LOCATOR R-Tx Abutment Placement12A1 Identify the system and diameter of each implant.Remove the healing abutments.2B2A-2B Using a periodontal probe, measure theheight of the gingiva at the highest point and selectthe cuff height of the LOCATOR R-Tx Abutment thatcorresponds to that measurement. If it is 2mm, choosea 2mm cuff height. An additional 2mm of abutmentheight will extend above the gingiva to accommodatethe seated Denture Attachment Housing and NylonRetention Insert.33Place the LOCATOR R-Tx Abutment into theimplant using a .050”/1.25mm* hex driver and handtighten. Radiograph each interface to confirm that theAbutments are fully seated on the implants by placingthe film perpendicular to the interface.44Complete the seating of the R-Tx Abutment by usinga calibrated torque wrench. Tighten the LOCATOR R-TxAbutment to 30Ncm.NOTE: LOCATOR R-Tx Abutments designed forimplants with an internal thread diameter of 1.4mmmust be torqued to 20Ncm. The use of higher torquevalues than recommended could cause a fracture ofthe LOCATOR R-Tx Abutment.A direct or indirect technique may be used forprocessing the Denture Attachment Housings into thedenture. Please refer to page 8 for direct and page 11for indirect processing techniques for the LOCATORR-Tx Removable Attachment System.7

LOCATOR R-Tx Processing Denture Attachment Housings into the Denture - Direct Technique11 Place a White Block-Out Spacer around eachAbutment and press it down to the tissue. Snap aDenture Attachment Housing with a pre-loaded BlackProcessing Insert onto each Abutment, pressing downfirmly.22Apply fit check marking paste to the intaglio surfaceof the denture. Insert it into the mouth in position overthe Denture Attachment Housings. This will mark areaswhere the denture will need to be relieved to allowspace for the Housings to be picked up.3A3B3A-3B Relieve the marked areas with theCHAIRSIDE Recess Bur. ZEST recommends using slightpressure and a small rocking motion to get the tip of theBur started, followed by a straight downward motionto create the desired recess site. This Bur has distinctdepth landmarks which indicate where to stop whendrilling for the Denture Attachment Housing.4B4A-4B Use the CHAIRSIDE Undercut Bur to cutan undercut around the circumference of the reliefareas for mechanical retention. Cut lingual/palatal ventwindows in the denture with the CHAIRSIDE Vent Bur tovisualize full seating and for excess material to vent.LOCATORR-TX4A8

Apply asmall amount of CHAIRSIDE Attachment ProcessingMaterial around the circumference of each Housing.Place CHAIRSIDE Material into the relief areas of thedenture and seat it over the Housings and onto thetissue. Have the patient close into light occlusion andhold while the CHAIRSIDE Material sets. Please refer toCHAIRSIDE Attachment Processing Material IFU for settimes.NOTE: Excessive occlusal pressure during the settingtime may cause tissue recoil against the denture baseand could contribute to dislodging and prematurewear of the Inserts.66 Disengage the denture from the Abutments andremove from the mouth. Verify that the DentureAttachment Housings have been securely processedinto the denture. Fill any voids with CHAIRSIDE Materialand light cure. The material will bond to itself and willcure within 30 seconds with light application. Removeany excess material with the CHAIRSIDE Grind Bur.77 Use the CHAIRSIDE Trim Bur to remove any excessacrylic material remaining on the denture and to makeany needed modifications to the denture.88 Use the CHAIRSIDE Polish Bur to create a smoothfinish of the denture.9

When usingthe removal end of the Tool, engage the ProcessingInsert and tip the Tool towards the distal or mesialof the intaglio surface until the Insert completelydisengages from the Denture Attachment Housing.Use the metal slot feature on the back end of the Toolto remove the Insert from the tip of the Tool. This canbe accomplished by sliding the Insert into the openingof the metal slot and using an upward motion toremove the Insert.101110Place the selected final Retention Insert into eachDenture Attachment Housing using the insertion (IN)end of the Tool. During try-in, start with the lowestretentive option. When using the insertion end of theTool, engage the Retention Insert from a slight angleand upright the Tool while pressing down.11Seat the denture, press down to engage the Insertson the LOCATOR R-Tx Abutments and verify theocclusion. Instruct the patient on how to remove andinsert the denture. If the retention is not satisfactory,remove the Inserts and replace with the next level ofretention. Please refer to the Retention Inserts sectionof the Components Reference on page 2 for availablelevels of retention. Instruct the patient on proper homecare maintenance and required recall visits.10

Ensurethat each recess has enough space for the height anddiameter of the LOCATOR R-Tx Impression Copings.6.5mm5mm22 Place a LOCATOR R-Tx Impression Coping on eachAbutment and press down firmly. Syringe a mediumbody impression material around the circumference ofeach coping. Fill the impression tray and insert it overthe copings and onto the tissue. Allow the material toset and remove the impression tray.3311Seat the appropriate diameter LOCATOR R-TxAnalog into each Impression Coping and send theimpression to the laboratory.

LOCATOR R-Tx Processing Attachment Housings into the Denture - Indirect Technique (continued)44 Verify that the Analogs are secure in the ImpressionCopings and pour a model.55 Fabricate the baseplate and wax rim on the cast forthe bite registration. The Denture Attachment Housingswith Black Processing Inserts may be processed into thebaseplate to provide stabilization during record makingand try-in.66BITE RECORDSPlace the bite block into the mouth and record the jawrelation. Take an impression of the opposing arch andpour the cast. Select a shade for the denture teeth.77Articulate the models and proceed with the dentureteeth set up.12

LOCATOR R-Tx Processing Attachment Housings into the Denture - Indirect Technique (continued)88DENTURE TRY-INPlace the try-in denture into the mouth and verify thefit, attachment engagement, aesthetics, phonetics andocclusion.99 Finalize and flask the denture for processing.Separate the flask and boil away all wax. Place theDenture Attachment Housings with Black ProcessingInserts on the Analogs and press down firmly. Placethe cast back into the flask and verify that there isno contact with the teeth. Close the flask and processthe denture. Remove the denture from the flask, finish,and polish.101011AOptional Step: A Processing Spacer could be usedinstead of the Denture Attachment Housing during thefabrication of the denture if the clinician prefers to pickup the Denture Attachment Housings chairside. TheProcessing Spacer creates a recess in the denture toallow for the Denture Attachment Housing to be seatedwithout any interference with the surrounding dentureacrylic.11B11A-11BRemove the Black Processing Insert fromthe Denture Attachment Housing using the removal(OUT) end of the Retention Insert Tool. When using theremoval end of the Tool, engage the Processing Insertand tip the Tool towards the distal or mesial of theintaglio surface until the Insert completely disengagesfrom the Denture Attachment Housing.Use the metal slot feature on the back end of the Toolto remove the Insert from the tip of the Tool. This canbe accomplished by sliding the Insert into the openingof the metal slot and using an upward motion toremove the Insert.13

During try-in, start with the lowestretentive option. When using the insertion end of theTool, engage the Retention Insert from a slight angleand upright the Tool while pressing down.1313DELIVERYPlace the denture in the mouth and press down toengage the Retention Inserts on the R-Tx Abutments.Verify the occlusion. Instruct the patient on how toremove and insert the denture. If the retention is notsatisfactory, remove the Retention Inserts and replacewith the next level of retention. Please refer to theRetention Inserts section of the Components Referenceon page 2 for available levels of retention. Instruct thepatient on proper home care maintenance and requiredrecall visits.14

MAINTENANCE OF THE LOCATOR R-Tx ABUTMENTReline of Implant-Retained Tissue Supported Overdentures1A1B1A-1B Remove the Retention Inserts from thedenture using the removal (OUT) end of the RetentionInsert Tool. When using the removal end of the Tool,engage the Retention Inserts and tip the Tool towardsthe distal or mesial of the intaglio surface until the insertcompletely disengages from the Denture AttachmentHousing.Use the metal slot feature on the back end of the Toolto remove the Insert from the tip of the Tool. This canbe accomplished by sliding the Insert into the openingof the metal slot and using an upward motion to removethe Insert.22Using a trephine drill, remove the existing DentureAttachment Housings from the overdenture.34A153Place a white Block-Out Spacer around eachAbutment and press it down to the tissue. Snap a newDenture Attachment Housing with a pre-loaded BlackProcessing Insert onto each Abutment, pressing downfirmly. Try in the overdenture to ensure each recessis large enough to accommodate the new DentureAttachment Housing with no interference with theacrylic.4B4A-4B Apply impression adhesive to the intagliosurface of the overdenture and apply a medium bodyimpression material. Take an impression and have thepatient close into light centric occlusion. Remove theoverdenture and verify that all the Denture AttachmentHousings are picked up in the impression.

MAINTENANCE OF THE LOCATOR R-Tx ABUTMENTReline of Implant-Retained Tissue Supported Overdentures (continued)5Seat the appropriate diameter LOCATOR R-TxAnalog into each Denture Attachment Housing andpour a model.66 Verify that the Analogs are secure in the BlackProcessing Inserts and pour a master cast.7857Mount the cast with the denture in a reline jig.8 Separate the reline jig and remove the impressionmaterial from the overdenture. Seat the overdenture andthe Denture Attachment Housings back on the Analogsand reline.Remove the overdenture from the reline jig, finishand polish.16

Verifythe occlusion. If the retention is not satisfactory, removethe Retention Inserts and replace with the next level ofretention. Please refer to the Retention Insert sectionof the Components Reference on page 2 for availablelevels of retention. Instruct the patient on proper homecare maintenance and required recall visits.17

IMPORTANT INFORMATION ABOUT THELOCATOR R-Tx REMOVABLE ATTACHMENT SYSTEMINDICATIONSThe LOCATOR R-Tx Attachment System is designedfor use with overdentures or partial dentures, retainedin whole or in part, by endosseous implants in themandible or maxilla.CONTRAINDICATIONSNot appropriate where a totally rigid connection isrequired. Use of a single implant with divergenceof greater than 30 degrees from vertical is notrecommended. Not appropriate to be used for a fixedrestoration.CAUTIONFederal (USA) law restricts this device to the sale byor on the order of a licensed dentist.SINGLE-USE DEVICESThe LOCATOR R-Tx Attachment System components,with the exception of the Retention Insert Tool, aresingle-use devices.Single-Use Devices must not be reused or resterilized.LOCATOR R-TX RETENTION INSERT TOOLDual-sided tool for easy insertion and removal of theLOCATOR R-Tx Retention Insert. Metal slot feature onthe back of the Tool allows the clinician to disengagea Retention Insert.The Tool is designed for multiple uses. If the Toolbecomes worn or damaged, obtain a replacementTool.When using the removal end (OUT) of the Tool,engage the Processing Insert and tip the Tool towardsthe distal or mesial of the intaglio surface until theInsert completely disengages from the DentureAttachment Housing.Use the metal slot feature on the back end of the Toolto remove the Insert from the tip of the Tool. This canbe accomplished by sliding the Insert into the openingof the metal slot and using an upward motion toremove the Insert.When using the insertion (IN) end of the Tool, engagethe Retention Insert from a slight angle and uprightthe Insert Tool while pressing down.CLEANING Disassemble the instruments(i.e. Retention Insert Tool) Soak the instruments in enzymatic cleaning solution(mixed according to manufacturer’s instructions) bycompletely submerging them for 20 minutes. Scrubthe components using a soft-bristled, nylon brushuntil all soil has been removed. Remove the instruments from the enzymatic cleaningsolution and rinse in tap water for a minimum of 3minutes. Make sure to thoroughly flush internal holes/crevices of the instruments that have difficult to reachareas. If additional cleaning is needed, place instrumentsin an ultrasonic cleaner (with enzymatic cleaningsolution prepared according to manufacturer’sinstructions) making sure that they are completelysubmerged, and sonicate for 10 minutes. Remove the instruments from the ultrasonic cleanerand rinse for 3 minutes making sure to thoroughlyflush cleaning solution out of the holes/crevices and/or difficult to reach areas. Remove excess moisture from the instruments with aclean, absorbent, non-shedding wipe.STERILIZATIONWrap the components using a wrap that is FDAcleared for the indicated cycles. All components andinstruments are supplied NON-STERILE. Titaniumabutments and instruments may be sterilized byAutoclave sterilization using the following parameters:Gravity Pre-VacuumAutoclaveAutoclaveTemperature132 C132 CExposure Time15 minutes4 minutesDry Time30 minutes20 minutesIn order to ensure that the Inserts are sterilized/disinfected (all microorganisms including Clostridiumsporogenes and Bacillus subtilis spores are eliminated),the Inserts must be soaked for a minimum of 3 hours ina liquid sterilant at room temperature.NOTE: An FDA approved liquid chemical sterilant forcritical devices that are heat-sensitive and incompatiblewith sterilization methods such as steam and gas/vapor/plasma low temperature processes may beused following the manufacturer’s directions for thesterilization of the device.18

OVERDENTURE INSERTION, REMOVAL, ANDCLEANINGGUIDELINES For The Clinician And PatientTo reduce wear on LOCATOR R-Tx Abutments, it iscritical that clinicians and patients perform routinemaintenance on both the LOCATOR R-Tx Abutment,the Denture Attachment Housing and the RetentionInsert. It is also important that patients understandthe proper overdenture maintenance that should beperformed at home to guard against retention loss ofthe Retention Insert within the Denture AttachmentHousing. The following are guidelines to consider:INSERTING AND REMOVING ANOVERDENTURETo insert the overdenture, the patient should ensurehe/she can feel that it is properly positioned abovethe LOCATOR R-Tx Abutments prior to applyingpressure. The patient should use both hands andsimultaneously press down on each side to firmlyseat the overdenture into place.The patient should avoid biting the overdenture intoplace as this force will result in improper wear ofthe LOCATOR R-Tx Abutment and may affect thelongevity of the prosthesis.The patient should remove the overdenture byplacing one finger under the left edge and one underthe right edge of the overdenture and pull one sideupward at a time. Once the overdenture is removed athorough cleaning is recommended.CLEANING AN OVERDENTUREMaintaining proper hygiene is vital to the success ofan overdenture, helping it last longer and functionproperly. Similar to natural teeth, dental plaque willalso form on the surface of an overdenture. If theplaque is not removed it will continue to accumulate.It is for this reason that the overdenture shouldbe taken out for cleaning daily. Patients shouldfollow these two simple steps daily for cleaning anoverdenture.1Fill a washing basin with warm water to preventfracture of the overdenture. Apply detergent onto asoft bristle toothbrush and thoroughly clean everysurface of the overdenture.If a denture cleaning solution such as Polident or Efferdent is used, it is recommended that thedenture be soaked for fifteen minutes or less.NOTE: Excessive soaking could negatively impactthe Retention Inserts’ performance.192 Brush the LOCATOR R-Tx Abutments with a softbristle toothbrush paying attention to the dualretentive bumps. The undercut areas must always beclean. Dental floss may also be used in the undercutregions of the Abutments for additional cleaning.NOTE: Aggressive brushing of the Abutment andbrushing with a hard bristle toothbrush will increasethe wear of the Abutment. Use a soft bristletoothbrush and visit the dentist for regular inspectionand maintenance of the Abutments, Retention Insertsand Attachment Housings.ADDITIONAL NOTES OF CAUTIONFailure of the patient to follow oral hygiene protocolsand appropriately care for the overdenture may alsoresult in inflamed tissue around the implant, leading tothe development of peri-implantitis. Throughout time,peri-implantitis may cause the implant to becomemobile and fail. Please ask patients to consider thefollowing when caring for their overdentures: Avoid using abrasive toothpaste to clean theoverdenture. The coarse particles in the toothpastemay scratch the surfaces of the overdenture,enhancing the potential for plaque accumulation. Chewing tobacco may get caught in the RetentionInserts and scratch the Abutments, considerablyreducing the life of the Abutments, retentivefeatures of the Retention Inserts and ultimatelymay affect the dental implants. Do not soak the overdenture in bleach or anyother products not designed for use with denturecleaning as these can harm the retentive feature ofthe Retention Insert, which may ultimately causeadditional wear on the Abutment. Refrain from picking at the Abutments withtoothpicks or other foreign objects. Refrain from eating without the overdenture inplace as food will scratch the Abutment and mayresult in failure of the dental implant. Oral rinse such as Listerine mouthwash can beused safely without any negative effect on theAbutments or Retention Inserts. Do not wash the overdenture in the dishwasher.
