CASE STUDYTufts University School of Dental MedicineIn 2013, Tufts University School of Dental Medicine(TUSDM) implemented ExamSoft at the privateKey Findingsuniversity in Boston, Massachusetts, becoming thefirst college on campus to adopt ExamSoft. As theCOVID-19 pandemic forced institutions to implementTUSDM is a long-timeExamSoft client in theirmission to advanceonline education, the dental school quickly transitionedto remote proctoring with great success and plans tocarry it forward in future semesters.COVID-19 pushed theschool to begin remotelearning with ExamMonitorand ExamID.OVERVIEWdental education.Tufts University School of Dental Medicine(TUSDM) has been part of Tufts University formore than 150 years, making it one of the firstschools of dentistry in the United States. It isalso one of the largest schools of dentistryin the country with approximately 400 newstudents enrolling in its graduate and post-ExamSoft has helpedgraduate programs each year.TUSDM achieve examsecurity goals, and usesreports to strengtheninstruction and studentsuccess rates.TUSDM began using ExamSoft in 2013. Dr.Nadeem Karimbux, the school’s current dean, who was then associate dean for academicaffairs and editor for the Journal of Dental

Education, proposed delivering exams via ExamSoft. As part of itsmission, TUSDM seeks to advance dental education, integratingtechnology in a meaningful way and creating a world-class learningenvironment. ExamSoft was an excellent fit — a high-tech solutionwith the potential to advance the quality of education, board exampassing rates, and exam security. The School of Dental Medicine was the first school at the universityto adopt ExamSoft. Since then, three other Tufts programs haveadopted ExamSoft. When TUSDM initially started using ExamSoft, finalexams had different types of questions — multiple choice, open-ended, if/then statements, etc. Startingin January 2020, all final exams were transitioned to a format consistent to the one proposed by JointCommission on National Board Examination — entirely multiple choice and with integrated clinical cases.Having to re-write large portions of content was not easy, but has paid dividends for students sitting forboard exams.

Along with the effort to improve board exam preparedness, TUSDMstarted to take a deep dive on the use of psychometrics, integratingbest practices for teaching. The team used point biserials to identifyvariances in performance to discover opportunities to improvequestions and overall curriculum assessment. ExamSoft reportsprovide statistical data points for each exam item so that faculty canclearly see the performance of each item among high- and lowperforming students and make adjustments to strengthen the qualityof instruction and/or exams. In addition to leveraging the psychometric reports, TUSDM providesstudents with Strengths & Opportunities reports to boost studentadvancement. Students can review exam results almost instantly andnot only get their score but also see how they performed in specificareas so that they know which topics to focus on in the future.“The use ofpsychometrics has reallychanged our practicesand our profession.Evidence-based dentistryis something that is veryimportant, so we madethat parallel with our“The Strengths & Opportunities reporting feature hasteaching. EXAMMONITOR DURING COVID-19While ExamSoft has delivered a host of benefits to TUSDM, the biggest benefit came whenthe COVID-19 pandemic forced students to learn and test remotely. TUSDM had already beenconsidering exam proctoring tools to help manage exam administration in large classrooms;COVID-19 accelerated the decision-making process. It was generally accepted by faculty that a technology solution would be necessary as the schoolshifted to remote learning and exam-taking. But there was also hesitancy — a natural skepticismabout how this would work. Nevertheless, adopting ExamID and ExamMonitor were logicalextensions of TUSDM’s ExamSoft usage.ExamID confirms the students’ identity prior to allowing access to an assessment. ExamMonitoris a tool for remote exam proctoring that offers a combination of A.I. and human review. Whenfaculty can’t be there in person, or when there are too few faculty members to proctor a largescale exam, ExamMonitor continuously observes exam-takers with video and audio throughoutthe exam. The team started the adoption process with legal issues in mind. Requiring students to takeexams with their sound and movement being monitored would require consent. The legal teamdrafted terms that students agreed to before beginning each exam.A select group of faculty members agreed to pilot ExamMonitor. Leadership communicatedwith a group of students who had undergrad experience using remote proctoring — and a highdegree of comfort — with remote proctoring to “test drive” the tool and report out.

One of the biggest challenges TUSDM faced was the use of scratchpaper. Working out problems on scratch paper had been commonpractice for TUSDM students. With ExamMonitor turned on,students looking away from their screens (perceived to be lookingat resources outside of the test) sometimes resulted in thosestudents being flagged. After the first few exams of the emergencyremote learning of Spring, the school decided to stop permittingscratch paper. However, a recent release of Examplify addressesthis issue by allowing a notes section – digital scratch paper.Tahir Hamza, a second-year master’s student and teachingassistant at TUSDM, was one of the students to take his final examfor the Spring semester remotely through ExamSoft. He enjoyedthe transition to remote learning though he did initially have someconcerns about technology.“You do have in the back of your mind about whether your laptop“ExamSoft makes theprocess of conductingexams remotelypossible and does itin a smooth way.”— Tahir Hamza,Graduate Studentwill work,” Tahir said. “But I never had any issues. I was also a littleconcerned about generating my picture through ExamID becausemy baseline image was from two weeks before the exam, and Igrew some facial hair in that time, but it was not a problem.”Even with short notice, implementing ExamID and ExamMonitorwent smoothly. The Tufts team felt like it helped them achieve examsecurity goals and plans to use it on all final exams going forward.“One thing I’ve really appreciated about ExamSoft is howthey work to foster a community spirit. We’ve learned somuch from other dental schools, and conversely, shareour best practices at every turn. One really helpful thingwas setting up playground courses. At Tufts, we createda playground course where students and residentshave all the resources in terms of the technologiesthat we’ll use in the program. With this, students know

exactly what they need to know and have a solid resource for troubleshooting. In this space, we’re focused solely on the functionality thatis most relevant to our students so they don’t have to wade throughthe whole universe of manuals and Q&As. This has been tremendouslyhelpful in training new students.”— Dr. Irina Dragan, Director of Faculty Education & Instructional DevelopmentLooking at the EvidenceTufts University School of Dental Medicine always encourages its students to focus on the patient andevidence-based dentistry. They are taught the importance of looking at all the evidence in making patienttreatment decisions to provide the best possible service to patients. Leadership at Tufts feels like ExamSofthelps them practice what they preach. It enables faculty and students to see all the evidence—to see whatthey understand or don’t. They are able to see if their exams are appropriately challenging, balanced, andcomprehensive.
