STATEMENT OF PURPOSEThis document provides direction to all Subcontractors* workingfor Lockheed Martin Space Systems Company (LMSSC) atKennedy Space Center (KSC), Florida.This document supplements the numerous Federal ((i.e.Occupational Safety and Health Administration (OSHA),National Fire Protection Act (NFPA), etc.), State ((i.e. FloridaDepartment of Environmental Protection (FDEP), etc.) and Local(Kennedy Space Center (KSC), United States Air Force (USAF),etc.) safety and health and environmental laws, regulations andpermit requirements incumbent upon LMSSC activities andshould not be considered a substitute for those requirements.Subcontractors are at all times required to maintain fullcompliance with those laws, regulations, and permitrequirements applicable to the performance of their work.This includes permits issued directly to NASA/Kennedy SpaceCenter that effect LMSSC. Copies of these permits can beobtained from LMSSC when applicable.LMSSC Environmental, Safety and Health Department has theauthority to immediately shut down any operation which is notcompliant with applicable safety and health and environmentallaws, regulations and permit requirements or which endangerspersonnel and/or property. When possible, shutdown would beordered through LMSSC Administration (herein known as theLMSSC Point of Contact or POC).Under extremecircumstances involving immediate danger to human health, theenvironment, or flight hardware/critical systems, a shutdownmay be made by LMSSC Environmental, Safety and Healthpersonnel without consultation.In addition, any personobserving an act or operation that poses an imminent danger topersonnel or property can utilize the “stop” command.Immediately after using the “stop” command, notify asupervisor, lead, and LMSSC POC or ESH personnel.Anyshutdown of Subcontractor operations due to non-compliancewill be at the Subcontractor’s expense.Subcontractor ESH Handbook1Revision BASIC, June 28, 2000

GENERAL REQUIREMENTSAll Subcontractor employees shall remain vigilant at all timeswhile on the job site. In addition to rules and regulationspresented in this booklet, all Subcontractors are required tocomply with applicable OSHA, NFPA, FDEP, EPA, or otherapplicable Federal, State, or local regulations and theequipment manufacturer’s recommended use and operatinginstructions.Subcontractors working for LMSSC at Kennedy Space Center(KSC) must adhere to the following general standards ofconduct. Violations or lack of timely correction of identifieddeficiencies will result in removal from the job.ENVIRONMENTAL, SAFETY & HEALTH (ESH) PLANAll Subcontractors working for LMSSC shall provide a job-andsite-specific, Environmental, Safety & Health (ESH) Plan. Thisplan shall include (at a minimum) but not be limited to, contactnumbers for on-site and off-site management personnel, personand number responsible for on-site ESH compliance, briefdescription of work to be performed and schedule/shiftinformation.MAXIMUM WORK TIME (MWT)MWT requirements are intended to ensure that employees donot work excessive hours which can compromise personnelsafety: operational safety or mission/construction success.MWT limits apply to all personnel in critical and non-criticalpositions. Prior to any employee working in excess of 12 consecutivehours, concurrence from his/her supervisor who in turnshall obtain approval from LMSSC POC and ESH must beattained. Personnel shall not work in excess of 7 consecutive dayswithout at least one full day off.Subcontractor ESH Handbook4Revision 5/18/2010

Personnel shall not work more than 60 hours during awork week.(7 day period).Personnel shall not work more than 2,500 hours during arolling 12-month period.Prior to any employee working in excess of 240 hoursduring any four consecutive weeks, concurrence fromhis/her supervisor who in turn shall obtain approval fromLMSSC POC and ESH must be attained.SMOKINGSmoking or use of any tobacco products is only allowed indesignated areas at KSC and not on LMSSC premises. Smokingis not permitted within 50 feet of areas whereflammable/hazardous materials are being stored or used.MEDICAL/FIRST AIDFor serious and life-threatening accidents/emergencies, ccupational Health Facility, located West of the O&C on thecorner of Second Street and C Avenue in the KSC IndustrialArea. Emergency medical assistance can be obtained by dialing911 on internal KSC phones or by cell phone at 867-7911 (KSCEmergency Services).Off-shift medical care requirements are handled through KSCEmergency Medical Services, which are to be dispatchedthrough the Center-wide 911 system. If non-urgent medicalattention is required, the patient may elect to go to a localhospital or other facility (i.e. Fire Station or KSC VisitorsCenter) to receive medical attention, or may elect to report tothe KSC Medical Facility (OHF) the next day. All injuries needto be reported as soon as possible to your Supervisor and tothe LMSSC POC.Accidents involving blood or Other Potentially InfectiousMaterials (OPIMs) must be cleaned up by personnel trained inSubcontractor ESH Handbook5Revision 5/18/2010

exposure control methods. Contact LMSSC ESH to arrange forcleanup of accident areas where blood or OPIMs are present.Subcontractor employees working in areas where chemicals arebeing used must have access to eyewash and/or showerfacilities. The Subcontractor shall ensure that their employeesare aware of the closest facility eyewash/shower location.This station shall be less than 10 seconds away from work areand the path to the station shall be clear. If an eyewashstation or shower is not immediately accessible for emergencyuse within the work area, the Subcontractor shall providetemporary stations as necessary.WORK SITE CONTROLSubcontractors shall coordinate their efforts on a noninterference basis with LMSSC activities. Control of specificwork areas (including placement of appropriate signs andbarricades) shall be the responsibility of the Subcontractor andshall be acceptable to NASA-KSC, LMSSC and other NASAprime contractors.The Subcontractor is responsible forproviding work plans/schedules (including definition of the meansto be used to control Subcontractor work areas) to LMSSC forinclusion into master schedules.Definition of OperationsCritical Operations shall include, but limited to the following: Any critical lift (lift of flight hardware or lifting overflight hardware) Propellant transfer operations High Pressure Gas operations Vehicle or vehicle component pressurization Personnel entry into tanks or vehicle Ordnance installation Ordnance checkout and electrical connect/disconnect Vehicle erection Vehicle de-erectionSubcontractor ESH Handbook6Revision 5/18/2010

High acoustic energy level activitiesStructural, material stress or strength testsInitial gaseous pressurization and evacuation/ventingof vehicle propellant tanks/pressure vessels/GN2 flowsOperations involving high voltage or ionizing radiation(X-Rays, range of high frequency high energyelectromagnetic wavesContractor/Site Activation work (construction)Cryogenic operations (cold flows & transfers)Contractor/Site Deactivation WorkRF TestingElectrical testing (on flight hardware, ground supportequipment, tooling, etc.)Battery Hazardous material/waste shipments/handlingEtc.Non-critical operations shall include all operations that are notconsidered critical or otherwise.OPERATIONAL CLEARS (HAZARDOUS OPERATIONS)Due to the nature of some activities at KSC, there will betimes when work areas will be inaccessible due to hazardousoperations. While this includes launch days, there are alsoseveral pre-operational and preparatory activities that requiredownwind (or radius) clears to prevent potential exposure totoxic propellants or blast areas.Staying in touch with the LMSSC POC will help keep youinformed of these operational clears, many of which areplanned well in advance.However, sometimes hazardousoperations and resulting clears cannot be accurately predictedin advance since they depend on factors such as weatherconditions (wind direction/speed, atmospheric inversion, etc.)and integration into the master schedule. interruptions may be imposed with very little notice andSubcontractors should maintain awareness of activities going onaround them so that their work areas are readily securable inthe event of these situations.On occasion, Subcontractor activities may also requirecoordination and clear zones. Work such as X-ray inspection,high pressure operations, certain electrical operations, etc.rely on timely notification and integration of these activitiesinto the schedule.Additional safety and/or securityarrangements may be necessary to accommodate theseactivities and as much notice as possible should be given by theSubcontractor.Quiet HoursThe O&C also serves as the primary sleeping quarters for theAstronauts and certain periods of time prior to launch andlanding require no “loud noises” to be made within the IndustrialOperations Zone (IOZ). These times are announced severalweeks in advance of a launch/landing and shall be strictlyadhered too. Operations taking place during this period shallbe approved by LMSSC POC prior starting the work.HOUSEKEEPINGAll Subcontractor work areas shall be maintained in an orderlymanner, with housekeeping being an on-going task, completed asthe work progresses. At no time shall the Subcontractor blockexit pathways, emergency egress routes, pathways or roadwaysfrom buildings/work areas without prior coordination withLMSSC and ESH. In addition, areas marked on the floor or byother visual barriers shall be strictly adhered to in accordanceto the IOZ 6S Implementation Plan. No storage of placing ofother material in these designated areas is permitted unlessspecifically authorized by the IOZ 6S Implementation Plan andLMSSC ESH.Subcontractor ESH Handbook8Revision 5/18/2010

SITING TEMPORARY FACILITIES/EQUIPMENT/UTILITIESPrior to establishing temporary lay down areas, mobilization ofproject trailers, or establishment of temporary utility services(power, communications, water/sewer, etc.), proper notificationmust be provided and approval must be obtained throughLMSSC POC, NASA-KSC, and other NASA prime contractors.Contact LMSSC POC with details of the proposed action(s) todetermine applicable requirements.RECORDKEEPING AND REPORTINGRetention of records/documents (including certifications, etc.)required by applicable Federal, State and local requirementsare the responsibility of the Subcontractor and shall be madeavailable to LMSSC for inspection and submittal upon request.WEATHER HAZARDSThere are two types of lightning watches/warnings.A Phase I Lightning Watch is issued to provide advance warningof the approach of lightning-producing storms.They aretypically issued 30 minutes prior to the arrival of lightning.During a Phase I Lightning Watch “Critical Operations” withinthe IOZ Building (inside the building envelop) on-going work cancontinue as planned.Depending on the work area, differentactions are required during Phase I Lightning Watches. Forareas outside the IOZ, personnel shall make necessarypreparations to seek shelter if a Phase II Lightning Warning isannounced.A Phase II Lightning Warning is issued when lightning isimminent or occurring in the 5 nautical mile boundary of thespecified lightning alert area.If warranted, Phase IILightning Warnings may be issued by the NASA KSC/WeatherSquadron without issuance of a Phase I Lightning Watch.Generally speaking, all personnel shall seek shelter uponissuance of a Phase II Lightning Warning. No critical operationcan begin during a Phase II Lightning Warning. If a criticalSubcontractor ESH Handbook9Revision 5/18/2010

operation has not yet begun while in a Phase I LightningWatch, NASA KSC/Weather (853-8484) shall be contacted todetermine if a Phase II Lightning Warning is predicted in theforeseeable future.If a Phase II Lighting Warning ispredicted to occur within the period of time it would take tocomplete the critical operation then starting the operation isprohibited. If a Phase II Lighting Warning is announced duringa critical operation then the operation shall be taken to thenext safest stopping point and held until the Phase II Warningis terminated and is not predicted to reoccur until the proposedcritical operation can be completed. LMSSC Systems Safetyand ESH shall approve all operations that can occur during aPhase II Lightning Warning.Other weather watches and warnings are useful in preventingpersonnel exposure to hazardous weather situations such ashigh winds, hail, and tornadoes.Subcontractor personnelshould remain aware of the weather conditions in their workareas and respond to aural warnings as they are issued.HURRICANE REQUIREMENTSDuring hurricane season, Subcontractors are responsible forhurricane preparations in their work areas; these preparationstaking precedence over all other work 48 hours prior toprojected landfall of a hurricane (Hurricane Condition[HURCON] III).See the current revision of LMSSCEmergency Preparedness Plan (i.e. Hurricane Plan is embeddedwithin this Plan) for details of Subcontractor responsibilities.[REST OF PAGE INTENTIONALLY LEFT BLANK]Subcontractor ESH Handbook10Revision 5/18/2010

GENERAL (CONTINGENCY) REQUIREMENTSSometimes, despite all the safety, health and environmentalprecautions taken to prevent incidents and accidents fromhappening, things go wrong. In the event “things go wrong”(e.g., spill of hazardous materials, including fuels, chemicals,etc., injury to personnel or damage to flight hardware, groundsupport equipment, or other systems, or even simply an “out ofthe ordinary” circumstance that could lead to potentiallyhazardous situations), immediately safe the work area andcontact LMSSC POC and ESH. Ensure the incident scene is notdisturbed, All injuries occurring while performing contractedwork on or at the work site shall be reported immediately toLMSSC POC and ESH. If it is not possible to safe the areawithout endangering workers or those around the area,immediately evacuate the area and contact emergencyauthorities (911 or 867-7911) and LMSSC POC and ESH.EMERGENCY NOTIFICATION AND RESPONSE Facility Fire Alarms consist of loud, continuous bells andringing as well as a visible strobe. When you hear orsee the facility fire alarm, evacuate to the closest exitand report to your marshalling area, 200 feet upwindfrom the facility. Area Warnings consist of a warbler sound and indicatethere is danger in the area. When you hear an areawarning, evacuate the area and report to the marshallingarea. Tornado Warnings are a 3to 5 minute steady siren towarn those outside to seek shelter immediately in asubstantial building. Corridors in the interior hallwaysoffer the best protection.Stay clear of exteriorwindows. The Emergency Alert System (EAS) is a buzzer-typenoise on television or radio. All personnel shall listenand follow emergency instructions.Subcontractor ESH Handbook11Revision 5/18/2010

The Public Address System (PA) is used to announcegeneral information, weather advisories, toxicreleases and or other emergencies.EMERGENCY INFORMATION Flashing Lights in facilities indicate a hazardouscondition or at least one which indicates cautionshall be taken. A flashing amber light indicates that hazardousoperations are underway. Only personnel essentialto the operation should be present. Do not enter. A red flashing light indicates an emergency situationor area that shall not be entered. A blue flashing light indicates that radio frequency(RF) waves are being transmitted and non-essentialpersonnel should not approach. A white flashing strobe on the Fire Alarm indicates anemergency situation requiring all personnel toimmediately evacuate the facility.FIRE/EXPLOSIONMeans of egress shall be continuously maintained free of allobstructions or impediments to full instant use in the case offire or other emergency at all times during constructionactivities. No equipment, furnishings, decorations, or otherobjects shall obstruct exits, pathway to emergency exits, orblock visibility thereof to an emergency exit.Any door,passage, or stairway that is neither an exit nor a way of exitaccess shall be posted as “NO EXIT” or “NOT AN EXIT”.In the event of a fire or explosion in/near a Subcontractorwork area, all personnel should evacuate the hazardous area,providing notification to all other personnel in the area, asappropriate (e.g., verbal, pulling fire alarm, sounding audiblesignal, etc.). Always use the stairs when evacuating, neverutilize the elevators.Subcontractor ESH Handbook12Revision 5/18/2010

After evacuation to a safe location contact the NASA-KSCFire Department at 911 on internal KSC phones or by cellphone at 867-0911 and provide details of the incident. Afterthe emergency notification is made, immediately contact theLMSSC POC.A written report, providing details of the incident, must besubmitted to the LMSSC POC within 24 hours. The reportshould include details including the nature and severity of theincident, witnesses, cause(s), if known, and corrective actionstaken/planned.SPILL REPORTING/RESPONSE (INCLUDES “NEAR MISSES”)Subcontractors shall immediately report all releases ofhazardous materials or wastes to the LMSSC POC and to ESH.For releases which can be cleaned up by Subcontractorpersonnel (within their capability and training), LMSSC ESHmay direct the Subcontractor to complete the cleanup. At thetime of notification, the Subcontractor shall provide thefollowing information: This is an Emergency SpillYour name and phone numberExtent of injuries, fire or explosionSpecific location of spillName of the substance releasedQuantity released in volume (gallon), or surface areacoveredIs spill contained?Worst case credible quantity of material that could bereleasedPotential risk to human health or the environment, ifknownCause of the incident (if known)Cleanup actions underwaySubcontractor ESH Handbook13Revision 5/18/2010

In the event that the hazardous material release exceeds thecapability and training of the work crew (e.g., over 5 gallons ofliquid, any amount of mercury, any amount of hypergolicpropellants), all personnel should clear the area in the vicinityof the release. The Subcontractor should alert all personnelaround the area of the occurrence and, if possible, keep thearea clear until the designated emergency response teamarrives. All personnel should avoid contact with or breathingthe released materials.A written report, providing details of the incident, must besubmitted to the LMSSC POC within 24 hours. The reportshould include details including the nature and severity of theincident, witnesses, cause(s), if known, and corrective actionstaken.WORKPLACE VIOLENCEPhysical acts of violence, threats, harassment, intimidation orother disruptive behavior is strictly prohibited on LMSSC orKSC premises.Persons engaging in such activity will beremoved from the premises and may be subject to disciplinaryaction and criminal penalties. Report all incidences of this typeof behavior by calling 911 (867-7911 on a cell phone) foremergency scenarios or 867-2121 for non-emergency scenariosas well as LMSSC POC. If you witness this type of behavior,do not attempt to handle it yourself. Avoid confrontation andcall for help.ACCIDENT/ILLNESS/INJURY REPORTING(INCLUDES “NEAR MISSES”)In the event of an accident, injury or occupational illnessrelated to a Subcontractor’s work activity, the Subcontractorshall immediately secure the accident/injury scene and notifythe LMSSC POC and ESH. This reporting also includes “nearmiss” incidents, which are defined as unintended events whichhad the potential to cause personal injury, adverse impact toSubcontractor ESH Handbook14Revision 5/18/2010

the environment, or significant damage or loss to product,hardware, ancillary equipment, or facility but did not.A written report, providing details of the incident, must besubmitted to the LMSSC POC within 24 hours. The reportshould include details including the nature and severity of theincident, witnesses, cause(s), if known, and corrective actionstaken/planned.LOCATION-SPECIFIC REQUIREMENTSCertain locations at KSC are designated as “restricted” or“controlled” due to security requirements, hazards associatedwith the area(s), etc. Examples of “restricted” or “controlled”areas include: Operations and Checkout (O&C) FacilityLaunch Complexes (LC) 39A, 39B, (KSC)Vehicle Assembly Building (VAB)Space Station Processing Building (SSPF)Orbiter Processing Facility (OPF)In order to work in these locations, certain additionalrequirements may apply. Attached to this handbook are factsheets for the Operations and Checkout Building.SITE-SPECIFIC SAFETY BRIEFINGDue to the unique hazards present in many areas of KSC,LMSSC ESH will conduct Pre-Work Briefings to educateSubcontractor personnel.The Pre-Work Briefing will bemandatory for work conducted at the O&C or other areas atthe discretion of LMSSC. Details of the briefing will include: Locations of propellant storage areas, their hazards andcharacteristics; Propellant area entry requirements (if applicable); Evacuation routes/emergency procedures;Subcontractor ESH Handbook15Revision 5/18/2010

Aural Warning system, sirens, status lights;Fire alarms and evacuation routes/procedures;Meteorological warnings/procedures;Contact names/numbers;Control of work areas;Locations of smoking areas, rest rooms, water fountains;and PPE/equipment requirements. All attendees at the Pre-Work Briefing will be required to signan attendance roster. The Subcontractor will be responsiblefor ensuring that briefings are obtained and understood by anyadditional personnel arriving at the work site after the initialPre-Work Briefing and providing documentation of this trainingto LMSSC POC upon request.[REST OF PAGE INTENTIONALLY LEFT BLANK]Subcontractor ESH Handbook16Revision 5/18/2010

LOCATION-SPECIFIC (CONTINGENT) REQUIREMENTSSubcontractors required to work on or near work stations whileflight hardware is present may also be subject to additionalrequirements. The potential for damage to the vehicleand/or catastrophic damage/injuries associated with an accidentinvolving flight hardware makes it necessary to impose specificrestrictions and requirements on Subcontractors.Subcontractor efforts within a 25-foot radius of flighthardware shall be subject to the requirements outlined below:Work within 10 feet of flight hardware– The Subcontractorshall provide a Flight Hardware/Vehicle Protection Plan forreview and prior approval by LMSSCC which details protectivemeasures to be taken (such as tool tethering).Means ofcontrolling electrical power and pneumatic pressure systems(including required interruptions in service) shall be described indetail in the plan. Work within clean room areas will requireSubcontractor personnel to wear appropriate protectiveclothing, as directed.Welding or flame cutting within 50 feet of flight hardware –Welding or flame cutting within a 50-foot radius of flighthardware requires prior LMSSC approval.
