IntroductionThe Bridge Breast Network (BBN) plays a critical role in the fight against breast cancer byproviding access to breast health services and information in a timely manner to women (and men)who would otherwise have no recourse and would suffer in silence. The BBN provides lowincome, uninsured and under-insured North Texans with access to early detection, diagnostic andtreatment services for breast cancer. Our case management team and extensive network of medicaland community partners work hand-in-hand to provide a continuum of care through all stages ofthe breast cancer journey. The BBN provides access to the following services:Early Detection Screening Mammograms (limited service area) Diagnostic Mammogram Sonograms BiopsiesTreatment Surgery Medical Oncology (chemotherapy) Radiation Oncology One year follow-upThis report will provide a closer look at who we are, what we do, who we serve, and how weachieve our mission. If you have questions about our program, please call 214-821-3820 or emailinfo@bridgebreast.org. If you have comments that relate specifically to the 2014-15 AnnualReport, please email terry@bridgebreast.org.3

Bridging the Gap - Our PresidentDear Friends,2015 marked the 23rd anniversary of TheBridge Breast Network. Since its founding in1992 by Dr. Sally Knox, the organization hasprovided over 150,000 North Texans withbreast cancer education, diagnostic andtreatment services. The Bridge Breast Networksaves the lives of uninsured and underinsuredwomen by bridging the gap between a breastcancer diagnosis and necessary life savingtreatment. One in eight women will be diagnosed withbreast cancer in her lifetime. Those women can survive if they have access to treatment.Unfortunately, we at The Bridge Breast Network know that many are not so fortunate and diefrom the disease solely because they could not afford diagnostic services like mammograms tocatch the disease in its early stages nor the life-saving treatment once it is diagnosed. Last year withyour help, we provided 1,667 women with access to breast health services, and life-saving treatmentto 131 women. We could not have accomplished this without you. We could not have saved these 131 lives. Ourdonors, supporters and medical partners are heroes. Thank you for helping us to build a bridgefrom diagnosis to treatment and together we are saving lives.Jessica Hoffmann, Board President4

What We Do The Bridge Breast Network serves individualsliving in North Texas, where funding andproviders are available. During the intakeprocess, our patient navigators determine ifeach potential client meets our guidelines andif the program can assist them. If a potentialclient qualifies for Medicaid, a case managerassists with the application process and willnavigate them to a provider.Patientnavigators evaluate Dallas and Tarrant Countyresidents to determine if they qualify forassistance at the appropriate county hospitaland assist with access transition whenpossible. If the potential client does notqualify for assistance through state, federal orother funding sources, and if BBN fundingpermits, they are treated through the BBN.The BBN evaluates services on a case-by-casebasis depending on funding for a specificcounty and the availability of BBN providers, and our financial resources. The BBN plays a critical role in the fightagainst breast cancer. Our efforts to reducemortality and the incidents of late-stagedisease are achieved through our network of300 medical providers and 150 communitypartners who work hand-in-hand with ourstaff to provide a continuum of care. TheBBN program includes breast and healtheducation to encourage healthy lifestylechoices, good nutrition, exercise, earlydetection strategies and clinical follow-up toreduce the incidents of late-stage disease forbreast cancer and other chronic and/orpreventable diseases. Patient navigation andaccess to breast health services are necessaryto increase survival. Our early detectionpatient navigators and malignant casemanagers schedule appointments, assemblethe medical team, order labs and schedulemedical procedures, such as mammograms, biopsies, surgery, medical and radiationoncology. The BBN also provides emotionalsupport, connects individuals to communitysupport groups and other resources, providessurvivorship services such as mastectomygarments, prosthesis and lymphademasupplies; but most importantly, we serves as abridge of hope during the breast cancer journey. Our StaffTerry Wilson-GrayErika CuellarElizabeth GuzmanExecutive DirectorMalignant Case ManagerIntake Coordinator./Patient NavigatorDr. Maria GonzalezGina RobinsonBrenda LucioProgram ManagerMalignant Case ManagerPatient NavigatorDonella MoranCarla GurrusquietaMalignant Patient NavigatorSteven Washington,Staff AccountantMedical Office SpecialistTamara SandersDevelopment Assistant8
