INCUMBENT:REPORTS TO: Manager, Housing andCommunity DevelopmentPROGRAM: HousingAPPROVED BY: Director, Programs andPlanningDATE APPROVED/REVISED: October 2020HOURS: 35 hours/weekPOSITION SUMMARY:Reporting to the Manager, Community Development and Housing, the Coordinator, Housing Case Managementis responsible for the clinical oversight of community-based housing support programs and the activities of theHousing Case Management team.The Coordinator, Housing Case Management will be required to work week days as well as evenings or weekendsas required.ESSENTIAL JOB RESPONSIBILITIES: Program Coordinationo Oversee and direct the activities of the Housing Case Management programs.o Develop and maintain program standards, procedures and outcomes for urban and rural programs.o Oversee and manage reporting requirements for active partnerships.o Review clinical letters, documentation and care plans. Provide feedback and guidance as required.o Assist staff with service recipient issues and escalate to the Manager as required.o Review and update program tools, including forms, templates and care plans, as required.o Manage referrals to the program to minimize wait times for service.o Review program outcomes, performance metrics and statistics.o Assist with coordinating annual unit visits to rent supplement recipients.o Work collaboratively with community partners to identify community needs and implement supportprograms that improve housing outcomes for individuals.o Submit regular program reports to Manager.
