand education for homeless families» Joining homeless agency boards of directors» Serving as a leading community partner on theSouth Florida Hunger Coalition—a resource forraising food, funds and community awareness» Having Holy Cross Associates donate anddistribute food to small food pantries in BrowardCountyIn fiscal year 2014, our homeless services andhunger initiatives also provided:» Services to 2,177 homeless individuals» More than 3 tons of food to Broward’s food pantries

SchoolHealthProgramThe School Health Program gives thousandsof students access to school-based vision,hearing, immunization and scoliosisscreenings, as well as health educationclasses and immunizations. In addition,classes on CPR, blood-borne pathogens andstudent-related health concerns (diabetes andHIV/AIDS, for example) are available to schoolpersonnel. The program also provides healtheducation in nontraditional school settings,such as the Broward County JuvenileDetention Center.One of the goals of the School HealthProgram is to raise the immunization ratesof school-age children in our community.In addition to providing immunizations fromthe Centers for Disease Control’s Vaccinesfor Children initiative to eligible children,members of the program actively participatein Broward’s Immunization Coalition.In fiscal year 2014, the School HealthProgram provided:» 5,841 screenings» 1,990 immunization reviews» 4 82 immunizations» 491 referrals for follow-up» 88 health education classes(1,065 students and 428 adults)

GoodstartProgramThe Goodstart Program provides healtheducation and promotes wellness toat-risk preschool students at theirdaycare facilities. In addition, monthlycaregiver workshops educate providerson children’s health issues. At theseworkshops, caregivers obtain thenecessary credits to stay licensed andremain in good standingfor employment.Partnering with Broward Impact,Goodstart presents monthly trainingfor seniors working with young children.In addition, a partnership with the JanMoran Collier City Library promoteshealth literacy through a monthlybook club. Partners inBreast HealthThe Partners in Breast Health (PIBH) programaims to increase access to breast health servicesin underserved communities throughout BrowardCounty. The goal is to save lives by reducing barriersand providing education and early detection.Partner community health agencies, federallyqualified health centers and outreach workers givereferrals to the program, and women are assessedfor program eligibility. At an appointment, a physicianand/or an advanced registered nurse practitioner(ARNP) may provide one-on-one education, generalhealth assessments, a clinical breast exam anda mammogram referral for women ages 40 andolder (or for younger women with symptoms ofbreast cancer). Follow-up care assists patients withdiagnostics, care and treatment. Special needs areoften met via the hospital’s financial assistanceprogram. PIBH receives private and grant funding forscreening and diagnostic services.In fiscal year 2014, PIBH provided1,253 screenings and diagnostic services(mammograms, ultrasounds, magnetic resonanceimaging [MRI] tests and biopsies).

Diabetes:Chronic DiseaseManagementEncouraging individuals and their health careproviders to use preventive health services, suchas disease self-management, chronic diseasemanagement, and participation in primary medicalhomes are key components in helping our communityremain healthy.During this past year and a half, Holy Cross Hospitalhad a dedicated nurse provide outreach educationto clients with diabetes in our community. A lack ofunderstanding about diabetes (and related diagnoses)often contributes to hospital admission, readmissionsand frequent visits to the emergency room. TheDiabetic Nurse Educator gives health information topeople with diabetes, their families and caregivers,paying special attention to health literacy and culturaland linguistic appropriateness. Understanding howto take control of diabetes empowers our communitymembers to take control of their health and future.In fiscal year 2014, our diabetes care included:» Lifestyle coaching: 116 people» Group education: 74 people» Blood sugar, total blood cholesterol andA1c screenings: 285

HIV andHepatitis CTestingSince 2008, Holy Cross Hospital hasprovided easy access to rapid HIVtesting. In 2014, we were able to addaccess to rapid hepatitis C testingfor our community. Testing servicesare located at targeted outreach sitesand aboard the Mobile Health Unit.All clients are given pre- and posttest counseling, and those peopleconfirmed to be positive are directedto appropriate care and treatment. Inaddition to the HIV and hepatitis tests,clients are offered blood pressure, bloodglucose and cholesterol screenings.In fiscal year 2014, Holy Cross providedHIV testing services for 8,791 peopleand performed 3,938 HIV tests. Thepositivity rate was 0.2 percent.We also performed 551 hepatitis C tests,and the positivity rate was 3 percent.
