If the student is unable to demonstrate satisfactory skillperformance and/or is unsafe in the clinical externship, the student will be required toreturn to Pathways College campus for additional training in the unsatisfactory areas.During the externship course, the student may be required to return to campus for weeklyscheduled meetings with the Instructor to provide continued communication and feedbackregarding the clinical externship experience. The clinical externship the clinical Instructorwill provide frequent daily ongoing verbal communication and feedback regarding thestudent’s progress in the clinical setting. The formal written Clinical PerformanceEvaluation occurs twice during the externship period, once at midpoint and again at theend of the externship period.If a student’s work habits are not satisfactory at the first midpoint evaluation, the Instructorwill meet with the student and review the areas of weakness and outline a plan forimprovement. If work performance is still unsatisfactory at the end of the externshipperiod, the student may be required to serve an additional externship. A student may onlyrepeat the clinical externship once. If the student fails on the second clinical externshipthe student will be dropped from the program.The Clinical Performance Evaluation Form will be used as a terminal evaluation of thestudent at midpoint and upon completion of the clinical externship. The guidelines forassessing the student’s performance will be reviewed periodically with the student andappropriate marks are made on the Clinical Performance Evaluation Form grid identifyingareas of strength ( ) or weakness (-) related to the guidelines. Based on that informationeach objective will be identified as having been met in a “satisfactory” manner, or“unsatisfactory” manner. Unsatisfactory grades will be accompanied with Instructordocumentation evidence. ALL CLINICAL EXTERNSHIP OBJECTIVES MUST BE METIN A SATISFACTORY MANNER for successful completion of the course.Note: Specific guidelines for assessing the student’s abilities are listed under eachobjective on the Clinical Performance Evaluation Form.10

ABSENCE/MAKE-UP POLICYTo qualify for State Certification as a Nursing Assistant, the student must attend aminimum of 50 hours of theory (classroom) and 100 hours of supervised clinical trainingin a nursing facility. A strict mandatory daily attendance record is maintained.1. Attendance is recorded and maintained daily.2.
